Impact of topical cetylated fatty acid cream on hand osteoarthritis: a randomized, double-blind clinical trial.

In addition to topical non-steroidal anti-inflammatory drugs that are used in hand osteoarthritis, there are other drugs that can reduce pain in osteoarthritis, such as topical cetylated fatty acids. Previous studies have reported that topical CFAs are effective in all knee OA patients, with slightly higher evidence for those with advanced disease. The mechanism of pain in OA knee and OA hand is similar. Furthermore, there are no previous studies on CFAs in OA hand. Patients fulfilling the American College of Rheumatology criteria for hand OA participated in this randomized, double-blind, plac

Reason Profiles for not Returning to Preinjury Activity Level Following Anterior Cruciate Ligament Reconstruction- A Latent Class Analysis With Subgroup Comparison of Patient-Reported Outcome Measures.

BACKGROUND: Given the high proportion of athletes who do not return to sports (RTS) after anterior cruciate ligament reconstruction (ACLR), strategies are needed to identify at-risk patients and optimize rehabilitation for successful RTS after ACLR. PURPOSE/HYPOTHESIS: This study used latent class analysis (LCA) to characterize a unique clustering of reasons why athletes do not return to their preinjury activity level after ACLR. We hypothesized that patients with high pain scores and high levels of fear would be less likely to return to their preinjury activity level. STUDY DESIGN: Cohort s

Measuring static and dynamic lower limb alignment in patients with advanced knee osteoarthritis using markerless motion capture.

PURPOSE: Marked lower limb malalignment is associated with the progression of knee osteoarthritis (OA). Markerless motion capture is a computer-vision tool that can measure lower limb alignment throughout both static and dynamic tasks. RESEARCH QUESTIONS: The primary objective of the study was to quantify static and dynamic lower limb alignment for patients diagnosed with advanced medial and lateral knee OA. This analysis also explored sex differences in alignment from the static and dynamic tasks. The secondary objective was to investigate if the mean knee adduction angle during quiet standi

Statistical analysis plan for the Robotic-Assisted Surgery and Kinematic ALignment in total knee arthroplasty (RASKAL) registry-nested randomized trial.

AIMS: There is uncertainty over whether robotic-assisted surgery (RAS) or kinematic alignment (KA) improves outcomes in total knee arthroplasty (TKA). The Robotic-Assisted Surgery and Kinematic ALignment in TKA trial (RASKAL) is a registry-nested, multicentre, randomized, blinded, 2 × 2 factorial trial involving approximately 300 TKA patients. RASKAL will assess the effectiveness of RAS, KA, or both to improve clinical, operative, functional, radiological, and survivorship outcomes when compared to computer-assisted surgery (CAS) and mechanical alignment (MA), respectively. We describe the sta

Does Timing of Hindfoot Reconstruction Before or After Primary Total Knee Arthroplasty Impact Reoperation Rates?

BACKGROUND: Total knee arthroplasty (TKA) and hindfoot reconstruction (HFR) are often used to treat osteoarthritis and deformities of the knee and hindfoot. Despite the proven clinical effectiveness of these treatments, postoperative patient dissatisfaction is high. The knee and ankle are adjacent joints in the lower extremity, and angular deformity of either joint is known to affect positioning of the anatomical and mechanical axes of the entire limb. Although it is understood this relationship exists, there are limited studies evaluating outcomes of ipsilateral procedures on patients with pa

Association Between Tourniquet Use and Patient-Reported Outcomes Following Total Knee Arthroplasty: A Multicenter Comparison.

BACKGROUND: Total knee arthroplasty (TKA) is one of the most commonly performed elective procedures in North America. While advancements have been made in patient optimization, surgical technique, and implant design, tourniquet use remains a contentious issue as it relates to patient outcomes and postoperative experience. METHODS: As part of the PEPPER trial, we identified 5,684 patients who underwent primary TKA, of whom 4,866 (85.6%) underwent surgery with a tourniquet (the YT group) and 818 (14.4%) underwent surgery without a tourniquet (the NT group). The cohort was predominantly female (

Kinematic alignment achieves a tibial joint line angle that is more parallel to the floor in single-leg stance than mechanical alignment : a radiological analysis of a randomized controlled trial.

AIMS: Previous research on knee kinematics has demonstrated that achieving a tibial joint line angle (TJLA) parallel to the floor in the single-leg stance phase of gait may restore native knee kinematics and optimize compartmental compressive loads in total knee arthroplasty (TKA). However, it is currently unclear which surgical alignment strategy best achieves this. Therefore, this study sought to determine whether kinematically aligned (KA) or mechanically aligned (MA) TKAs produce a TJLA closer to parallel. METHODS: A total of 95 patients were randomized to KA (n = 46) or MA (n = 49). Cons

Stem cell injections for osteoarthritis of the knee.

BACKGROUND: Stem cells are specialised precursor cells that can replace aged or damaged cells and thereby maintain healthy tissue function. Stem cell therapy is increasingly used as a treatment for knee osteoarthritis, despite the lack of clarity around the mechanism by which stem cell therapy may slow down disease progression in osteoarthritis, and uncertainty regarding its benefits and harms. OBJECTIVES: To assess the benefits and harms of stem cell injections for people with osteoarthritis of the knee. A secondary objective is to maintain the currency of the evidence, using a living system

Effects of Walking Aids on Knee Adduction Moment After Total Knee Arthroplasty: A Gait Analysis Study.

BACKGROUND: Total knee arthroplasty (TKA) is a common intervention for treating end-stage knee osteoarthritis (OA). Postoperatively, altered biomechanics may increase the risk of OA progression in the contralateral knee. Knee adduction moment (KAM) is a key indicator of medial knee joint loading and is associated with OA progression. Walking aids are often prescribed to reduce joint stress during walking; however, their comparative effects on KAM in patients undergoing TKA remain unclear. This study aimed to evaluate the effects of different walking aids (double canes [D-canes], rollators with

Research progress on biodegradable polymer-based drug delivery systems for the treatment of knee osteoarthritis.

Knee osteoarthritis (KOA) is a disease that involves multiple anatomical and physiological changes in the knee tissues, including cartilage degeneration, bone remodelling and formation of bony encumbrances, which leads to clinical manifestations including pain, stiffness, swelling and limitation of knee function. Knee osteoarthritis is a chronic joint disease characterised by degenerative cartilage lesions and secondary osteophytes in the knee joint. The symptoms of knee osteoarthritis tend to progress slowly, and at this stage, the number of patients with KOA is increasing. However, due to th
