Transradial Perilunate Fracture-Dislocation With Lunotriquetral Coalition.

Perilunate fracture-dislocations are devastating injuries, and their occurrence in the presence of a congenital carpal coalition is exceedingly rare. This combination fundamentally alters normal wrist biomechanics and significantly complicates surgical decision-making. We present a case of a 52-year-old female with acute hand ischemia following a high-energy motorcycle collision. Radiographic evaluation revealed a transradial perilunate fracture-dislocation complicated by a unilateral Minnaar type 3 congenital lunotriquetral coalition. Emergent closed reduction was performed, which immediately

[Our Experience with Trabecular Metal Total Ankle System].

PURPOSE OF THE STUDY: One of the surgical treatment options for advanced ankle joint destruction with various etiologies is the total joint replacement. Its significant upside is the preservation of range of motion of the ankle joint and less stress on forefoot joints compared to ankle arthrodesis. Since 2022, we have been using the Zimmer Trabecular Metal Total Ankle inserted via a lateral transfibular approach. This study aims to evaluate the initial outcomes and experience with this implant. PATIENTS AND METHODS: Between 2022 and 2024, 65 total ankle replacements were performed in 63 patie

Spectral frequency and broadband noise analysis of the firefighter environment: Potential implications for cardiac health.

Firefighters are well known to have elevated risk of adverse health outcomes across a wide range of diseases and injuries, including cancer, hearing loss, fatal cardiac events, post-traumatic stress disorder (PTSD), sleep disorders, and many other adverse outcomes. It has been speculated that the noise environment is a significant risk factor for several of these diseases. In particular, the impact of noise on cardiac outcomes has recently been identified. An environmental survey of noise that included spectral and broadband analysis was conducted across all 14 firehouses in a southeastern cit

Anti-Inflammatory Treatment and Active Physical Therapy Have Differing Effects on Functional, Mechanical, and Histological Properties in a Rat Model of Post-Traumatic Elbow Contracture.

Post-traumatic joint contracture (PTJC) frequently occurs in the elbow after injury, decreasing range of motion (ROM) and causing dysfunction. Physical therapy improves ROM but does not address underlying inflammation and fibrosis. Combining physical exercise with biological treatment has shown positive results in other physiological systems. Objective: evaluate whether active physical therapy and anti-inflammatory drug treatment, alone or in combination, could preserve mechanics and function in a rat model of PTJC. Elbow PTJC was surgically induced in rats followed by joint immobilization (IM

Fibroblast-derived CCL2 driven by MIF promotes joint capsule fibrosis via macrophage polarization regulation.

Post-traumatic joint contracture (PTJC) is driven by persistent joint capsule inflammation and subsequent fibrosis. CC motif chemokine ligand 2 (CCL2) is recognized as a key regulator of sustained inflammation. However, the relevant regulatory mechanism involved in CCL2 production in PTJC has not been fully elucidated. In this study, we investigated whether MIF can facilitate CCL2 production from fibroblasts and regulate joint capsule fibrosis following PTJC. Our data demonstrated that PTJC-induced elevation of CCL2 levels was synchronous with MIF. Administration of MIF inhibitor 4-IPP at the

Targeting RIG-I alleviates renal tubular epithelial cells PANoptosis during post-traumatic rhabdomyolysis.

Post-traumatic rhabdomyolysis poses a significant threat to human life and health, primarily due to crush syndrome-associated acute kidney injury (CS-AKI). A critical factor contributing to this condition is the destruction of the tubular epithelial barrier, which results from the death of tubular epithelial cells (TECs) caused by myoglobin (Mb) accumulation. In this study, we identified a novel programmed cell death (PCD), termed PANoptosis, occurring in TECs in both in vivo and in vitro CS-AKI models. This process is induced by Mb, with RIG-I serving as the apical sensor molecule for damage-

Will a malunited proximal phalanx affect the outcomes of vascularized PIP joint transfers?

BACKGROUND: Post-traumatic osteoarthritis of the proximal interphalangeal joint (PIPJ) substantially impairs hand function, and vascularized joint transfer (VJT) can be used to restore range of motion (ROM). However, malunion, particularly dorsal angulation of the proximal phalanx (P1), can occur postoperatively. We aimed to analyze the causes and impact of malunited P1 on surgical outcomes in VJT cases and provide insights regarding configuration and corrective osteotomy. METHODS: The authors retrospectively reviewed 61 VJT cases from 2008 to 2022, with the primary indication being PIPJ oste

Multiple Second to Fifth Carpometacarpal Fracture-Dislocations: A Case Report on the Surgical Management of a Rare Hand Injury.

Multiple carpometacarpal (CMC) joint fracture dislocations involving the second to fifth rays are rare injuries, typically resulting from high-energy trauma. Due to the complex anatomy of the CMC joints and associated swelling, these lesions are frequently overlooked, leading to delayed diagnosis and suboptimal outcomes. We report the case of a 21-year-old male patient who sustained simultaneous dislocations of the second through fifth carpometacarpal joints with associated fractures following a high-energy trauma to the hand due to a motorcycle accident. Initial radiographic evaluation confir

Arthrodesis for post-traumatic ankle osteoarthritis leads to a reduced function of the affected limb with changes in gait and pedobarography: a retrospective cross-sectional study.

BACKGROUND: Ankle arthrodesis is an established procedure for end-stage ankle osteoarthritis. Besides the known limited functionality, gait and foot arch changes are accompanying pathologies, which are little investigated. The aim of the present study was to determine the influence of ankle arthrodesis on gait, pedobaropraphy and functional outcome. METHODS: Thirtyfive patients with unilateral ankle joint fusion after post-traumatic osteoarthritis and a mean follow-up of 60.8 months (SD 18.5, range 32-92) were included. Primary outcome were a 3-D gait analysis including pedobarography. Americ

The coronoid as the key fragment of trans-ulnar fracture-dislocations of the elbow: Insights from a retrospective cohort comparison using the coronoid-centric Mayo classification system.

