The Effects of Perioperative Intra-Articular Corticosteroids During Manipulation Under Anesthesia After Total Knee Arthroplasty: A Retrospective Study.

BACKGROUND: Postoperative stiffness after total knee arthroplasty (TKA) is uncommon, but may lead to major knee dysfunction. Manipulation under anesthesia (MUA) is an effective tool for improving postoperative stiffness. We hypothesized that intra-articular corticosteroid administration during MUA better preserves postmanipulation range of motion (ROM) compared to MUA without a glucocorticoid adjunct. METHODS: A retrospective review of 103 MUAs by six arthroplasty surgeons at our institution from 2014 to 2023. Patients were included if they underwent MUA after a primary TKA for osteoarthritis

Lubricin protects against cartilage degeneration following anterior cruciate ligament transection in rats.

Despite the growing prevalence of post-traumatic osteoarthritis (OA), early and effective treatment options to delay surgical intervention remain limited. Lubricin is a naturally occurring glycoprotein produced by articular chondrocytes and synovial fibroblasts that functions as the principal boundary lubricant in joints. Recently, a recombinant lubricin-like glycoprotein (rhLub) with codon optimized mucin domain was reported to have a sustained residence time (> 6 weeks) following injection into the knee joint of healthy rats. To evaluate outcomes of rhLub injection, skeletally mature Sprague

Posterior Horn Meniscus Centroid Position Is Altered Soon After Noncontact ACL Injury in Males and Females.

Anterior cruciate ligament (ACL) injury and subsequent changes in the magnitude and distribution of contact stress about the articular surfaces of the knee are associated with post-traumatic osteoarthritis. Soon after ACL injury, changes in tibial articular cartilage thickness occur that can be explained, in part, by abnormal positioning of the tibia relative to the femur. However, little is known about the effects of ACL injury on the positions of the menisci. The purpose of this case-control study was to determine the effect of ACL injury on posterior horn meniscus centroid (PHMC) position r

Single Intra-Articular Anakinra (IL-1Ra) Versus Betamethasone in Rabbit Post-Traumatic Knee Osteoarthritis: IL-8 Suppression and Chondrocyte Viability.

Post-traumatic osteoarthritis (OA) is driven by a sustained inflammatory response and progressive osteochondral damage. We compared the effects of a single intra-articular injection of anakinra (Interleukin-1 receptor antagonist; IL-1Ra) with those of betamethasone on structural cartilage outcomes and Interleukin-8 (IL-8) immunopositivity in a rabbit model of post-traumatic knee OA, and assessed the cytotoxicity of anakinra in primary human OA chondrocytes. Fifteen male New Zealand White rabbits underwent bilateral anterior cruciate ligament transection and partial medial meniscectomy and were

Decreased Thigh Muscle Cross-Sectional Area Following Anterior Cruciate Ligament Injury Depends on Concomitant Meniscal Injury and Subject Sex.

This study characterized acute changes in thigh muscle cross-sectional area (CSA) in subjects who suffered their first severe knee trauma. Ninety-one males and females who suffered ACL rupture with or without concomitant meniscus injury underwent bilateral mid-thigh axial T1-weighted, fast field echo magnetic resonance imaging early after the index trauma and prior to reconstruction. The extensor and flexor muscle groups were segmented bilaterally, and CSA measured. Within-person injured-to-contralateral normal side differences in CSA (injured minus normal limb) were evaluated. Robotic-assisted total knee arthroplasty in extra-articular post-traumatic deformities: A case series with one-year outcomes.

PurposeExtra-articular femoral or tibial malunion can distort anatomical landmarks, compromise the reliability of conventional alignment guides, and complicate total knee arthroplasty (TKA). Robotic-assisted TKA (RA-TKA) may improve the precision and reproducibility of bone preparation and component positioning in these anatomically challenging cases. This study reports one-year clinical and radiographic outcomes after single-stage RA-TKA performed without corrective osteotomy in patients with post-traumatic extra-articular deformities.MethodsThis retrospective case series included nine patien

When microsurgery is not an option: staged delayed reverse sural artery flap with ischaemic preconditioning for limb salvage in an elderly patient with chronic post-traumatic osteomyelitis-a case report.

