Image-based robotic-assisted patellofemoral arthroplasty for isolated patellofemoral osteoarthritis: Short-term survivorship, clinical and radiographic outcomes.

INTRODUCTION: Patellofemoral arthroplasty (PFA) is a valuable option for selected patients with isolated patellofemoral osteoarthritis (PFOA), where component positioning and patellar tracking are critical determinants of outcome. Robotic assistance may improve these aspects, but available literature remains limited. This study evaluated early survivorship, complications, and reoperations after image-based robotic-assisted PFA (RA-PFA). Secondary aims were to assess clinical outcomes, return to sport, radiographic patellofemoral parameters, and agreement between planned and final implanted com

Fewer poor joint awareness results with cementless TKA compared with the same-design cemented implant.

BACKGROUND: Comparisons of cemented and cementless TKA have focused on mean outcomes and generally reported equivalence. We hypothesized that a 3D-printed full-titanium cementless TKA and the same-design cemented implant would not differ significantly in average joint awareness but would differ in the proportion of patients with the poorest results, and we compared the two both at the mean and across the outcome distribution. METHODS: Prospective two-cohort study of 109 primary unilateral TKAs (54 cementless, 55 cemented) for end-stage knee osteoarthritis, with complete 24-month follow-up. Ex

From herbal hope to hormonal havoc: chronic ashwagandha use and HPA axis suppression.

SUMMARY: This case report describes a 55-year-old postmenopausal woman who presented with Cushingoid features and progressive weight gain over a year. Evaluation revealed low basal cortisol levels and a suboptimal response to the ACTH stimulation test, indicative of adrenal insufficiency. Further investigation into her medication regimen uncovered her consistent use of approximately 950 mg of ashwagandha daily for over a year, prescribed by a local practitioner for symptomatic relief of bilateral knee osteoarthritis. A review of the existing literature suggested a potential cortisol-suppressin

Ultra-Congruent Versus Cruciate-Retaining Inserts in Bilateral Simultaneous Total Knee Arthroplasty: A Prospective Randomized Controlled Trial.

BACKGROUND: The prosthetic design landscape for total knee arthroplasty (TKA) is extensive. Modified surface geometries aim to replicate native kinematics for superior outcomes, yet evidence remains conflicting regarding optimal design efficacy based on patient-reported and objective metrics. The primary aim of this study was to compare the patient satisfaction between ultra-congruent (UC) and cruciate-retaining (CU) insert designs in bilateral simultaneous TKA, and the secondary aim was to study the parameters validating potential reasons behind any differences in the outcomes between these t

Total knee Arthroplasty: Patient expectations and outcomes in a resource constrained setting.

OBJECTIVE: Knee osteoarthritis significantly impacts the quality of life, particularly in advanced stages. Total knee arthroplasty (TKA) is a common treatment, but 20-30% of patients remain dissatisfied despite clinical improvements. This study evaluates TKA outcomes using patient-reported outcome measures (PROMs) in a low-middle-income country context. METHODOLOGY: A prospective cohort study from Aga Khan University Hospital of 70 patients with end-stage knee osteoarthritis undergoing TKA was conducted from May, 2023 to December, 2023, Improvement in Knee injury and Osteoarthritis Outcome Sc

Reduction in complication rate and hospital length of stay following robotic arm-assisted simultaneous bilateral total knee arthroplasty when compared to conventional techniques: a cohort study with minimum one year follow-up.

Bilateral simultaneous total knee arthroplasty (TKA) may be indicated in patients with symptomatic bilateral knee arthritis; however, it is associated with higher morbidity and mortality risk compared to staged procedures. We retrospectively reviewed prospectively collected data comparing bilateral simultaneous robotic arm-assisted TKA (RA-TKA) with conventional TKA (CO-TKA), with a minimum 12-month follow-up period. The primary outcomes of interest were 12-month mortality, revision and reoperation rates, as well as inpatient complication rates. Secondary outcomes of interest were surgical dur

Same patient, same total knee arthroplasty, different outcomes? A pilot study comparing conventional vs. OMNIBotics Robotic-Assisted HLS KneetecTM (Corin®).

INTRODUCTION: Total knee arthroplasty (TKA) is a well-established treatment for end-stage knee osteoarthritis. Robotic-assisted TKA (RA-TKA) has emerged over the past decade, enabling patient-specific implantation, precise bone cuts, and intraoperative control of gap balance and alignment. The HLS KneetecTM is a third condyle posterior stabilized TKA, initially designed for implantation using conventional manual instrumentation (C-TKA), and intended for use with an adjusted mechanical alignment. It can now also be implanted with robotic assistance. This pilot study aimed to evaluate the associ

Acute Periprosthetic Joint Infection After Primary Total Knee Arthroplasty: Experience From a Secondary Public Hospital in Northern Mexico.

