Radiographic evaluation provides essential information regarding the diagnosis and treatment of musculoskeletal disorders. We evaluated the ability of hip specialists to reliably identify important radiographic features and to make a diagnosis based on plain radiographs alone. Five hip specialists and one fellow performed a blinded radiographic review of 25 control hips, 25 hips with developmental dysplasia (DDH), and 27 with femoroacetabular impingement (FAI). On two separate occasions, readers assessed acetabular version, inclination and depth, position of the femoral head center, head spher

BACKGROUND: Inherited predisposing risk factors for deep venous thrombosis (DVT) have been clearly identified in Caucasians, but there are fewer reports evaluating these factors in the Japanese. This study was undertaken to assess the predisposing risk factors for DVT following elective total hip arthroplasty (THA) in Japanese patients.
METHODS: We studied 60 patients who underwent THA. The study group consisted of 30 patients who developed DVT diagnosed by duplex sonography examination; the 30 patients who did not develop DVT served as a control group. They were matched in age, sex distribut

Lesions of the acetabular labrum have been suspected to be one factor responsible for failures of Chiari osteotomy. We undertook a prospective investigation to adress this question. Twenty-six adult patients (mean age 34.5 years) with 26 dysplastic hips were enrolled consecutively. All the hips studied showed arthritic changes. The labrum was inspected by arthroscopy during surgery (19 shelf acetabuloplasties and 7 Chiari procedures). All hips were followed for a minimum of 12 years (12-14 years). Sixteen dysplastic hips (62%) were found to have labrum tears. Computed tomography (CT)-arthrogra

INTRODUCTION: Different pelvic osteotomies and various shelf procedures are used for the operative treatment of hip dysplasia. Slotted acetabular augmentation (SAA) is a well-established technique for the treatment of children and adolescents with hip dysplasia. It has not been widely accepted for treating hip dysplasia in adults although good outcomes have been reported with other augmentation techniques in adults.
MATERIALS AND METHODS: Since 1997, SAA has been used for the prevention of hip arthrosis in 14 dysplastic hips in 12 female patients. The median age at operation was 38.5 (17-42)

OBJECTIVE: To evaluate acetabular morphology and the prevalence of dysplasia in a Saudi population, and compare it with others in different geographical areas.
METHODS: The study took place at King Fahd University Hospital, Al-Khobar, Kingdom of Saudi Arabia from May 2005 to April 2006. We recruited radiographs of 104 patients of 40-88 years of age. We digitally measured the center edge angle (CEA) of both hips.
RESULTS: Of the 208 hips examined, 3 hips (1.44%) had mild-to-moderate acetabular dysplasia but there was no case with severe dysplasia (CEA<or=20 degree).
CONCLUSION: The study sho

Osteoarthritis (OA) presupposes the interaction of systemic and/or local factors. In hip joint OA, congenital or developmental malformation is believed to constitute an individual risk factor for premature degeneration. Hip dysplasia (HD) is such a malformation. The radiological and epidemiological studies had several aims: To critically evaluate the radiological source material of the Copenhagen Heart Study: The Osteoarthritis Substudy, consisting of 4,151 standardized, weight bearing pelvic radiographs recorded 1991-1994. To qualify or disqualify the radiological source material for further

The present work deals with a series of 28 patients (a total of 31 hips) suffering from coxarthrosis in dysplasia of the hip. All patients had undergone arthroplasty where an autograft of cortico-cancellous bone was taken from the head of the patientís femur. In 21 of these hips the prosthetic components were not cemented while the other 10 cases were. The patients (average age 57 years 5 months) underwent radiography and were clinically evaluated using the Harris Hip Score. The average follow-up was 6 years and 7 months. In the 10 hips which had been cemented, the average pre-operative Harris

OBJECTIVES: We evaluated the types and the results of surgical treatment performed for congenital scoliosis.
METHODS: Forty-one patients (26 females, 15 males; mean age 12.8 years) with congenital scoliosis were included. The patients were classified according to the Winter's system. Transpedicular hemiepiphysiodesis, "egg shell" procedure, and anterior hemiarthrodesis and posterior convex fusion were performed in infantile (n=1) and juvenile (n=8) patients. In the adolescent (n=32) group, posterior in situ fusion was performed for rigid curves (n=15), posterior fusion after correction with p

The aim of this study was to assess the effects of any anti-adhesion barrier gel used after operative hysteroscopy for treating infertility associated with uterine cavity abnormalities. Gynecologists might use any barrier gel following operative hysteroscopy in infertile women for decreasing de novo adhesion formation; the use of any barrier gel is associated with less severe de novo adhesions and lower mean adhesion scores. Nevertheless, infertile women should be counseled that there is at the present no evidence for higher live birth or pregnancy rates. There is a lack of data for the outcom

