Validation of Baveno VII Criteria using Liver and Splenic Elastography for Predicting Esophageal Varices Severity in Chronic Liver Disease: A Multiparametric, Observational Study.

BACKGROUND AND AIMS: Baveno VII criteria prioritize Liver Stiffness Measurement (LSM) for portal hypertension risk stratification, primarily using transient elastography. We aimed to evaluate two-dimensional-shear wave elastography (2D-SWE) cutoffs, assess the etiology-dependent utility of spleen stiffness (SSM), and introduce a novel Combined Hepato-splenic Stiffness Index (CHSSI). MATERIALS AND METHODS: This ambispective, observational study enrolled 100 chronic liver disease patients. Participants underwent 2D-SWE for LSM and SSM, alongside endoscopy as the reference standard. A novel mult

Adjuvant pegylated interferon-alpha after nucleos(t)ide analog withdrawal drives a temporally coordinated expansion of regulatory B cells and contraction of HBV-specific T cells.

BACKGROUND AND AIMS: The NUC-B study showed that a 4-month course of pegylated interferon-alpha (PEG-IFNα) enhanced the outcome and reduced hepatic flares following nucleos(t)ide analog (NUC) withdrawal in HBeAg-negative chronic hepatitis B. Here, we investigated differential T- and regulatory B-cell (BREG) changes before and during these interventions in a substudy of 41 representative patients. APPROACH AND RESULTS: Antigen-specific T cells were primarily evaluated using quality-controlled IFN-γ ELISpot with 15 pools of genotype-matched HBV core/small-surface peptides, confirmed by multipar

Microbial cues and T cell immunity: deciphering their interactions in chronic liver disease.

Chronic liver disease (CLD) poses a major global health challenge, as its progression to cirrhosis and hepatocellular carcinoma significantly contributes to worldwide morbidity and mortality. Mounting evidence has identified the gut microbiota as a critical modulator of CLD progression, primarily via the gut-liver axis. The liver is continuously exposed to gut-derived signals through the portal circulation, while reciprocally secreting bioactive mediators that shape intestinal microbial composition and function. This bidirectional network enables intricate crosstalk between the gut microbiota

ORF2 of hepatitis E virus: genetic and antigenic variation across genotypes and implications for immunity, diagnostics and vaccine development.

Hepatitis E virus (HEV) causes over 19 million infections annually, yet the molecular determinants governing its diverse clinical phenotypes and cross-genotype immunity remain incompletely synthesized. The ORF2 capsid protein is the sole structural component of the virion, the primary neutralizing antibody target and the basis for all commercial diagnostics and the only licensed vaccine (Hecolin®). While current literature often treats HEV as a monolithic antigenic entity, ORF2 sequences diverge by 15-25% across the eight recognized genotypes (GT1-GT8), with critical substitutions concentrated

Impact of histologic hepatic steatosis on virological response to nucleos(t)ide analogue therapy in patients with chronic hepatitis B.

Chronic hepatitis B remains a major cause of cirrhosis and hepatocellular carcinoma worldwide. We investigated whether histologic hepatic steatosis affects hepatitis B virus deoxyribonucleic acid (HBV DNA) suppression in patients receiving antiviral therapy. This retrospective observational study included 307 adult patients with chronic hepatitis B who received tenofovir or entecavir therapy and underwent pretreatment liver biopsy. Demographic, laboratory, virological, radiological, and histopathological data were retrieved from the electronic medical record system. Laboratory parameters inclu

Acute Hepatitis C Infection in a MSM Living With HIV in Trinidad.

Hepatitis C virus (HCV) infection is widespread globally, however the data on HCV is sparse in the anglophone Caribbean particularly among persons living with HIV. Acute HCV infection is frequently asymptomatic and in resource limited countries, there are numerous challenges with diagnosis and treatment. We present the case of a 45 year old man who has sex with men (MSM) living with HIV in Trinidad with sexually acquired HCV infection in whom there were elevated liver transaminases and low HCV RNA levels that signified a strong immune mediated response in an attempt to clear the virus, but thi

Combined RNA- and DNA-targeting CRISPR ribonucleoproteins enhance suppression of HBV transcripts.