BACKGROUND: Trans-ulnar fracture-dislocations of the elbow are rare injuries with complex fracture patterns and variable outcomes. Traditional classification systems offer limited prognostic value. A recently introduced coronoid-centric Mayo classification distinguishes injury subtypes based on coronoid attachment and identifies trans-ulnar basal coronoid (TUBC) fractures as a particularly challenging entity. This study evaluated outcomes across Mayo fracture types and explored factors associated with inferior results in TUBC injuries. MATERIALS AND METHODS: In this retrospective cohort study

Comparison of innate and adaptive immune responses to joint injury during the onset and progression of post-traumatic osteoarthritis in common murine models.

Post-traumatic osteoarthritis (ptOA) following joint injury accounts for at least 12% of cases of symptomatic OA. Surgical restoration of joint biomechanics does not prevent ptOA development, suggesting other factors influence individual OA risk. One suggested factor is the immuno-inflammatory response in the joint, with both adaptive and innate immune responses implicated. Whether different joint injuries incite different acute inflammatory responses, potentially explaining variable ptOA risk, has not been explored. Male 10-12-week-old C57BL/6 mice were randomly assigned to one of 6 intervent

Maternal omega-6 to omega-3 conversion reduces osteoarthritis pain-related behaviors in adult female offspring.

BACKGROUND: A parental high-fat diet (HFD) enriched in inflammatory omega-6 polyunsaturated fatty acids (n-6 PUFAs) increases the severity of post-traumatic osteoarthritis (PTOA) in adult offspring, a risk factor which lacks therapeutic interventions. This study investigated whether rebalancing the maternal omega-6 to omega-3 fatty acid (n-6/n-3 FA) ratio during pregnancy and lactation could mitigate PTOA severity in adult offspring. METHODS: Wildtype (WT) and fat-1 female mice, which endogenously convert n-6 to n-3 PUFAs, were fed either a control or n-6-enriched HFD and mated to control-fed

Post-traumatic anterior glenohumeral microinstability in nonathletic patients satisfactory short-term outcomes of a case-control study of concurrent intra-articular soft arthroscopic Latarjet technique and suture-anchor capsular plication.

INTRODUCTION/OBJECTIVES: Arthroscopic management of post-traumatic anterior glenohumeral micro-instability, in the non-athletes, has not yet been thoroughly investigated. The current study compared the outcomes of antero-inferior capsulo-labral repair for the management of anterior glenohumeral microinstability (following a single-event trauma in the nonathletes) versus those of Bankart repair for the management of recurrent anterior gleno-humeral dislocation/macro-instability. METHODS: Conducted between March 2018 and February 2024, this case-control study included 62 patients allocated acco

Mitochondrial unfolded protein response as a central stress-integration hub: mechanisms and implications in disease contexts.

The mitochondrial unfolded protein response (UPRmt) is a conserved mitochondrial stress response that is activated by mitochondrial dysfunction to maintain proteostasis. Although UPRmt has been extensively studied in aging and cancer, its role in trauma and critical illness remains poorly understood. Here, we propose a unifying conceptual framework in which UPRmt functions as a central stress-integration hub that senses and coordinates adaptive responses following acute injury. We systematically review the mechanisms of UPRmt activation triggered by diverse insults and highlight how UPRmt inte

Wnt/β-catenin signaling regulates fibrotic atrophy of intra-articular adipose tissue in post-traumatic osteoarthritis.

Synovial joints like the knee are home to adipose tissue depots whose anatomy and functions are closely intertwined with that of other intra-articular soft tissues such as synovium, underscoring the growing understanding that joints are multi-tissue organs. Traumatic joint injury and the onset of osteoarthritis (OA) dramatically remodel the intra-articular adipose niche, marked by infiltration of fibrotic tissue postulated to underpin OA-associated joint stiffness and pain, yet we know very little about the disease-associated dynamics of joint adipose remodeling nor the mechanisms driving thes

Preclinical Evaluation of Human Donor-Derived Micronized Bone Marrow Stroma/Parenchyma Versus Bone Marrow Aspirate Concentrate in a Rat Model of Post-Traumatic Knee Osteoarthritis.

Bone marrow aspirate concentrate (BMAC) is widely used as a source of mesenchymal stromal/stem cells (MSCs) for musculoskeletal regeneration; however, BMAC lacks essential bone marrow extracellular matrix (ECM) components, a critical component of the stem cell niche that regulates MSC survival, paracrine signaling, and regenerative capacity. We previously demonstrated that an ECM-retaining micronized bone marrow product (BMAX™) preserves pro-regenerative MSC phenotypes in vitro. Human bone marrow from a single donor was processed into conventional BMAC or BMAX™. Post-traumatic osteoarthritis w

Biomechanical analysis of ACL, PCL, and/or meniscus transection in Turkey: A potential large OA animal model.

Osteoarthritis (OA) is a progressive disease characterized by cartilage degradation, subchondral bone remodeling, and osteophyte formation, leading to pain and functional decline. Animal models are crucial for studying injury mechanisms, biomechanical changes, and potential OA treatment. The bipedal turkey model shows promise as a valuable large-animal model due to its arthroscopic feasibility and relevance to traumatic knee injury research. However, the underlying kinematic mechanisms remain poorly understood. This study aimed to elucidate these mechanisms through biomechanical analysis of th
