Premature greater trochanteric epiphysiodesis secondary to intramedullary femoral rodding.

Five skeletally immature patients developed premature closure of the greater trochanteric physis consequent to placement of an intramedullary rod for primary treatment of a femoral diaphyseal fracture. Each patient developed increased femoral neck valgus as compared with the contralateral hip. To date, however, none of these patients has had any functional disability, although one has developed mild radiographic subluxation. In addition, anatomic specimens demonstrated the likelihood of traversing a portion of the greater trochanteric physis. Other methods of fracture treatment, either operati

Kashin-Beck's disease.

A case of Kashin-Beck's disease is presented. It is an acquired, disabling, polyarthritic, degenerative condition of early onset, sometimes leading to a variable degree of dwarfism. It occurs endemically in certain Asian areas. Treatment is, if possible, preventive. In the established disease the therapy is that of any other form of secondary osteoarthritis. Reconstructive surgery and removal of loose bodies may be needed in severe cases (11). Early detection, facilitated by careful family history taking, is a prime requisite, especially in non-endemic areas (7).

Rapid destructive osteoarthritis: clinical, radiographic, and pathologic features.

Twenty-seven cases of an unusual, poorly recognized destructive hip arthropathy with radiographic findings of rapid severe joint destruction are presented. Radiographic findings mimicked those of other disorders such as septic arthritis, rheumatoid and seronegative arthritis, primary osteonecrosis with secondary osteoarthritis, or neuropathic osteoarthropathy, but none of the patients had clinical, pathologic, or laboratory evidence of these entities. All patients underwent hip arthroplasty, and osteoarthritis was confirmed at pathologic examination. Rapid progression of hip pain and disabilit

Acceptability and efficacy of two associations of paracetamol with a central analgesic (dextropropoxyphene or codeine): comparison in osteoarthritis.

A double-blind randomized parallel group trial was undertaken to compare the acceptability and efficacy of 2 forms of analgesic treatment, DI-Antalvic (Houde Laboratories, Puteaux, France) (30 mg dextropropoxyphene and 400 mg paracetamol per capsule) and Efferalgan-Codeine (UPSA Laboratories, Rueil Malmaison, France) (30 mg codeine and 500 mg paracetamol per tablet) prescribed for 1 week at doses of 6 capsules/day and 6 tablets/day, respectively, in 141 outpatients with active osteoarthritis of the knee or hip. The principal aim of the trial was concerned with acceptability, with efficacy as i

[The Balgrist acetabulum with uncemented endoprosthesis of dysplastic coxarthrosis and other acetabulum defects].

The Balgrist socket, one of the first cementless and non-threaded press-fit acetabular replacements, ensures primary stable fixation. Tapered design and self-locking fixation of the Balgrist socket achieve initial stability not only in osteoarthritis secondary to congenital subluxation but also in acetabular bone stock damage due to other etiologies. The non-threaded design of the socket permits easier femoral head bone grafting for reconstruction of the acetabular rim.

Orthopaedic problems in old dogs.

With advancing years a dog may suffer from a variety of conditions of its musculoskeletal system which adversely affect its ability to exercise and may cause it to be retired from activities in work and sport for which it has been trained. Arthritis is common, and in many cases arises from developmental errors suffered in puppyhood, such as hip dysplasia, osteochondrosis and growth plate disorders. Trauma to joints (ligament ruptures, dislocations and fractures) may also be the precursor of degenerative joint changes later in life. It is important, therefore, for all such conditions to be corr

Roentgenographic assessment of the biologic fixation of porous-surfaced femoral components.

Certain roentgenographic signs have value in predicting the fixation of a cementless femoral component to bone by osseointegration. Other signs have value in predicting the gross stability of a cementless femoral component. The authors have determined the specificity and sensitivity of the signs for osseointegration in cases in which the histologic fixation has been confirmed after implant removal. The authors have also determined the specificity and sensitivity of the signs for gross implant stability in cases in which the stability has been confirmed at reoperation. Statistical methods were

MR and CT appearance of iliopsoas bursal distention secondary to diseased hips.

Occasionally hip joint disease may extend into surrounding structures, including the retroperitoneum, via the iliopsoas bursa. The enlargement of this bursa may present as an inguinal or pelvic mass that may affect other surrounding structures and can result in a multitude of clinical presentations. The two cases presented herein of iliopsoas bursa distention secondary to hip disease demonstrate the excellent specificity of CT and magnetic resonance in differentiating this clinical entity from other causes of groin masses.

Osteoarthrosis and congenital dysplasia of the hip in family members of children who have congenital dysplasia of the hip.

Four hundred and eight siblings, parents, and grandparents of seventy-eight children from the New England area who had congenital dysplasia of the hip were evaluated, by clinical examination and by measurements of the acetabulum on pelvic radiographs, for the signs and sequelae of congenital dysplasia of the hip. Six siblings and four mothers (representing seven of seventy-eight families) had been diagnosed with congenital dysplasia of the hip during childhood. The other ninety-one siblings were asymptomatic and had no radiographic evidence of dysplasia of the hip. In the adults in these famil

Failure of acetabular autogenous grafts in total hip arthroplasty. Increasing incidence: a follow-up note.

We reported previously on the application of an autogenous femoral-head graft to the acetabulum during total hip arthroplasty for compensation of marked osseous deficiency in patients who had arthritis secondary to severe congenital dysplasia or dislocation of the hip. An average of seven years postoperatively, the graft seemed to have been a successful adjunct to the arthroplasty. Five years later, to assess our long-term results, we reviewed the findings in the same forty-six hips (thirty-seven patients) that we had studied previously. An average of 11.8 years after the total replacement and

Epidemiology of hip and knee osteoarthritis.