INTRODUCTION: Chronic hepatitis B virus (HBV) infection remains a major global health problem and there is no curative treatment currently available. RNA-targeting CRISPR systems offer a potential alternative to DNA-editing nucleases by enabling direct degradation of viral transcripts without introducing genomic breaks. In this report, we evaluated the antiviral activity of synthetic guide RNA (gRNA)/Cas13d ribonucleoproteins (RNPs) targeting HBV RNA and a combination use with DNA-targeting RNPs. METHODS: We designed 10 gRNAs targeting conserved regions across the HBV transcriptome followed b

Association of nucleos(t)ide analogue treatment history with mortality in HBV-DNA-positive HBV-ACLF: a multicenter cohort study.

BACKGROUND AND AIMS: Hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF) is associated with high short- and long-term mortality. Prior nucleos(t)ide analogue (NA) treatment history, particularly treatment withdrawal and virological resistance, may contribute to ACLF development and influence prognosis after onset. However, the clinical impact of different NA treatment histories in patients with HBV-ACLF remains insufficiently characterized. This study evaluated the associations of baseline HBV-DNA status and prior NA treatment history with mortality in patients with HBV-ACLF.

Immunoinformatics-based design of a multi-epitope mRNA vaccine candidate targeting the major human-infecting hepatitis E virus genotypes 1-4.

BACKGROUND: Hepatitis E virus (HEV) is one of the leading causes of acute and chronic hepatitis among humans across the globe and is linked to severe diseases in immunocompromised individuals and pregnant women. Despite its substantial global burden, only one vaccine is licensed (in China and Pakistan), limiting its global accessibility. Furthermore, no globally approved mRNA vaccine against HEV is currently available, highlighting the need to develop strategies for alternative vaccines. METHODS: In this study, the amino acid sequences of ORF2 capsid proteins representative of HEV genotypes 1

Higher plasma kynurenine-tryptophan ratio predicts incident hepatic fibrosis in women with HIV.

OBJECTIVE: Chronic immune activation and microbial translocation may contribute to hepatic fibrosis in people with HIV. We evaluated whether plasma kynurenine-to-tryptophan (KT) ratio and other biomarkers of microbial translocation predict incident fibrosis in women with HIV. DESIGN: Longitudinal cohort study of 709 women with HIV from the Women's Interagency HIV Study with baseline plasma biomarkers and serial vibration-controlled transient elastography. Participants with baseline fibrosis or viral hepatitis were excluded. Outcomes included incident fibrosis, steatotic liver disease (SLD)-as

Early Antiviral Therapy and Hepatocellular Carcinoma Incidence in Chronic Hepatitis B Patients with Normal Alanine Aminotransferase Levels: A Prospective Phase II Study.

BACKGROUND/AIMS: Whether antiviral therapy with nucleos(t)ide analogs is necessary for chronic hepatitis B (CHB) patients with normal alanine aminotransferase (ALT) levels remains controversial. The D2AS score is a validated model for identifying patients at high risk of hepatocellular carcinoma (HCC) despite normal or mildly elevated ALT levels. We explored whether the observed HCC incidence with immediate antiviral therapy differed from that predicted by the D2AS model. METHODS: In this prospective, multicenter phase II trial, 115 non-cirrhotic CHB patients with ALT <80 U/L and a D2AS score

The HBV basal core promoter mutation confers a replicative advantage and transcriptionally reprograms hepatocytes toward HCC subtypes.

BACKGROUND: Hepatitis B virus (HBV) basal core promoter (BCP) and precore (PC) mutations occur during chronic HBV infection with the BCP-mutant being associated with increased hepatocellular carcinoma (HCC) risk. OBJECTIVES: The effects of these mutants on viral replication, hepatocyte biology, and carcinogenesis remain poorly defined. To address this, we characterized BCP- and PC-mutants using human hepatocyte chimeric mice and asked whether the resulting infection-induced transcriptomes correspond to subsets of human HBV-related hepatocellular carcinomas (HBV-HCCs). DESIGN: Isogenic wild-t

Changing epidemiology and etiology of hepatocellular carcinoma in Europe: implications for clinical practice and health policy.

