Transposable Element Dysregulation in Hepatocellular Carcinoma: Epigenetic Mechanisms, Immune Remodeling, and Translational Opportunities.

Hepatocellular carcinoma (HCC) commonly develops in the context of chronic liver injury, viral hepatitis, metabolic dysfunction, alcohol-related liver disease and cirrhosis. Although immune checkpoint inhibitor-based combination therapies have improved the treatment of unresectable HCC, primary and acquired resistance and the lack of reliable predictive biomarkers continue to limit precision treatment. Transposable elements (TEs) and TE-derived sequences represent a regulatory layer linking epigenetic disruption, aberrant transcription and immune remodelling. In HCC, DNA hypomethylation, alter

"Five Decades On: metALD Emerges as Liver Disease's New Frontier".

This year marks the 50th anniversary of the American Liver Foundation (ALF). Over the five ensuing decades since its inception, the ALF has advocated for and advanced research and public awareness in liver disease. The rise and subsequent improvement of viral hepatitis prevalence during this time contrasts with rising incidence of alcohol-related liver disease and metabolic dysfunction-associated steatotic liver disease (MASLD). Moreover, the concomitant actions of alcohol toxicity in context of metabolic liver disease are now recognized as metabolic and alcohol-related liver disease (metALD).

Revisiting Liver Monitoring During Methotrexate Therapy in Rheumatoid Arthritis: Implications of HBV Reactivation, MASLD, and Non-invasive Fibrosis Assessment.

Liver function monitoring remains a cornerstone of safety surveillance during methotrexate (MTX) therapy for rheumatoid arthritis (RA). Traditionally, monitoring has focused on hepatitis B virus (HBV) reactivation and chronic MTX hepatotoxicity. Because HBV persists in hepatocytes as covalently closed circular DNA (cccDNA) even after apparent clinical resolution of infection, all individuals with previous HBV exposure remain at risk of viral reactivation. Accordingly, screening for previous HBV infection before initiating MTX is essential, and regular monitoring of HBV DNA should be considered

Case Report: Unexpected HBV reactivation upon nucleos(t)ide analogue discontinuation despite 17 years of sustained HBsAg loss post-liver transplantation.

Long-term administration of nucleos(t)ide analogues (NAs) has substantially mitigated the risk of post-transplant hepatitis B virus (HBV) recurrence. However, the safety and feasibility of NA withdrawal in liver transplant (LT) recipients, even following prolonged virological suppression and sustained hepatitis B surface antigen (HBsAg) loss, remain poorly defined. We present the case of an 84-year-old male with a >40-year history of chronic HBV infection, who progressed to HBV-related cirrhosis and hepatocellular carcinoma, culminating in a deceased-donor liver transplantation (DDLT) in 2004.

Evaluation of the Effects of Peiminine on Disease Activity Indices and Inflammatory Markers in an Experimental Model of Autoimmune Hepatitis.

Autoimmune hepatitis (AIH) is a chronic immune-mediated liver disease that can progress to cirrhosis and liver failure if untreated. Current therapies, mainly corticosteroids, are effective but limited by adverse effects and incomplete responses, prompting the search for safer alternatives. Peiminine, an alkaloid derived from Fritillaria species, has demonstrated anti-inflammatory and antioxidant properties in several disease models. This study evaluated its efficacy in a concanavalin A (ConA)-induced mouse model of AIH. Male C57BL/6 mice were divided into six groups, including ConA-injured an

A multicenter multistate modeling on the natural history of liver decompensation: Longitudinal analysis of 1777 patients.

BACKGROUND: Cirrhosis often remains silent until decompensation, after which survival deteriorates sharply. Contemporary, transition-aware estimates are limited, especially in Asian-predominant cohorts. We analyzed trajectories from compensated advanced chronic liver disease (cACLD) through sequential decompensations to death. METHOD: In this multicenter study, we analyzed retrospective data from an adult cohort with liver stiffness ≥15 kPa from vibration-controlled transient elastography across 19 institutions internationally. Decompensation included ascites, hepatic encephalopathy, spontane

S100B in brain-gut-liver crosstalk: from glial activation to multiorgan inflammation.

S100 calcium-binding protein B (S100B) is conventionally viewed as a marker of astrocytic injury, blood-brain barrier (BBB) disruption, and neurological disorders. Emerging evidence places S100B within the broader brain-gut-liver axis of inflammatory signaling. This review proposes that S100B is a glial-derived integrating effector that couples enteric glial activation, barrier dysfunction, hepatic fibrosis, and hepatic encephalopathy (HE) into a multi-organ inflammatory network. Its spatial expression map, release kinetics, alarmin (damage-associated molecular pattern, DAMP) activity, and imm

Intracellular Zn2+ levels regulate susceptibility to hepatitis B virus infection by modulating NTCP-mediated viral entry.

