Remote and In-person Supervised Exercise in Patients with Knee Osteoarthritis (RISE-KOA): study protocol for a non-inferiority randomized controlled trial.

BACKGROUND: Knee osteoarthritis (OA) is a prevalent joint condition resulting in years lived with disability. A first-line treatment recommended by clinical guidelines is the therapeutic exercise to control pain and improve physical function. One possible approach for exercise supervision is telehealth using video calls, as it can be an effective alternative to in-person physical therapy for treating musculoskeletal conditions, expanding community access to physical rehabilitation. In this scenario, this study aims to investigate whether a muscle-strengthening exercise program for the lower li

Can EEG-Neurofeedback Training Enhance Effective Connectivity in People With Chronic Secondary Musculoskeletal Pain? A Secondary Analysis of a Feasibility Randomized Controlled Clinical Trial.

INTRODUCTION: Persistent musculoskeletal pain is associated with altered functional and effective connectivity (EC) between cortical regions involved in pain processing. Especially, disruptions in the infraslow fluctuation (ISF) frequency band can contribute to pain persistence. ISF electroencephalography-neurofeedback (EEG-NF) has emerged as a potential non-invasive neuromodulatory intervention targeting cortical brain regions to restore balance and modulate pain-related pathways. However, limited research explores its effect on EC, a measure of directional information flow critical to pain e

Measurement of Value in Uncomplicated Total Knee Arthroplasty: Patient-Level and Provider-Level Value Analyses of a 1-Year Episode of Care.

BACKGROUND: Patient-level value analysis (PLVA) has been applied to several orthopaedic procedures but has not yet been utilized to assess the value of total knee arthroplasty (TKA). The purpose of this study was to evaluate the 1-year episode of care for TKA with use of PLVA to identify characteristics that influence value at both the patient and surgeon level. METHODS: The institutional patient-reported outcome (PRO) database was queried for all patients who underwent TKA from 2020 to 2022. Patients were excluded on the basis of an index revision procedure, a pathology other than primary os

Factors That Increase the Risk of Prosthetic Joint Infection Within 90 Days After THA and TKA: A Nationwide Population-based Study.

BACKGROUND: Prosthetic joint infection (PJI) after THA and TKA is a serious complication. Although previous studies have identified risk factors for overall and late-onset PJI, limited research has focused specifically on infections occurring within the immediate 90-day postoperative period, a time frame critical for assessing surgical quality and early recovery outcomes. QUESTIONS/PURPOSES: What patient and clinical factors are associated with the development of PJI within 90 days after primary THA or TKA? METHODS: This was a retrospective, controlled study drawn from a longitudinally maint

Relation of vertical ground reaction forces while walking to contralateral structural worsening in adults with unilateral knee osteoarthritis: the Multicenter Osteoarthritis Study.

BACKGROUND: It is unclear why knee osteoarthritis (KOA) progresses from a unilateral to bilateral joint disease. This study aimed to evaluate relations between vertical ground reaction forces (VGRFs) while walking and structural worsening in the unaffected limb of adults with unilateral KOA. METHODS: Discrete metrics were extracted from the unaffected limb VGRF signal while walking at a self-selected speed. Structural worsening of the unaffected knee was evaluated over 2 years with radiographs (primary) and MRIs (secondary). Logistic regression models evaluated associations between VGRF metri

Comparison of Short-Term Outcomes and Survivorship of Three Modular Dual Mobility Implants in Primary Total Hip Surgery.

Background: Total hip arthroplasty (THA) is a common procedure, yet instability and dislocation remain leading causes of revision. Dual mobility (DM) acetabular constructs improve stability, but comparative data across modular DM systems are limited. This study compared the safety and efficacy of three modular DM implants in primary THA, focusing on acetabular revision and functional recovery. Methods: We retrospectively reviewed 963 primary THAs performed from 2016-2024 using three modular DM systems. Patients with revision or bilateral THA, age < 18, or <2 years of follow-up were excluded. O

Does Periacetabular Osteotomy Affect the Load Distribution on the Knee?

BACKGROUND: Periacetabular osteotomy (PAO) treats developmental dysplasia of the hip (DDH) by reducing load on the hip and improving joint function. Untreated DDH affects lower extremity alignment and alters knee morphology, with valgus alignment more pronounced in hip osteoarthritis secondary to DDH. While PAO may influence knee mechanics, its association with subchondral bone density in the tibiofemoral joint remains unclear. QUESTIONS/PURPOSES: (1) To what degree is PAO associated with changes in the distribution of subchondral bone density in the knees of patients with DDH? (2) Is PAO ass

Posterior Tibial Slope Is Associated With the Chondral Wear Pattern of the Medial Tibial Plateau.

