Substantial improvement in short-term survivorship in patients receiving robotic arm-assisted medial unicompartmental knee arthroplasty : a retrospective cohort study.

AIMS: Robotic arm-assisted unicompartmental knee arthroplasty (R-UKA) has been shown to improve alignment and accuracy of component placement compared with conventional unicompartmental knee arthroplasty (C-UKA). Despite this, there is conflicting evidence for whether this has an impact on revision rates. METHODS: We performed a retrospective analysis of a prospective database, comparing patients who underwent R-UKA using the Stryker Restoris MCK implant with those who underwent C-UKA using the cemented Oxford Partial Knee implant, between January 2002 and December 2024 at The Prince Charles

Blood flow restriction with different loads versus moderate intensity resistive exercises in patients with knee osteoarthritis: a single-blind randomized controlled trial.

BACKGROUND: Knee osteoarthritis (KOA) is a common condition causing pain, muscle weakness, and functional impairment. In addition, many patients cannot tolerate resistance training due to pain. Blood flow restriction (BFR) has been proposed as a low-load alternative strategy to improve muscle function, but the effectiveness of passive BFR application remains unclear in KOA. Therefore, this study compared passive BFR at 50% and 70% limb occlusion pressure (LOP) with moderate-intensity resistance training (MI-RT) in KOA. METHODS: A three-arm, parallel groups, single-blind randomized controlled

The impact of changing Coronal Plane Alignment of the Knee classification on early functional outcomes after primary robotic-assisted total knee arthroplasty.

AIMS: We aimed to determine the impact of changing a patient's Coronal Plane Alignment of the Knee (CPAK) phenotype on early functional outcomes after robotic-assisted total knee arthroplasty (RA-TKA). METHODS: We conducted a retrospective multicentre cohort study of 297 patients undergoing primary RA-TKA. Standardized pre- and postoperative long-leg radiographs were obtained. Radiological measures included the lateral distal femoral angle (LDFA) and medial proximal tibial angle (MPTA). Pre- and postoperative CPAK classification was derived by determining the arithmetic hip-knee-ankle (HKA) a

Fewer poor joint awareness results with cementless TKA compared with the same-design cemented implant.

BACKGROUND: Comparisons of cemented and cementless TKA have focused on mean outcomes and generally reported equivalence. We hypothesized that a 3D-printed full-titanium cementless TKA and the same-design cemented implant would not differ significantly in average joint awareness but would differ in the proportion of patients with the poorest results, and we compared the two both at the mean and across the outcome distribution. METHODS: Prospective two-cohort study of 109 primary unilateral TKAs (54 cementless, 55 cemented) for end-stage knee osteoarthritis, with complete 24-month follow-up. Ex

Risk factors for long-term analgesic prescription after hip or knee arthroplasty for osteoarthritis: results of a multivariable regression analysis based on health insurance claims data from 137,875 patients with postoperative inpatient rehabilitation.

BACKGROUND: Continued analgesic prescription after elective total hip arthroplasty (THA) or total knee arthroplasty (TKA) indicates persistent pain and increases the risk of adverse drug reactions, particularly in older patients or those with chronic conditions. The aim of this study was to identify independent risk factors for long-term analgesic prescription in patients who have undergone arthroplasty followed by inpatient orthopaedic rehabilitation. METHODS: The study is based on nationwide claims data from Germany's largest statutory health insurance provider (AOK) for the period from 202

Is There a Need for a Central Stem within the Keel of Primary Tibial Baseplates in Total Knee Arthroplasty.

Clinical studies suggest a role for the keel of the tibial component in preventing aseptic loosening in total knee arthroplasty (TKA). To analyze the role of a central stem within the keel, we compared two designs of tibial components for the Triathlon TKA (Stryker): standard keel components that have a central stem and fins (Triathlon universal tibial baseplate) and finned components that have fins alone and lack a central stem (Triathlon primary tibial baseplate). Questions: (1) is there a difference in all-cause revision rate or revision rates for loosening? (2) Is there a difference in rev

Establishing Minimal Clinically Important Difference and Substantial Clinical Benefit Thresholds for Objective Gait Metrics After Total Knee Arthroplasty.

BACKGROUND: Assessment of objective gait recovery in patients who are undergoing total knee arthroplasty (TKA) is becoming increasingly prevalent. However, few studies determined minimal clinically important difference (MCID) or substantial clinical benefit (SCB) thresholds in arthroplasty populations for clinical interpretation. This analysis aimed to calculate these values for various gait metrics following TKA. METHODS: A secondary analysis of a longitudinal observational cohort of patients who underwent primary TKA was performed. Objective gait metrics were collected via wearables and sma

[Effect of tranexamic acid on blood loss control in patients undergoing primary total knee arthroplasty].

Total knee arthroplasty (TKA) is an important surgical method for treating advanced knee osteoarthritis(KOA). It could effectively restore knee joint function and relieve pain. However, perioperative blood loss may cause postoperative anemia, increase the demand for blood transfusion, and thereby affect the prognosis of patients and increase medical burden. Tranexamic acid (TXA), as an anti-fibrinolytic agent, plays a significant role in controlling blood loss during perioperative period of TKA. TXA mainly works by competitively inhibiting the binding of plasminogen to fibrin, thereby preventi

Spectrum of Models for Assessing the Cost Effectiveness of Total Knee Replacement Implants: A Comparison of Discrete-Time Cohort Markov and Continuous-Time Individual-Level Multistate Models.

BACKGROUND AND OBJECTIVE: A primary elective total knee replacement is routinely used for patients with advanced osteoarthritis. Knee implants differ in characteristics (constraint, fixation, mobility), costs, need for revisions and other health outcomes, and so models evaluating their relative cost effectiveness are required to optimise decision making. Economic modelling approaches differ in complexity, the simplest in use being discrete time Markov models (DTMMs). Continuous-time Markov models (CTMMs) can capture transition timing in finer detail, and can more flexibly relax the constant ha

Total and partial knee arthroplasty versus non-surgical interventions of the knee for moderate to severe osteoarthritis.

RATIONALE: Total and partial knee arthroplasty (TKA, PKA) are common treatments for moderate to severe knee osteoarthritis. While effective in symptom management, these surgeries carry potential risks, necessitating careful evaluation of benefits and harms. High-quality evidence comparing TKA/PKA to placebo, sham, or non-surgical interventions is essential to support informed decision-making. OBJECTIVES: To assess the benefits and harms of TKA and PKA for people with moderate to severe knee osteoarthritis compared to placebo, sham (efficacy), or non-surgical interventions for the knee (effect

Patient-reported outcomes are comparable between imageless robotic-assisted and computer-navigated total knee arthroplasty: A retrospective cohort study.

PURPOSE: The benefits of computer-navigated total knee arthroplasty (CN-TKA) are well established, and although early studies have demonstrated some benefits of robotic-assisted total knee arthroplasty (RA-TKA), there is limited evidence on the clinical and patient-reported outcome measures (PROMs) directly compared to CN-TKA. As RA-TKA continues to gain popularity, it is important to compare outcomes with the more established technique to justify its use. This study compares PROMs for imageless RA-TKA versus CN-TKA, and between RA-TKA performed with either ROSA or CORI. METHODS: This was a r