Retroversion of the acetabular dome has been associated with hip pain and osteoarthritis in several studies. Nevertheless, this acetabular alignment received little attention when studying the radiological outcome of pelvic osteotomies in childhood. We therefore reviewed the charts and X-rays of 73 patients with congenital dislocations, who underwent 97 pelvic osteotomies in childhood. We focussed our attention on anterior overcoverage or retroversion of the acetabular dome, respectively. Eighty-six Salter and 11 LeCoeur osteotomies were performed on patients with a mean age of 4.8 years. The

We compared 28 total hip arthroplasties done in dysplastic hips after previous Chiari osteotomy (group I) with a well-matched control group of 50 primary procedures (group II) done during the same period at an average follow-up of 5 years (range, 25-199 months). Group I required significantly less acetabular augmentation, had significantly shorter operative times, had less intraoperative blood loss, and had fewer complications than group II. There was no significant difference between the 2 groups in terms of clinical or radiographic outcome. Total hip arthroplasty after a successful Chiari os

By using a mathematical model of the adult human hip in the static one-legged stance position of the body, the forces acting on the hip, peak stress in the hip joint and other relevant radiographic and biomechanical parameters were assessed. The aims were to examine if the peak stress in dysplastic hips is higher than in normal hips and to find out which biomechanical parameters contribute significantly to higher peak stress. The average normalized peak stress in dysplastic hips (7.1 kPa/N) was markedly higher (to approximately 100%) than the average normalized peak stress in normal hips (3.5

We evaluated the results of 27 acetabular reconstructions in 21 patients with secondary osteoarthritis resulting from congenital dysplasia of the hip in which the acetabular bone defects were restored with impacted morcellized bone-grafts in combination with a cemented cup. At an average follow-up of 7 years, 7 months (range, 5-15 years), 2 hips were revised. One cup was revised after 27 months for sciatic nerve problems; the other hip was revised for aseptic loosening of the cup at 12 years' follow-up. The cumulative survival of the acetabular reconstruction was 96.3% at 5- and 10-year interv

We designed 2 new types of proximally coated stems (the FMS and FMS-anatomic) based on the endosteal geometry of femora with congenital dislocation or dysplastic hip. The FMS was symmetric while the FMS-anatomic was asymmetric. We compared the proximal fit and fill to the femoral canal, contact stress, relative motion, and load transfer to the femur of 5 stems (FMS, FMS-anatomic, Omnifit, Omniflex, and individual stem) using three-dimensional computer simulation and finite element analysis. The FMS and FMS-anatomic showed a significantly greater fit and fill than conventional stems. The disper

We evaluated the results of rotational acetabular osteotomy (RAO) for the treatment of dysplastic hips with end-stage osteoarthrosis. Sixteen patients, aged 15-45 years at the time of surgery, were reviewed at a mean follow-up of 8 years (range 3-17 years). Remodeling of the hip joint occurred in half of the patients, with significant clinical improvement. A subsequent total hip arthroplasty, however, was done within 2 years after RAO in two other patients who had had large bone cysts in the femoral head and acetabulum. We suggest that RAO may be the procedure of choice for selected young pati

We implanted 51 Metal-Cancellous Cementless Lübeck (MCCL) prostheses into 45 patients with dysplastic hips and followed 49 hips (96.1%) for five to nine years. One had needed revision for stem fracture and one for infection; the clinical outcome of the other 47 hips was assessed using the Merle d'Aubigné and Postel hip score. All hips were either excellent (63%) or good (37%). Three patients (6%) had mild thigh pain at six months, but this had settled within two years. Serial radiographs showed stable fixation with bone ingrowth in all hips, with increased density of the cancellous bone in con

Hip dysplasia, a congenital and developmental deformity characterized by malorientation and a reduction of contact area between the femur and acetabulum, is the most common cause of osteoarthritis of the hip. According to current estimates, dysplasia accounts for nearly 76% of all cases of osteoarthritis, and many who are affected require a total hip replacement before the age of 50. It is theorized that in the poorly oriented and deformed pelvis, a reduction in contact area leads to an increase in contact pressure during normal activities. Currently, clinicians attempt to reposition the joint

We describe a "new" mild malformation of the phalanx, which we call the "angel-shaped phalanx" (ASP) because of its resemblance to the little angels used for the decoration of Christmas trees. A particular middle phalangeal type of ASPs is found in a distinct variety of multiple epiphyseal dysplasia with marked retardation of bone age and severe coxarthrosis in adult life, previously reported as "hereditary peripheral dysostosis" [Bachman, 1967: Proc R Soc Med 60:21-22; Giedion, 1969: Fortschr Rontgenstr 110:507-524]. However, these authors overlooked the unique configuration of the middle pha

The Balgrist socket, one of the first cementless and non-threaded press-fit acetabular replacements, ensures primary stable fixation. Tapered design and self-locking fixation of the Balgrist socket achieve initial stability not only in osteoarthritis secondary to congenital subluxation but also in acetabular bone stock damage due to other etiologies. The non-threaded design of the socket permits easier femoral head bone grafting for reconstruction of the acetabular rim.
