Unraveling the Cause of Hemolytic Anemia in the Emergency Department: A Destructive Path.

Hemolytic anemia is a rare but potentially life-threatening condition in the emergency department (ED) that requires timely recognition and early intervention. Hemolytic anemia results from either the intravascular or extravascular breakdown of red blood cells. The most common causes of secondary hemolytic anemia include lymphoproliferative disorders, autoimmune disorders, and infections. This case will demonstrate how an urgent diagnosis of hemolytic anemia can lead to the underlying cause: diffuse large B-cell lymphoma. This case illustrates how laboratory evaluation, early subspecialty invo

Variable hepatic and renal manifestations following grass carp gallbladder ingestion: a three-patient case series from Vietnam.

Fish gallbladder ingestion remains a traditional folk remedy in several Asian countries but may result in severe hepatorenal toxicity. We report three patients admitted to the Poison Control Center, Bach Mai Hospital, Hanoi, Vietnam, following ingestion of grass carp (Ctenopharyngodon idella) gallbladders. A 44-year-old man developed severe hepatocellular injury (peak AST 4442 U/L, ALT 5115 U/L), moderate acute kidney injury (AKI), and prolonged severe hypertension requiring intravenous nicardipine infusion for 88 hr. A 48-year-old man presented with severe AKI (peak creatinine 749 µmol/L) and

S100B in brain-gut-liver crosstalk: from glial activation to multiorgan inflammation.

S100 calcium-binding protein B (S100B) is conventionally viewed as a marker of astrocytic injury, blood-brain barrier (BBB) disruption, and neurological disorders. Emerging evidence places S100B within the broader brain-gut-liver axis of inflammatory signaling. This review proposes that S100B is a glial-derived integrating effector that couples enteric glial activation, barrier dysfunction, hepatic fibrosis, and hepatic encephalopathy (HE) into a multi-organ inflammatory network. Its spatial expression map, release kinetics, alarmin (damage-associated molecular pattern, DAMP) activity, and imm

Short-course prophylaxis with the Toronto HCV protocol for recipients of non-liver solid organ transplants from donors with hepatitis C viraemia: a retrospective, matched cohort study.

BACKGROUND: Organs from donors positive for hepatitis C virus (HCV) nucleic acid testing (NAT-positive) expand the non-liver solid organ donor pool. Standard post-transplantation direct-acting antiviral therapy requires an 8-12-week course, which might add logistical, financial, and pharmacological complexity. We therefore aimed to evaluate long-term follow-up from an initial pilot trial and outcomes after adoption of the short-course Toronto HCV protocol as the institutional standard of care in HCV-negative recipients of organs from HCV viraemic donors. METHODS: This retrospective, matched c

Expression of miR-214 in Pediatric Acute Leukemia: Correlations with Leukemia Subtypes, Familial Predisposition, and Hepatitis B Virus Infection.

BACKGROUND: miR-214 dysregulation has been implicated in various cancers, yet its role in pediatric acute lymphoblastic leukemia (ALL) is not fully understood. This study evaluated miR-214 expression in newly diagnosed pediatric ALL patients and explored associations with leukemia subtypes, family history, and hepatitis B virus (HBV) infection. METHODS: Bone marrow aspirates were collected from 44 pediatric ALL patients (26 B-ALL, 18 T-ALL) and five non-leukemic controls. qRT-PCR quantified miR-214 expression. Clinical data included HBV status and family history of hematologic malignancy. Rel

Evolution of Macrophage Function in Sepsis-Induced Cardiomyopathy: Cellular Networks from Immune Imbalance to Myocardial Injury.

Sepsis-induced cardiomyopathy (SICM) is an acute, often reversible myocardial depression driven by the host response to infection. Cardiac macrophages are central signaling hubs that integrate systemic danger signals and orchestrate myocardial dysfunction. We introduce a "Macrophage Functional Continuum" model, conceptualizing macrophages as dynamic signal integrators undergoing three interconnected phases: sensing and initiation, intrinsic reprogramming and fate determination, and multicellular network execution. Conceptually, these phases represent functional modules that may overlap in time

Case Report: Severe mixed autoimmune hemolytic anemia associated with acute cytomegalovirus infection in an immunocompetent adult.

BACKGROUND: Cytomegalovirus (CMV)-associated hemolytic anemia (HA) is rare in immunocompetent hosts and may present with heterogeneous serologic phenotypes, including direct antiglobulin test (DAT)-negative hemolysis, warm autoimmune hemolytic anemia (AIHA), cold antibody-mediated AIHA, and rarely mixed AIHA. Because mixed AIHA requires evidence of both warm autoantibodies and clinically significant cold autoantibody activity, this phenotype may be underrecognized without comprehensive immunohematologic testing. CASE PRESENTATION: We report a case of CMV-associated HA that was diagnosed as mi

Portal vein thrombosis increases short-term mortality among hospitalized patients with cirrhosis in a global cohort.

BACKGROUND: Portal vein thrombosis (PVT) in cirrhosis is associated with adverse outcomes, but determinants of PVT and its impact on short-term outcomes in hospitalized patients across global regions remain unclear. METHODS: The multinational CLEARED consortium prospectively enrolled adults with cirrhosis admitted non-electively after consent. Patients were followed during hospitalization and 30 days post-discharge. Radiologically diagnosed PVT was analyzed in relation to demography, etiology, clinical features, hospital course, and 30-day outcomes, including mortality. RESULTS: Among 7,721

Potentially fatal hemorrhagic duodenal ulcers caused by cytomegalovirus infection complicating autoimmune hepatitis with acute liver failure, successfully treated with multiple interventional radiology procedures.

