Markers of joint tissue turnover in joint fluids from hips with osteonecrosis of the femoral head.

Osteonecrosis of the femoral head often results in secondary osteoarthritis of the hip joint; however, the pathologic processes underlying the destruction of articular cartilage are not fully understood. Molecular markers in the hip joint fluids were measured to examine the changes in turnover of cartilage and other joint tissues. Marker data were related to clinical, radiological, and histopathological changes in the articular cartilage of the hip. Forty-five patients (median age: 43 years) were studied. The median time between the onset of symptoms and sampling of hip synovial fluid was 6 mo

[Arthrosis--histology and pathogenetic approaches].

Osteoarthitis is the most common joint disease affecting over 60% of the elderly population. It is characterized by the progressive erosion of articular cartilage leading to incapacity of movement. In the great majority of instances, osteoarthritis appears insidiously, without apparent initiating cause. This primary form is usually oligoarticular involving hip, knee, cervical vertebrae, interphalangeal joints of the fingers or tarsometatarsal joints of the feet. In case of an underlying systemic disease or local injury the cartilage destruction is considered as secondary osteoarthritis. The pa

[Imaging of chronic hip pain in adults].

Adult hip pathologies are mainly represented by the degenerative disease, so called "osteoarthrosis, or more precisely coxarthrosis". The means of imaging are exposed, according to their specific value: X Rays (measurement of the characteristic angles of the adult hip), Arthrography, CT Scanner, Arthro-CT Scanner, MRI, Bone Scintigraphy, Ultrasonography. Clinical findings differentiate a mechanical syndrome and an inflammatory syndrome. The coxarthrosis is the most frequent, under two forms: primary (idiopathic) coxarthrosis and secondary coxarthrosis. Primary (idiopathic) coxarthrosis has a l

High placement of an acetabular component inserted without cement in a revision total hip arthroplasty. Results after a mean of ten years.

BACKGROUND: Revision of an acetabular component in a patient who has severe periacetabular bone loss is a complex problem, particularly when there is not enough bone stock to allow placement of an acetabular component near the normal anatomical hip center. A valuable option for revision in such a situation is placement of a hemispherical shell, fixed with screws and without cement, against the superior margin of the acetabular defect. The resulting hip center is more proximal than that seen following a typical primary total hip replacement. METHODS: Forty-six hips in forty-four patients were

Autosomal dominant hypoparathyroidism associated with short stature and premature osteoarthritis.

Familial hypoparathyroidism is an unusual and genetically heterogeneous group of disorders that may be isolated or may be associated with congenital or acquired abnormalities in other organs or glands. We have evaluated a family with a novel syndrome of autosomal dominant hypoparathyroidism, short stature, and premature osteoarthritis. A 74-yr-old female (generation I) presented with hypoparathyroidism, a movement disorder secondary to ectopic calcification of the cerebellum and basal ganglia, and a history of knee and hip replacements for osteoarthritis. Two members of generation II and one m

[Incidence of joint-specific risk factors in patients with advanced cox- and gonarthroses in the Ulm Osteoarthrosis Study].

AIM OF STUDY: To determine the prevalence of joint specific risk factors in patients with different patterns of advanced hip and knee osteoarthritis (OA). METHODS: We performed a cross-sectional multicenter study in four orthopaedic hospitals in the southwest of Germany. A detailed medical history (date and nature of trauma, conservative and surgical treatment of congenital or acquired joint disorders known as secondary causes of OA) and radiographic evaluation (sequelae of hip dysplasia, slipped capital femoral epiphysis or other malformations) was obtained in 809 patients with advanced hip

Bipolar arthroplasty for recurrent total hip instability.

Bipolar arthroplasty has been reported as a method for correction of recurrent dislocations of total hip replacements. This retrospective review of six patients with multiple dislocations of total hip replacements treated by conversion to simple bipolar hip arthroplasty confirms a 100% success rate in eliminating hip instability during a follow-up period of 2 1/2 to 5 years. However, given the high rate of postoperative discomfort and abnormal gait associated with this procedure, it should be used only when other revision techniques prove to be unsuitable.

A new technique for morphologic measurement of the femur. Its application for Japanese patients with osteoarthrosis of the hip.

The cementless femoral prostheses developed in Western countries have commonly been used for Japanese patients, but it has been noted that it is difficult to achieve a good fit between the cementless femoral prostheses and some femora. In Japan, there are many patients with secondary osteoarthrosis (OA) of the hip, but Western designed femoral prostheses seem to be mainly for use with primary OA of the hip. To design femoral prostheses better suited to Japanese patients, a new method to study the morphometry of the femur is required--one which can determine the particular features of the femur

The Metal-Cancellous Cementless Lübeck total hip arthroplasty. Five-to-nine-year results.

We implanted 51 Metal-Cancellous Cementless Lübeck (MCCL) prostheses into 45 patients with dysplastic hips and followed 49 hips (96.1%) for five to nine years. One had needed revision for stem fracture and one for infection; the clinical outcome of the other 47 hips was assessed using the Merle d'Aubigné and Postel hip score. All hips were either excellent (63%) or good (37%). Three patients (6%) had mild thigh pain at six months, but this had settled within two years. Serial radiographs showed stable fixation with bone ingrowth in all hips, with increased density of the cancellous bone in con

Diagnostic imaging of osteoarthritis.