Primary liver cancer, of which hepatocellular carcinoma (HCC) represents the vast majority, is on the rise globally. Despite an expected decrease in viral hepatitis-related HCCs, the number of new cases and deaths from liver cancer is predicted to increase by over 30% in 2050 in Europe. This is largely driven by an increase in metabolic dysfunction-associated steatotic liver disease-related HCC and an aging population. Since HCC usually develops in patients with underlying chronic liver disease, over half of all cases appear to be preventable by targeting modifiable risk factors. This series p

THE ROLE OF CELL THERAPY IN THE COMPREHENSIVE TREATMENT OF CHRONIC HEPATITIS C-RELATED LIVER CIRRHOSIS.

BACKGROUND: Despite the high efficacy of direct-acting antiviral (DAA) therapy for chronic hepatitis C virus (HCV) infection, a considerable proportion of patients continue to exhibit significant structural liver damage, including advanced fibrosis and cirrhosis. Therefore, the search for additional therapeutic approaches aimed at enhancing liver regeneration and improving hepatic functional status remains highly relevant. OBJECTIVE: To evaluate the clinical efficacy of fetal stem hepatocyte transplantation as part of a comprehensive treatment strategy for liver cirrhosis associated with chro

Integrated computational approaches for identifying and evaluating phytochemical inhibitors against HCV NS3/4A protease.

Hepatitis C virus (HCV) remains a major global health concern, causing chronic hepatitis, cirrhosis, and hepatocellular carcinoma (HCC), with an estimated 185 million people affected worldwide. In the absence of an FDA-approved vaccine, the development of effective antiviral strategies remains essential. The HCV NS3-4A protease, which mediates cleavage of the viral polyprotein at specific downstream sites of NS3 and is essential for replication-complex formation, represents a key therapeutic target. In this study, approximately 1,480 phytocompounds were compiled from the IMPPAT database and pr

Hepatic Steatosis in People With Chronic Hepatitis B in Africa: Prevalence, Correlates, and Clinical Significance.

To address data gaps around metabolic liver disease in Africa, within the Hepatitis B in Africa Collaborative Network (HEPSANET), we evaluated participants with chronic hepatitis B virus infection (CHB) for steatotic liver disease (SLD) using transient elastography with controlled attenuation parameter (CAP). Using multivariable logistic regression, we identified sociodemographic, anthropometric, and clinical associations with SLD (CAP ≥ 248 dB/m), and evaluated whether SLD severity was independently associated with fibrosis. Short-term impacts of SLD on mortality were also assessed with Cox p

RESONANCE: Protocol for a Nationwide Cohort Study of Chronic Liver Diseases Using the French National Health Data System.

BACKGROUND: Chronic liver diseases (CLDs) are frequent in Europe, including in France, and are mainly driven by alcohol, metabolic dysfunction (diabetes and obesity), and viral hepatitis. Although risk factors are well established and easy to identify, CLDs are frequently diagnosed at an advanced stage, translating into poor prognosis. Descriptive data on CLD burden in France remain scarce, and health trajectories of these patients are uncharted. However, such data are crucial to guide clinical practice, to determine the target population for personalized public health policies, and to develop

Becoming Physically Active and Risks of Mortality and Liver-Related Events in Adults With Chronic Hepatitis B: A Nationwide Cohort Study.

Physical activity (PA) confers several health benefits; however, whether increasing PA among physically inactive adults with chronic hepatitis B (CHB) is associated with lower risks of adverse clinical outcomes remains unclear. We used the nationwide Korean National Health Insurance database. We included 318,131 adults with CHB who were physically inactive at the initial health screening (Exam 1) and completed a subsequent screening within 4 years (Exam 2). Among them, 58,703 initiated moderate-to-vigorous PA (MVPA), defined as vigorous activity ≥ 20 min/day on ≥ 3 days/week or moderate activi
