Clinical outcome comparison of suture button suspensionplasty versus trapeziectomy with ligament reconstruction and tendon interposition in patients with carpometacarpal joint arthritis: a systematic review of direct comparison studies.

INTRODUCTION: Trapeziectomy nowadays is predominantly done with Ligament Reconstruction and Tendon Interposition (LRTI) or Suture Button Suspensionplasty (SBS). This article is intended to highlight comparison between SBS and trapeziectomy with LRTI through direct comparison evidence. MATERIAL AND METHOD: The LRTI technique involves the removal of the diseased trapezium bone and creating suspension through the use of flexor carpi radialis tendon, while SBS stabilize the metacarpal base by suspending the first metacarpal bone. The record screening process of this systematic and meta-analysis w

Subluxation of the first carpometacarpal joint and age are important factors in reduced hand strength in patients with hand osteoarthritis.

OBJECTIVE: To investigate the determinants of hand strength in patients with hand osteoarthritis (OA). METHOD: Pinch and cylinder grip strength were measured in 527 patients with hand OA diagnosed by their treating rheumatologist from the Hand OSTeoArthritis in Secondary care (HOSTAS) study. Radiographs of hands (22 joints) were scored 0-3 (scaphotrapeziotrapezoid and first interphalangeal joints 0-1) on osteophytes and joint space narrowing following the Osteoarthritis Research Society International atlas. The first carpometacarpal joint (CMC1) was scored 0-1 for subluxation. Pain was assess

Outcomes of First Metacarpal Extension Osteotomy for Base of Thumb Arthritis.

BACKGROUND: First metacarpal extension osteotomy (FMEO) aims to correct the adduction deformity associated with thumb arthritis, as well as improve the congruity at the first carpometacarpal (FCMC) joint. However, the benefits of this procedure are currently unclear. The purpose of this study is to investigate the outcomes of FMEO in the treatment of FCMC joint arthritis. METHODS: Electronic databases were searched systematically for original data studies in the English language reporting outcomes following FMEO for base of thumb arthritis. Data were extracted from the text, tables, and figur

Mini TightRope® suture button indications for thumb basal joint arthritis.

Our aim was to explore the different indications of the Mini TightRope® suture button whether during first-line treatment or secondary surgery of thumb carpometacarpal osteoarthritis (OA). The indications for this new device are presented and discussed based on our own small case series and published data. Twenty-one patients treated with the Mini TightRope® were included in a prospective study. One of them was operated on both sides. Fourteen devices were used in first intention for Eaton stage IV OA; the patients underwent trapeziectomy and suture button suspensioplasty only. Four cases cons

Dynamic stabilization home exercise program for treatment of thumb carpometacarpal osteoarthritis: A prospective randomized control trial.

STUDY DESIGN: Randomized control trial. INTRODUCTION: Thumb carpometacarpal (CMC) osteoarthritis (OA) is a common cause of hand pain and disability. Standard conservative therapy (SCT) for thumb CMC OA includes an orthosis and instruction in joint protection, adaptive equipment, and pain relieving modalities. The dynamic stability home exercise (HE) program is complementary conservative therapy designed to strengthen the stabilizing muscles of the thumb CMC. PURPOSE OF THE STUDY: To investigate whether the addition of HE to SCT (SCT+HE) was more effective at reducing pain and disability in t

Psychologic Factors Do Not Affect Placebo Responses After Upper Extremity Injections: A Randomized Trial.

BACKGROUND: Studies on how psychologic factors influence the placebo effect have shown conflicting results in an experimental setting. Pessimists are more likely to experience a nocebo effect (feel worse after an inert intervention), whereas other studies suggest that patients with more symptoms of depression or anxiety or greater neuroticism have a greater response to a placebo. This is important because treatment benefits are potentiated by placebo effects, and optimal utilization of this phenomenon may improve clinical outcomes. QUESTIONS/PURPOSES: (1) What psychologic factors are associat

[Arthrodesis of the carpometacarpal joint of the thumb].

PURPOSE OF THE STUDY: Arthrodesis of the carpometacarpal (CMC) joint of the thumb is an option for treatment of degenerative and post-traumatic conditions affecting the CMC thumb joint. This procedure is indicated most frequently in patients with primary or secondary stage III rhizarthritis (by the Eaton classification of basal joint arthritis) of idiopathic, rheumatic or post-traumatic etiology. Patients with multidirectional CMC instability, usually traumatic in origin, undergo this surgery only occasionally. In our institution, the treatment is based on the Carroll technique. MATERIAL AND

[Long-term results of endoprosthetic replacement of the scaphoid bone of the wrist joint].

Eighty-nine late results (five to twelve years follow-up) of prosthetic replacement of the scaphoid bone with a silastic spacer are presented. In 76% the clinical course was uncomplicated; 24% needed secondary surgical procedures including two arthrodeses of the wrist joint. Flexion and extension of the wrist improved by 10.9%, radial and ulnar deviation by 15.0% on average. The first carpometacarpal joint showed minor restriction in 24.0%. The power grip was diminished by 32.0%, the lateral pinch by 25.0% on average. In 43.0% of the cases development or increase of intraosseous cysts was noti

The other arthritides. Roentgenologic features of osteoarthritis, erosive osteoarthritis, ankylosing spondylitis, psoriatic arthritis, Reiter's disease, multicentric reticulohistiocytosis, and progressive systemic sclerosis.

Osteoarthritis may be divided into primary generalized and secondary forms. Primary generalized osteoarthritis is characterized by narrowing of cartilage, marginal osteophytes, and absence of erosions. The most common sites of involvement are the distal interphalangeal joints of the fingers and the first carpometacarpal joint. Secondary osteoarthritis also results in narrowing of cartilage in the absence of erosions, but in regions of mechanical stress. Erosive osteoarthritis affects predominantly the proximal and distal interphalangeal joints, and evolves into bony fusion in 12 to 15 per cent