Lower extremity reconstruction in elderly patients with post-traumatic osteomyelitis presents a formidable challenge. We report a 70-year-old female with hypertension, dyslipidaemia, and bilateral knee osteoarthritis who sustained a Gustilo-Anderson type IIIB open right ankle fracture with exposed calcaneus following a fall. The course was complicated by chronic osteomyelitis, managed with partial talectomy and total calcanectomy, antibiotic-impregnated cement spacer placement, cement exchange and prolonged culture-directed intravenous antibiotics. Following infection eradication, definitive h

Simultaneous Bilateral Total Knee Arthroplasty in a Post-Traumatic Osteoarthritis Patient: A Case Report and Literature Review.

Osteoarthritis is the leading cause of disability in adults. Trauma is a predisposing factor contributing to the development of osteoarthritis. Total knee arthroplasty (TKA) is a promising treatment for end-stage osteoarthritis of the knee. TKA in post-traumatic osteoarthritis is technically challenging because of the deformity, bone defect, and ligament incompetence.

Enhanced mobility and pain relief of total hip arthroplasty in post-traumatic ankylosing hip: A case report.

INTRODUCTION AND IMPORTANCE: Ankylosed hips pose significant challenges due to severe joint stiffness and deformities. Total hip arthroplasty (THA) is a critical intervention that offers pain relief and improved function, despite the technical difficulties associated with altered biomechanics and preoperative deformities. This abstract highlights the complexities of THA in Ankylosing Spondylitis (AS) patients, emphasizing the need for specialized surgical approaches to optimize outcomes and mitigate the higher risk of complications. CASE PRESENTATION: A 20-year-old male presented with pain an

Limb Underloading in Walking Transmits Less Dynamic Knee Joint Contact Forces after Anterior Cruciate Ligament Reconstruction.

INTRODUCTION: Individuals with anterior cruciate ligament reconstruction (ACLR) often walk with a less dynamic vertical ground reaction force (vGRF), exemplified by a reduced first peak vGRF and elevated midstance vGRF compared with uninjured controls. However, the mechanism by which altered limb loading affects actual tibial plateau contact forces during walking remains unclear. METHODS: Our purpose was to use musculoskeletal simulation to evaluate the effects of first peak vGRF biofeedback on bilateral tibiofemoral contact forces relevant to the development of post-traumatic osteoarthritis

Maisonneuve fracture treated with short leg cast: A case report with 41-month follow-up.

RATIONALE: Maisonneuve fracture is a specific type of severe ankle injury. To our current knowledge, once a Maisonneuve fracture is diagnosed, the surgery is always recommended for fear of sequelae from inaccurate joint reconstruction. However, in this case, we treated a Maisonneuve fracture with a short leg cast, and the 41-month follow-up showed a favorable outcome with no post-traumatic osteoarthritis, chronic pain, and instability. Therefore, this case provides evidence for the feasibility of conservative treatment of Maisonneuve fracture. PATIENT CONCERNS: A female patient in her early t

Arriving at SKINTED (Surgery of the Knee, Injury to the Infrapatellar Branch of the Saphenous Nerve, Traumatic Eczematous Dermatitis): A Case Report.

Surgery of the knee, injury to the infrapatellar branch of the saphenous nerve, traumatic eczematous dermatitis (SKINTED) is a postsurgical localized dermatitis specifically linked to total knee arthroplasty (TKA). It is due to autonomic denervation following surgically inflicted nerve injury. It develops several months to years following a surgical trauma. It is being referred to by various names in the literature. Locoregional immune dysfunction due to lymphatic injury after surgery is the currently accepted theory. It must be distinguished from atopic dermatitis, allergic contact dermatitis

Asymmetric Post-Traumatic Knee Arthritis Is Closely Correlated With Both Severity and Time for Lower Limb Coronal Plane Malalignment.

OBJECTIVE: Mechanical alignment of the lower limbs has been suggested to cause abnormal uneven loading across the compartments at the knee, but its contribution to the initiation and progression of arthritis remains controversial. This study aimed to establish whether malalignment of the lower limb after trauma is associated with worsened arthritis scores in the theoretically overloaded compartment, and if arthritis scores continuously correlate with the degree of malalignment and time with deformity. DESIGN: After screening 1160 X-rays, 60 patients were identified with long-leg radiographs >

Chondral Injury Associated With ACL Injury: Assessing Progressive Chondral Degeneration With Morphologic and Quantitative MRI Techniques.