BACKGROUND: The influence of the posterior tibial slope on the knee has been extensively studied, particularly in the context of surgical outcomes in sports medicine, such as cruciate ligament reconstruction. From a biomechanical perspective, the posterior tibial slope affects not only knee kinematics but also contact pressure, potentially contributing to the development and progression of degenerative change in the joint over time. However, whether or not there is an association on the chondral wear pattern of the medial tibial plateau remains unclear. QUESTIONS/PURPOSES: (1) Is posterior ti

Early-Onset Degenerative Bone Changes as a Manifestation of Alkaptonuria: A Case Report.

Alkaptonuria is a rare inherited metabolic disorder characterized by progressive pigment deposition in connective tissues, leading to early degenerative joint disease and tendon fragility. Although uncommon, its clinical manifestations may closely resemble more prevalent musculoskeletal conditions, often resulting in delayed diagnosis. We present the case of a 53-year-old woman with long-standing, disabling osteoarticular complaints initially attributed to common degenerative disease, but markedly premature for her age. Due to worsening bilateral gonarthrosis, she underwent left total knee art

Primary Total Knee Arthroplasty in Patients Who Have Neuromuscular Disorders and Genu Recurvatum Using a Rotating-Hinge Implant: A Case Series With a Mean 4-Year Follow-Up.

BACKGROUND: Patients who have neuromuscular disorders and quadriceps muscle weakness may develop knee osteoarthritis along with progressive recurvatum deformity. Due to challenges in ligament balancing and the risk of knee hyperextension recurrence, the use of constrained implants may be required. The objective of this study was to report the mean four year clinical outcomes of patients who had neurological conditions and knee hyperextension and who underwent primary total knee arthroplasty (TKA) with a rotating-hinge implant. METHODS: Patients who had neuromuscular disorders and gonarthrosis

Functional and Clinical Outcomes of Total Knee Arthroplasty: A Prospective Study.

INTRODUCTION: A key to successful patient outcomes following knee procedures like total knee arthroplasty (TKA) is achieving normal knee alignment. Normal mechanical balance in the joints must be restored in order to lessen the failure rate of TKA, and precise assessment of lower extremity alignment and component location is thought to be the primary determinant of a good clinical outcome over the long term. OBJECTIVE: The aim of this study was to assess the early functional and clinical outcomes of TKA by analyzing post-operative mechanical axis and Knee Society Score (KSS) data. The study a

Prediction of postoperative gait speed change after bilateral primary total knee arthroplasty in female patients using a machine learning algorithm.

BACKGROUND: An important aim of total knee arthroplasty is to achieve functional recovery, which includes post-operative increase in walking speed. Therefore, predicting whether a patient will walk faster or slower after surgery is important in TKA, which has not been studied in previous literatures. Who walks faster and who walks slower after TKA? Can we predict these kinds of patients before surgery? HYPOTHESIS: Whether or not a patient walk faster after total knee arthroplasty can be predicted with preoperative characteristics. PATIENTS AND METHODS: In this retrospective cohort study, 128

Moving a skin incision to lateral side of tibial tubercle reduces the risk of persistent skin numbness compared with a regular midline one: A randomized paired study in bilateral total knee arthroplasty.

BACKGROUND: The midline incision during total knee arthroplasty (TKA) is known to damage the infrapatellar branch of the saphenous nerve (IPBSN), leading to lateral flap numbness. The aim was to evaluate the rate of persistent skin numbness (RSN) and area of skin numbness (AON) after a lateral-based skin incision compared with the standard midline incision in bilateral TKA patients. METHODS: Thirty-six patients undergoing bilateral TKA were included and randomly assigned to receive the lateral skin incision (lateral side of the tibial tubercle) on one knee and the standard midline incision on

Patellar resurfacing might not be mandatory in contralateral second total knee replacement even if performed during replacement of the first knee.

INTRODUCTION: The appropriateness or necessity of patellar resurfacing in total knee arthroplasty (TKA) is not clearly established. The objective of the present study was to compare the clinical and functional results of patients with bilateral involvement, operated on in one knee by TKA with patellar resurfacing and in the other knee by TKA without resurfacing. The hypothesis was that there was no difference in results between the two sides. METHOD: This was a retrospective cohort of patients operated on by TKA without patellar resurfacing in a second osteoarthritic knee after contralateral