Cytomegalovirus (CMV)-associated gastrointestinal disease can precipitate life-threatening hemorrhages; therefore, heightened vigilance is warranted in patients administered immunosuppressive agents. Moreover, acute liver failure compromises cellular immunity by disrupting immune regulatory mechanisms. Herein, we describe a case of duodenal ulcer caused by CMV reactivation during treatment for autoimmune hepatitis (AIH), complicated by acute liver failure. The patient was admitted with acute-onset AIH and steroid pulse therapy was initiated; however, progression to acute liver failure followed

BACKGROUND: Maudsley Family Therapy (MFT), and its manualised version, Family-Based Therapy (FBT), are the only well-established treatment interventions for adolescent anorexia nervosa (AN), with treatment efficacy primarily measured by improvements in eating behaviours and weight restoration. A crucial component of this therapy is an intensive home-based refeeding intervention that requires a substantial commitment from parents for up to one year. While this treatment works to restore weight in a proportion of adolescents, very little is known about its impacts on family distress, relationshi

ClC-7 links PIKfyve inhibition to Rab-dependent LRRK2 activity at lysosomes.

Increased activity of leucine-rich repeat kinase 2 (LRRK2) confers Parkinson's disease risk. LRRK2 dynamically localizes to lysosomal membranes in response to various stresses, yet the mechanisms by which distinct lysosomal perturbations are communicated to LRRK2 remain unclear. Here, we show that inhibition of the lysosomal lipid kinase PIKfyve promotes LRRK2 recruitment and signaling through a pathway that requires the lysosomal chloride/proton antiporter ClC-7. ClC-7 in turn controls the accumulation of multiple Rab GTPases on lysosomes. LRRK2 signaling under these conditions requires its e

Two new species of Capitellidae (Annelida, Polychaeta) from the southern coasts of China.

Senescent cells, which arise due to damage-associated signals, are apoptosis-resistant and can express a pro-inflammatory, tissue-destructive senescence-associated secretory phenotype (SASP). We recently reported that a component of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) surface protein, S1, can amplify the SASP of senescent cultured human cells and that a related mouse β-coronavirus, mouse hepatitis virus (MHV), increases SASP factors and senescent cell burden in infected mice. Here, we show that SARS-CoV-2 induces senescence in human non-senescent cells and exacerba

Corneal Innervation Research at a Crossroads: A Tool-Driven Roadmap for the Future.

IMPORTANCE: Hepatitis D virus (HDV) infection occurs in association with hepatitis B virus (HBV) infection and affects approximately 12 million to 72 million people worldwide. HDV causes more rapid progression to cirrhosis and higher rates of hepatocellular carcinoma than HBV alone or hepatitis C virus. OBSERVATIONS: HDV requires HBV to enter hepatocytes and to assemble and secrete new virions. Acute HDV-HBV coinfection is followed by clearance of both viruses in approximately 95% of people, whereas HDV superinfection in an HBV-infected person results in chronic HDV-HBV infection in more than

Liquid biopsy cell analysis and ex vivo culture as effective methods of therapeutic indexing of metastasis in B[a]P-induced lung cancer mice for an active targeted quercetin therapy.

UNLABELLED: Cytomegalovirus (CMV) is a human herpes virus with a worldwide seroprevalence of 60-100%, mainly known to cause severe life-threatening disease in immunocompromised patients. In immunocompetent hosts (IMCh), CMV causes a self-limiting mononucleosis-like infection, and severe pictures are less recognized. We report a case of a previously healthy 62-year-old woman evaluated in the Internal Medicine outpatient clinic for 3 weeks of progressive fatigue, generalised inflammatory arthralgias, hypogastric discomfort and daily persistent fever. On first examination, paleness and hepatomega

Short Course Therapy With Glecaprevir/Pibrentasvir for Early Hepatitis C Virus Infection: PURGE-C.

BACKGROUND: Shorter treatment courses for early hepatitis C virus (HCV) infection could simplify treatment approaches, particularly in key populations. METHODS: PURGE-C (A5380) was a single-arm, multicenter trial evaluating the treatment of early HCV (primary or reinfection) with 4 weeks of glecaprevir/pibrentasvir (G/P). Early HCV was defined as new detectable HCV RNA or alanine aminotransferase (ALT) elevation within 24 weeks of study entry. The primary endpoint was sustained virologic response (SVR) 12 weeks after prescribed treatment completion (SVR12). Re-treatment outcomes were also col

Understanding the Serologic and Molecular Epidemiology of Hepatitis A Infection in Puducherry and Surrounding Regions of India.

Hepatitis A virus (HAV), traditionally a paediatric illness in endemic regions, is now exhibiting an emerging age shift in symptomatic cases. This study investigates the sero-molecular epidemiology of Hepatitis A in Puducherry and surrounding regions. Between January and November 2024, 513 acute viral hepatitis cases were evaluated for serum IgM anti-HAV. Representative samples underwent sequencing and B-cell epitope analysis. Of the 150 patients (29.23%) tested positive for HAV, children under 12 years (50%) were most affected, with an increasing trend in adult infection (31.33%), while adole