Background Periprosthetic joint infection (PJI) is an uncommon but serious complication of total knee arthroplasty (TKA), and local data from secondary-level public hospitals in Mexico are limited. We measured the incidence of acute PJI after primary TKA at our hospital, described the patients who developed it, and established a local baseline for prevention and surveillance. Methods We retrospectively reviewed the medical records of 104 consecutive knee arthroplasty patients from October 2024 to September 2025. After excluding one revision and one bilateral procedure, 102 primary unilateral T

Remote and In-person Supervised Exercise in Patients with Knee Osteoarthritis (RISE-KOA): study protocol for a non-inferiority randomized controlled trial.

BACKGROUND: Knee osteoarthritis (OA) is a prevalent joint condition resulting in years lived with disability. A first-line treatment recommended by clinical guidelines is the therapeutic exercise to control pain and improve physical function. One possible approach for exercise supervision is telehealth using video calls, as it can be an effective alternative to in-person physical therapy for treating musculoskeletal conditions, expanding community access to physical rehabilitation. In this scenario, this study aims to investigate whether a muscle-strengthening exercise program for the lower li

Can EEG-Neurofeedback Training Enhance Effective Connectivity in People With Chronic Secondary Musculoskeletal Pain? A Secondary Analysis of a Feasibility Randomized Controlled Clinical Trial.

INTRODUCTION: Persistent musculoskeletal pain is associated with altered functional and effective connectivity (EC) between cortical regions involved in pain processing. Especially, disruptions in the infraslow fluctuation (ISF) frequency band can contribute to pain persistence. ISF electroencephalography-neurofeedback (EEG-NF) has emerged as a potential non-invasive neuromodulatory intervention targeting cortical brain regions to restore balance and modulate pain-related pathways. However, limited research explores its effect on EC, a measure of directional information flow critical to pain e

Relation of vertical ground reaction forces while walking to contralateral structural worsening in adults with unilateral knee osteoarthritis: the Multicenter Osteoarthritis Study.

BACKGROUND: It is unclear why knee osteoarthritis (KOA) progresses from a unilateral to bilateral joint disease. This study aimed to evaluate relations between vertical ground reaction forces (VGRFs) while walking and structural worsening in the unaffected limb of adults with unilateral KOA. METHODS: Discrete metrics were extracted from the unaffected limb VGRF signal while walking at a self-selected speed. Structural worsening of the unaffected knee was evaluated over 2 years with radiographs (primary) and MRIs (secondary). Logistic regression models evaluated associations between VGRF metri

Quantitative Magnetic Resonance Imaging of Lateral Compartment Articular Cartilage After Lateral Extra-articular Tenodesis.

BACKGROUND: Concerns have arisen that anterior cruciate ligament reconstruction (ACLR) with lateral extra-articular tenodesis (LET) may accelerate the development of posttraumatic osteoarthritis in the lateral compartment of the knee. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate whether the augmentation of ACLR with LET affects the quality of lateral compartment articular cartilage on magnetic resonance imaging (MRI) at 2 years postoperatively. We hypothesized that there would be no difference in T1rho and T2 relaxation times when comparing ACLR alone with ACLR + LET. STUDY

Medial Opening Wedge Osteotomy for Early Osteoarthritis of the Knee With Dr. Saigal's Plate: A Case Report With Review of Literature.

Knee pain in patients often involves varus deformity and unicompartmental osteoarthritis (OA). High tibial valgus osteotomy (HTO) is increasingly recognized as an effective treatment, as it realigns the knee's mechanical axis towards the healthier lateral compartment, delaying degenerative changes in the medial compartment and reducing the need for joint replacement. This case report discusses two patients with bilateral knee arthritis and varus deformity who underwent medial opening-wedge high tibial osteotomy (MOWHTO) using Dr. Saigal's plate (Nebula Surgical Pvt. Ltd., Gujarat, India). The

The temporal effects of variable tourniquet use on pain after total knee arthroplasty.

BACKGROUND: Tourniquet use in total knee arthroplasty (TKA) remains controversial. While widely studied, any temporal effects on pain and opioid utilization have not been investigated. We hypothesized that postoperative opioid utilization increases with increasing tourniquet times in TKA. METHODS: We retrospectively reviewed 1110 TKAs by three arthroplasty surgeons from October 2016 through September 2019. Exclusion criteria included ambulatory surgery, undocumented tourniquet times, simultaneous bilateral TKA, and diagnoses other than osteoarthritis, inflammatory arthritis, or osteonecrosis.

Treatment of grade III knee osteoarthritis with bone marrow stimulation and intraarticular injection of triamcinolone and hyaluronic acid combination; three case report and literature review.

INTRODUCTION AND IMPORTANCE: Osteoarthritis is a severe joint disease that affects more than 60% of the elderly and more common an idiopathic disease or unknown cause although there is also a secondary cause such as trauma, infection, neurological or metabolic disorders. The usual complaints that perceived by osteoarthritis patients is pain in the affected joint. Many patients and providers think of arthroplasty as the only surgical option for the treatment of osteoarthritis of the knee. However, there are several other surgical options for osteoarthritis which can be therapeutic alternatives.