This review has focused on the prevalence and risk factors associated with knee and hip osteoarthritis. Risk factors for knee osteoarthritis are obesity and major injury, and knee osteoarthritis probably fits into the generalized osteoarthritis diathesis. Repetitive use, such as in jobs requiring heavy labor and knee bending, probably increases the risk of knee osteoarthritis. Hip osteoarthritis is probably frequently secondary to developmental defects. As Rothman (182) has pointed out in discussing causation, this does not necessarily mean that the same factors do not also contribute to causi

Criteria for the classification of osteoarthritis of the knee and hip.

The Osteoarthritis (OA) Criteria Subcommittee of the American Rheumatism Association set out to develop (a) a classification of OA that includes recognised subsets; and (b) subsets of OA identified by a combination of clinical and laboratory features. For the purposes of classification, OA should be specified if of unknown origin (idiopathic, primary) or if related to a known medical condition or event (secondary). Clinical criteria for classification of idiopathic OA of the knee were developed through a multicentre study group involving 130 patients with OA and 107 comparison patients. Compar

[The natural course of osteoarthritis of the hip. Indication of conservative treatment in relation to osteophyte formation at the acetabular rim].

One hundred and eighty-five hips from 152 patients with primary or secondary osteoarthritis were studied in an attempt to assess the degrees of hip pain in contrast to radiological and other clinical findings. In 30.8% and 26.4% of the primary and secondary osteoarthritic hips respectively, hip pain showed some gradual decrease as time elapsed. Pain relief probably in association with osteophyte formation at the craniolateral acetabular rim occurred in 62.5% and 33.3% of the primary and secondary osteoarthritic hips respectively. Significant parameters observed in the primary osteoarthritic ca

Pelvic radiographs of 200 consecutive Japanese Oriental patients who were admitted for hip surgery at the Hospital of Kobe University in Japan were compared with those of 199 consecutive American white patients who were admitted for the same purpose to a New England hospital over a similar four and a half-year period between 1972 and 1976. One hundred and fifty-three Japanese Oriental and 157 American white patients had either primary or secondary osteoarthritis. The remainder had other types of hip pathology, such as avascular necrosis, rheumatoid arthritis, and ankylosing spondylitis. The ra

Conventional total hip arthroplasty for degenerative joint disease in patients between the ages of forty and sixty years.

We analyzed the records of 103 conventional hip arthroplasties in seventy-five active patients who were between the ages of forty and sixty years. After five to ten years of follow-up, an excellent or good clinical result was evident in 90 per cent. The radiographic appearance of the cement-bone interface of the acetabular component was stable in those hips, and no progressive acetabular radiolucency was found in 80 per cent of the hips. Of twenty hips with a complete acetabular radiolucency (Grade III or IV), only three had a secondary revision operation, two in conjunction with a fracture of

[Joint saving operation procedures in idiopathic femur head necrosis].

At first, this report deals with the characteristics of the idiopathic necrosis of the femoral head and the possibilities of operative treatment specially in the sense of saving the hip-joint. Beside old techniques of sanitation of the focus, nowadays the femoral osteotomy, mainly in the sense of a flexion-osteotomy is well to the fore. Hereby, the necrotic area can be turned out of the main loaded part of the hip-joint. We recommend a combination of femoral osteotomy with a ventral capsulotomy and tenotomy. Furthermore, there should be a sanitation of the focus, above all by drilling. Then we

[Insertion of Judet's prosthesis in high-region dislocation of the hip (author's transl)].

Osteoarthrosis of the hip (also known as coxarthrosis) as a result of a high-region dislocation of the hip, presents a difficult task for the orthopaedic surgeon, the solution of which is still under frequent and intense discussion. Total hip replacemnt (hip endoprosthesis) involves technical difficulties which result on the one hand, from the underdeveloped anatomic conditions (underdeveloped primary acetabulum with soft spongiosa on valgus position of the highly sclerosed secondary acetabulum associated with underdeveloped shaft of the femur); on the other hand, a repositioning is difficult

Osteoarthritis of the hip. Epidemiology and clinical role.

Among 6321 patients who had undergone X-ray examination of the colon 4.7 per cent proved to have osteoarthritis of the hip. A mild osteoarthritis was found to be equally common in both sexes, but in all age groups above 60 severe osteoarthritis of the hip was twice a common in women as in men. Osteoarthritis setting in at an early age was secondary to other hip conditions. In 50 per cent of the patients the arthritis was located laterally and in 24 per cent medially. The medially located osteoarthritis was less troublesome than the lateral. According to the present study, only half the patient

Hip dysplasia in the African Bantu.

Dysplasia of the hip is almost unknown in the Bantu peoples of Africa, except in cases of arthrogryposis and in those with partly European ancestry. Evidence is produced to show that the importance of intrinsic genetic factors in hip dysplasia far outweighs that of the extrinsic factors. These are of great importance only if allowed to act on a hip in which genetic factors have already determined dysplasia. Evidence is also produced to contradict the theory that the "piggyback" carrying method used by Bantu mothers is the reason for the immunity from dysplasia of the hip. The absence of "prima

[Changes of articular cartilage in coxarthrosis].

In cases of advanced coxarthrosis indicated for total endoprosthesis [a total of 216 observations made] the most frequent changes included cases of lowering, defibration or even complete abrasion of cartilage in the most exposed segment of the articular head [196x]. Small patches of connective cartilage [133x] were found in the areas of abrasion. There were rare cases of chondrocyte nests as a manifestation of proliferation [25x]. The usual picture of coxarthrosis had exceptions in the form of flatly worn-out surface areas (34X) and pannus (7X). These may have been cases of secondary coxarthro
