Ninety-three knees with patellar instability were repaired after conservative treatment failed. In 34 there was evidence of tibiofemoral incongruity consisting of an articular depression of the medial femoral condyle by the meniscus. In 254 knee joint arthroplasties performed during the period between 1972 and 1977, secondary operations were done in 31 knee joints, a frequency of 12%. In the rheumatoid group the frequency was 6%, in those with osteoarthritis, 18%. Loosening was the indication for reoperation in 14 cases and infection in seven. Ninety-one arthroplasties of the knee joint--all performed by the same orthopaedic surgeon, were followed for from 6 months to 6 years (mean 3.5 years). At the time of the primary operation the mean age of the patients, most of whom had rheumatoid arthritis, was 60 years. 111 patients with amputation of the thigh of the last world war were examined. Besides the complaints in their thigh they suffered mostly from low back pain and osteoarthritis in the knee of the other leg. For a medical report it is important to decide whether the low back pain comes from lumbar disc disease or from secondary scoliosis of the lumbar spine which is caused by the amputation. Secondary changes in the spine and in the other leg can also be expected when a lower limb is heavily injured.

Thirty-seven patients with unicompartmental osteoarthritis were treated by replacement arthroplasty using the Marmor modular prosthesis and each patient was followed for at least two years. Despite rigid preoperative criteria for the selection of cases, the over-all results were definitely inferior to those seen after either bicompartmental or tricompartmental replacement arthroplasty. Although degenerative joint disease is an old and exceedingly common problem, clinical investigators have not reached a consensus regarding the etiology of this disease. Comparative osteological analysis of the knee, hip, shoulder, and elbow joints of 789 individuals from four human skeletal populations (Black and White Americans, Pueblo Indians, and Alaskan Eskimos) indicates that age of onset, frequency, and location of degenerative changes are directly related to the nature and degree of environmentally associated stress, as reflected by the variable life styles of the populations sampled.

The clinical and roentgenological data on 30 patients with unsatisfactory recovery or recurrent symptoms after meniscectomy were analysed. The meniscectomy was regarded as having been the wrong choice of therapy in four patients and as ineffective therapy in 11. In 10 patients other knee lesions dominated postoperatively, in three the recovery was delayed for no obvious reason but the symptoms slowly disappeared, and in the remaining two patients a degenerative osteoarthritis developing some time after the meniscectomy was regarded as the cause of the recurring symptoms. Based on our results with high tibia osteotomy for gonarthrosis in 96 knees, the type of primary deformity was found to significantly influence the patient's subjective assessment of the results. Knees with valgus deformity secondary to changes in other joints are not suitable for correction by HTO. In addition, 193 patients with hip or knee arthrosis were examined retrospectively. Restriction of rotation on physical examination accurately differentiated arthrotic hips from normal ones.