Hepatitis B virus (HBV) infection remains a significant global health challenge and is a leading cause of chronic liver diseases, including cirrhosis and hepatocellular carcinoma. Although nucleos(t)ide analogs and interferon-based therapies suppress viral replication, they do not eliminate the viral reservoir, highlighting the need for novel therapeutic strategies. HBV utilizes the sodium taurocholate cotransporting polypeptide (NTCP) as a key receptor for hepatocyte entry; however, the underlying mechanisms governing this process remain incompletely understood. In this study, we demonstrate

A Case of Drug-Induced Liver Injury Associated With Saw Palmetto.

Saw palmetto (SP) is a widely used herbal supplement for benign prostatic hyperplasia (BPH) and is generally considered safe. However, rare cases of SP-associated drug-induced liver injury (DILI) have been reported. We describe a case of cholestatic hepatitis attributed to SP, highlighting the diagnostic challenges associated with herbal supplement use and polypharmacy. A 70-year-old male with a history of BPH, hypertension, chronic obstructive pulmonary disease, atrial fibrillation, and cholecystectomy presented with one month of jaundice, pruritus, and pale stools. Physical examination demon

Review Article: Cancer Surveillance for Early Detection of Liver Cancer in Patients with HBV Infection.

BACKGROUND AND AIMS: Chronic hepatitis B virus (HBV) infection remains a major cause of hepatocellular carcinoma (HCC). Although antiviral therapy reduces cirrhosis and liver failure, HCC may still develop in patients with HBV, including in non-cirrhotic livers. This review aimed to evaluate current HCC surveillance strategies in HBV infection, focusing on risk stratification and advances in early detection. METHODS: English-language literature published in PubMed and the Cochrane Database was comprehensively reviewed. This narrative review assessed the effectiveness of HCC surveillance strat

8 weeks versus 12 weeks of sofosbuvir-velpatasvir for treatment-naive, non-cirrhotic, chronic hepatitis C (RESOLVE): a multicentre, open-label, non-inferiority, randomised controlled trial in India.

BACKGROUND: Chronic hepatitis C virus (HCV) infection without cirrhosis can be treated with 12 weeks of sofosbuvir-velpatasvir combination therapy. We aimed to assess whether a shorter 8-week treatment regimen of this combination was non-inferior to the standard 12-week regimen in successfully clearing HCV infection in HCV-infected patients without evidence of cirrhosis. METHODS: RESOLVE was a multicentre, open-label, non-inferiority, randomised controlled trial, conducted in five publicly funded hospitals in India. We randomly assigned (1:1) treatment-naive adults (aged ≥18 years) with non-c

Hepatitis C RNA prevalence before and during broad access to direct-acting antivirals in people with HIV across six countries.

BACKGROUND & AIMS: Hepatitis C virus (HCV) RNA prevalence is a key measure in monitoring progress towards elimination. We estimate the effect of limited and broad availability of direct-acting antiviral (DAA) therapies on HCV RNA prevalence among people with HIV, accounting for variations in testing frequency. METHODS: Data from six countries of cohorts participating in the International Collaboration on Hepatitis Elimination in HIV Cohorts (InCHEHC) were included. Follow-up spanned from five years before each country's broad DAA-access date to database closure (5-8 years post-access). To add

Challenges in the detection and assembly of virus integration structures in human genomes.

Oncogenic viral infections are major contributors to cancer development worldwide. Tumor-associated viruses such as human papillomavirus (HPV), hepatitis B virus (HBV), Epstein-Barr virus (EBV), and Merkel cell polyomavirus (MCPyV) can promote malignant transformation through diverse mechanisms, including persistent viral gene expression, chronic inflammation, and, in some cases, integration of viral DNA into the host genome. Among these, HPV is one of the most clinically important DNA tumor viruses and is a major driver of cancers of the cervix, anus, penis, vagina, vulva, and oropharynx, col

Perceived Difficulty Accessing Primary Health Care and Its Associations With Sociodemographic Factors and Exposure to Violence in a Violent Urban Community in Rio de Janeiro, Brazil: Cross-Sectional Study.

BACKGROUND: Urban communities affected by chronic violence face persistent challenges in accessing primary health care (PHC). Although violence is widely recognized as a determinant of health inequities, there exists limited evidence on how direct exposure to violent events is related to individuals' perceptions of difficulty in accessing PHC services in highly vulnerable urban environments. OBJECTIVE: The study aims to examine how sociodemographic characteristics and exposure to violent events are associated with perceived difficulty in accessing PHC in standardized hypothetical mental healt

Neurological Complications of Oncogenic Viruses: Mechanisms, Clinical Manifestations, and Emerging Insights.

Oncogenic viruses are widely recognized for their pivotal role in malignant transformation; however, their capacity to induce neurological disorders has received comparatively less attention. Increasing evidence indicates that several oncogenic viruses, including human T-cell leukemia virus type 1 (HTLV-1), human papillomavirus (HPV), hepatitis B virus (HBV), hepatitis C virus (HCV), and Kaposi's sarcoma-associated herpesvirus (KSHV), are associated with a broad spectrum of neurological complications involving both the central and peripheral nervous systems. These manifestations arise through