Osteoarthritis is a common orthopedic problem in the small animal patient. In most cases, the degenerative changes in the joint are secondary to some predisposing cause. The objective of diagnostic imaging is to define the extent and nature of the degenerative changes and to identify the responsible underlying abnormality. To achieve this, appropriate imaging procedures should be used. Although conventional radiography is the most common method used to evaluate osteoarthritis, there has been an increase in the application of other imaging technologies during the past several years. These modal

Uncemented total arthroplasty in old quiescent infection of the hip.

We reviewed the outcomes of 24 cases (24 patients) of uncemented total hip arthroplasty done to treat secondary osteoarthritis due to quiescent previous infection. There were 16 men and eight women aged from 24 to 78 years (mean, 50 yr). Eight patients had proven or probable tuberculous infection and the other 16 patients had a past history of septic hip. The patients were followed for between 2.5 and 7 years (mean, 4.5 yr). According to the Harris hip rating system, 18 of the patients had excellent or good results. Three patients had a preoperative erythrocyte sedimentation rate of 40 mm/h or

Prevention of secondary coxarthrosis in slipped capital femoral epiphysis: a long-term follow-up study after corrective intertrochanteric osteotomy.

Fifty-one patients with unilateral severe (gliding angles 30 degrees-60 degrees) slipped capital femoral epiphysis (SCFE) treated by intertrochanteric corrective osteotomy were reexamined after 20-29 years (average 24 years) of follow-up; 55% of the patients showed neither radiographic signs of degenerative hip disease nor clinical symptoms, whereas 28% had moderate and 17% had severe osteoarthritis. These results are definitely superior to those reported in other series of patients with comparable slips treated by bed rest or in situ fixation only. Analysis of individual gliding angles and di

Diagnosis of osteoarthritis. Guidelines and current pitfalls.

Diagnostic and classification guidelines for osteoarthritis based on clinical and radiographic signs for the most frequently involved joints (knee, hip and hand) have been developed by the American College of Rheumatology. Excellent, new 'high tech' biochemical and imaging methods and guidelines for clinical evaluation of the condition and its therapy have recently been developed and validated. Accordingly, we have consistent and reliable methods for assessing the established condition, but they provide little help in making an early and correct diagnosis for the different forms of osteoarthri

Extreme variations in racial rates of total hip arthroplasty for primary coxarthrosis: a population-based study in San Francisco.

OBJECTIVES: To compare the incidence of all total hip arthroplasty, and total hip arthroplasty for primary coxarthrosis, among the Asian, black, Hispanic, and white populations living in one locale. METHODS: We identified all San Francisco residents who underwent total hip replacements (THR) in the 17 hospitals for adults within or near San Francisco County during a five year period. Preoperative pelvic radiographs were read without prior knowledge of the gender or race of the subject, to diagnose the specific hip diseases. Age standardised THR incidence by race and gender was determined, as

[Coxofemoral pathology in rheumatology consultation in Togo].

A retrospective study was conducted to determine the relative incidence and characteristics of the different forms of coxofemoral disease encountered in a hospital rheumatology clinic in Lomé (Togo). Hemoglobin electrophoresis was performed in all subjects included in the study. Of 2812 subjects examined over a 44-month period, 80, i.e. 2.8%, presented clinically and radiologically documented coxofemoral disease. Necrosis of the femoral head was the cause of coxofemoral disease in 36 subjects i.e. 45%, and was the most frequent etiology. In these 36 subjects hemoglobin was normal in 5 cases, t

Congenital hip dislocation and pediatric hip disorders.

The pediatric hip changes through development, leading to a challenging evaluation of pain. Common problems noted in children include congenital dislocation of the hip, Legg-Calve-Perthes disease, slipped capital femoral epiphysis, and transient synovitis. Hip disorders are also seen secondary to other childhood diseases, such as spina bifida. Long-term complications of all of these hip disorders can lead to joint deformity and osteoarthritis in adulthood.

Dosing time optimizes sustained-release ketoprofen treatment of osteoarthritis.

A double-blind randomized parallel-group trial was undertaken to evaluate the influence of the dosing time of sustained-release ketoprofen (SRK) on its acceptability and efficacy. The SRK was prescribed for 2 weeks (200 mg once a day) to 117 outpatients with osteoarthritis of the knee and/or hip. One group received SRK in the morning (at 8 a.m.) and the other group in the evening (at 8 p.m.). The principal aim of the trial concerned the acceptability, whereas efficacy was its secondary aim. The principal trial criterion was defined as the number of spontaneous recordings of adverse effects. Re

[An arthroscopic study of coxarthrosis].

Arthroscopy was performed on 110 hips with coxarthrosis secondary to hip dysplasia. The cartilage degeneration was then classified arthroscopically and the findings compared with radiographs. The patients ranged in age from 16 to 52 years old (mean 28.1 years). The cartilage degeneration by arthroscopy was classified as follows; Type 0: normal cartilage, Type I: irregular, Type II: fibrillation (a: membranous type, b: featherlike type, c: edematous type, or d: short type), Type III: erosion, Type IV: eburnation. The cartilage degeneration varied from Type 0 to Type IV corresponding with the st
