The longitudinal relationship between physical capacity and self-perceived physical functioning after total hip and knee arthroplasty.

OBJECTIVE: To examine longitudinal within-person effects between capacity and self-perceived performance during postoperative physiotherapy in patients with total hip and knee arthroplasty (THA/TKA). METHODS: We performed a secondary analysis of a cluster randomized controlled trial on physiotherapy after THA and TKA using random intercept cross lagged panel models. Capacity was assessed with the 40-m fast-paced walk test (40 m FPWT) and the 30-s chair-stand test (30-s CST), and self-perceived performance with the Oxford Hip/Knee Score functioning subscale (OHS/OKS) and the Hip disability/Kne

Risk factors for long-term analgesic prescription after hip or knee arthroplasty for osteoarthritis: results of a multivariable regression analysis based on health insurance claims data from 137,875 patients with postoperative inpatient rehabilitation.

BACKGROUND: Continued analgesic prescription after elective total hip arthroplasty (THA) or total knee arthroplasty (TKA) indicates persistent pain and increases the risk of adverse drug reactions, particularly in older patients or those with chronic conditions. The aim of this study was to identify independent risk factors for long-term analgesic prescription in patients who have undergone arthroplasty followed by inpatient orthopaedic rehabilitation. METHODS: The study is based on nationwide claims data from Germany's largest statutory health insurance provider (AOK) for the period from 202

Ceramic resurfacing: postoperative magnetic resonance imaging and clinical outcomes.

AIMS: Hip resurfacing arthroplasty offers a femoral bone preserving procedure that allows high function activities. Understanding what happens to the bone inside the head component in resurfacing has always been limited by metal artefact. The development of ceramic resurfacing has allowed much better visualisation into well-functioning resurfacings. Our aim was to investigate postoperative MRI imaging of ceramic hip resurfacing. METHODS: All participants underwent ceramic hip resurfacing using the ReCerf implant (MatOrtho Ltd). All procedures were performed by a single surgeon through an exte

Higher Area Deprivation Index Is Associated With Greater Practice-initiated Perioperative Communication Workload in Patients With Primary Total Joint Arthroplasty.

BACKGROUND: Although deficits in social determinants of health (SDOH) have been previously associated with adverse clinical outcomes after primary THA and TKA, their role in the perioperative communication workload remains poorly characterized. Even though it remains essential to appropriately identify and address modifiable SDOH before a procedure, orthopaedic practices must also have the resources to handle the coordination of care effectively. Understanding how deficiencies in SDOH can impact communication workload would help support effective resource planning and equitable patient engagem

Long-term outcomes of the modified Dunn procedure in moderate and severe slipped capital femoral epiphysis: a prospective case series with 7-year follow-up.

INTRODUCTION: Slipped capital femoral epiphysis (SCFE) is the most common hip disorder in adolescents and may lead to femoroacetabular impingement, early osteoarthritis, and long-term functional disability if inadequately treated. While in situ pinning remains the standard treatment for mild slips, it fails to correct the deformity in moderate and severe cases, potentially predisposing to degenerative changes. The modified Dunn procedure (MDP) was developed to restore proximal femoral anatomy through surgical hip dislocation while preserving vascular supply. The aim of the study is to evaluate

A Cemented Total Hip Arthroplasty Without Osteotomy for Severe Femoral Deformity in Fibrous Dysplasia: A Case Report.

A 76-year-old woman with fibrous dysplasia (FD) of the femur presented with severe deformity and secondary osteoarthritis. Total hip arthroplasty (THA) was performed using a cemented stem without femoral osteotomy, following thorough 3D preoperative planning. No intraoperative complications occurred. At the three-year follow-up, gait had significantly improved, and no loosening or fracture was observed. Cemented THA without osteotomy is a viable and less invasive option for elderly patients with FD-associated femoral deformity. This approach offers favorable implant positioning and minimizes t

Radiological stability of a quadrangular uncemented femoral component compared with its shortened version in primary total hip arthroplasty : a prospective randomized controlled trial.

AIMS: Cementless femoral fixation is widely used in total hip arthroplasty (THA), with increasing interest in shorter femoral component designs. These femoral components aim to preserve proximal bone, promote physiological load transfer, and facilitate minimally invasive approaches. Among these, 'shortened' femoral components, derived from conventional stems and retaining a diaphyseal extension, may offer advantages in terms of implant positioning. However, clinical evidence regarding their mechanical reliability remains limited. This study aimed to compare the two-year radiological stability

Periacetabular osteotomy of the hip: an 8-year follow-up of 96 consecutive cases.

INTRODUCTION: A periacetabular osteotomy (PAO) is a joint-preserving surgical option for treatment of acetabular dysplasia. The procedure aims to prevent, or at least delay, the development of osteoarthritis, and subsequent need for total hip arthroplasty (THA). The conversion rate to THA differs widely in the literature, but most of the studies have few patients, and the follow-up time is often short for THA as an endpoint. The aim of this study was to evaluate the long-term outcome after PAO surgery with the rate of conversion to THA as the primary outcome. MATERIALS AND METHODS: Patients ≥1

Unrestricted kinematic alignment for total knee arthroplasty : a mid- to long-term follow-up non-designer single surgeon study with survival analysis.

AIMS: Kinematic alignment (KA) offers a personalized approach by restoring each patient's pre-arthritic joint lines. Unrestricted KA (urKA) removes alignment boundaries defined by restricted KA concept, but concerns regarding reproducibility and clinical success persist. This study evaluates mid- to long-term survivorship and functional outcomes of urKA total knee arthroplasty (urKA-TKA) performed using a manual surgical technique by a single surgeon, with a minimum follow-up of three years. METHODS: We prospectively reviewed 229 consecutive urKA-TKAs performed between March 2014 and October