BACKGROUND: Anterior cruciate ligament (ACL) injuries are associated with a risk of post-traumatic osteoarthritis due to chondral damage. Magnetic resonance imaging (MRI) techniques provide excellent visualization and assessment of cartilage and can detect subtle and early chondral damage. This is often preceding clinical and radiographic post-traumatic osteoarthritis. HYPOTHESIS: Morphologic and quantitative MRI techniques can assess early and progressive degenerative chondral changes after acute ACL injury. STUDY DESIGN: Prospective longitudinal cohort. LEVEL OF EVIDENCE: Level 3. METHOD

PCL retention demonstrates better functional scores and gait patterns in total knee arthroplasty using a medial congruent insert-a prospective study.

PURPOSE: Despite Total Knee Arthroplasty (TKA) being one of the most successful procedures for end stage arthritis, nearly 20% of patients undergoing this procedure remain dissatisfied. Various design options have been introduced to reduce this cohort of patients. One such option has been the introduction of the medial congruent (MC) polyethylene design. This study was undertaken to evaluate outcome measures and gait analysis in patients undergoing bilateral single stage TKA where the posterior cruciate ligament (PCL) was retained or excised in contralateral knees. METHODS: 60 bilateral TKA's

Risk Factors for Periprosthetic Joint Infection after Primary Total Knee Arthroplasty.

Periprosthetic joint infection (PJI) is a major adverse event of primary total knee arthroplasty (TKA) from the patient's perspective, and it is also costly for health care systems. In 2010, the reported incidence of PJI in the first 2 years after TKA was 1.55%, with an incidence of 0.46% between the second and tenth year. In 2022, it has been published that 1.41% of individuals require revision TKA for PJI. The following risk factors have been related to an increased risk of PJI: male sex, younger age, type II diabetes, obesity class II, hypertension, hypoalbuminemia, preoperative nutritional

Do associations with hand OA vary by knee osteoarthritis phenotype? Cross-sectional data from the Multicenter Osteoarthritis Study.

OBJECTIVE: Osteoarthritis (OA) is highly heterogeneous and has both biomechanical and systemic components that may not have the same etiology. We therefore aimed to identify specific knee OA phenotypes that may be more strongly associated with hand OA to refine the criteria used to define multi-joint OA. DESIGN: We assessed data from the Multicenter Osteoarthritis Study (MOST). We ascertained hand OA from bilateral hand photographs; scores for each joint row were summed to yield an aggregate hand OA score. Knee OA was ascertained from bilateral posteroanterior knee radiographs read for Kellgr

Proximal Tibia Bone Stiffness and Strength in HR-pQCT- and QCT-Based Finite Element Models.

Injury to the ACL significantly increases the risk of developing post-traumatic osteoarthritis. Following injury, considerable focus is placed on visualizing soft tissue changes using MRI, but there is less emphasis on the alterations to the underlying bone. It has recently been shown using high-resolution peripheral quantitative computed tomography (HR-pQCT) that significant reductions in bone quality occur in the knee post ACL-injury. Despite the ability of HR-pQCT to show these changes, the availability of scanners and computational time requirements required to assess bone stiffness and st

Effect of knee joint loading on chondrocyte mechano-vulnerability and severity of post-traumatic osteoarthritis induced by ACL-injury in mice.

OBJECTIVE: The objective of this study is to understand the role of altered in vivo mechanical environments in knee joints post anterior cruciate ligament (ACL)-injury in chondrocyte vulnerability against mechanical stimuli and in the progression of post-traumatic osteoarthritis (PT-OA). METHODS: Differential in vivo mechanical environments were induced by unilateral ACL-injury (uni-ACL-I) and bilateral ACL-injury (bi-ACL-I) in 8-week-old female C57BL/6 mice. The gait parameters, the mechano-vulnerability of in situ chondrocytes, Young's moduli of cartilage extracellular matrix (ECM), and the
