Deep infection of a prosthetic joint is a devastating complication. One proposed mechanism of late prosthetic joint infection involves hematogenous spread from an extraarticular focus of infection. Two cases clearly demonstrate hematogenously acquired prosthetic joint infections, one caused by Clostridium perfringens and the other by Streptococcus pneumoniae. Arthritis in meningococcal disease is not uncommon and can be caused by either septic invasion of joints or allergic manifestations. Primary meningococcal arthritis (PMA) is, on the other hand, a rare form of meningococcal disease presenting as an isolated septic arthritis without other signs of meningococcal disease. Twenty-six consecutive patients with primary biliary cirrhosis (PBC) were subjected to clinical, radiological and serological assessment for evidence of rheumatic disease. Seven patients had asymptomatic liver disease, while the rest had symptoms indicating varying stages of advanced disease. A new type of endoprosthesis for reconstruction of the metacarpophalangeal (MCP) joint has been tested. The novel arthroplasty consists of a joint mechanism with two screw-shaped fixtures for bone anchorage. Eleven patients with refractory rheumatoid arthritis were submitted to a total lymphoid irradiation up to a dose of 20 Gy. Twelve cases of posterior sternoclavicular dislocation were seen over 15 years at the Victoria General Hospital and the Izaak Walton Kiliam Hospital for Children in Halifax, NS. Two patients required open reduction of the dislocation but the rest were treated by closed reduction. One of the former group required threaded K wires for stability, but all other dislocations were deemed stable after reduction and application of a figure-of-eight bandage and an arm sling. I report 12 cases of dislocation of the sternoclavicular joint. Eight cases were treated by closed reduction and redislocation occurred in five. The result was good in five out of these eight cases. Two cases with a redislocation and poor result were operated on: in one the sternoclavicular joint was successfully reconstructed with a palmaris longus tendon, and in the other the result was poor after medial resection of the clavicle. We prospectively studied 96 men with low back pain to evaluate the association of x-ray findings with clinical diagnosis, treatment, functional outcome, and satisfaction with care. Although Sjögren's syndrome (SS) is a very common autoimmune disease there are no uniformally accepted diagnostic criteria. In this study we evaluated various clinical parameters as criteria for this disorder. Fifty-two patients with sicca complaints but no evidence of other autoimmune rheumatic disorders were studied. These patients were further subdivided into definite primary SS (primSS) (n = 19) and possible primary SS (n = 33). The prevalence of symptomatic secondary Sjögren's syndrome in 66 patients with SLE recruited from a defined population in southern Sweden was 59%. In 26 patients (39%) the symptoms were mild and variable, related to disease activity, while in a smaller group (20%) the symptoms appeared to be constant. The latter patients had an increased frequency of thyroid disease and autoantibodies to SS-A. The patients in this group were older, but did not seem to differ in regard to other disease manifestations.
