Preclinical Evaluation of Human Donor-Derived Micronized Bone Marrow Stroma/Parenchyma Versus Bone Marrow Aspirate Concentrate in a Rat Model of Post-Traumatic Knee Osteoarthritis.

Bone marrow aspirate concentrate (BMAC) is widely used as a source of mesenchymal stromal/stem cells (MSCs) for musculoskeletal regeneration; however, BMAC lacks essential bone marrow extracellular matrix (ECM) components, a critical component of the stem cell niche that regulates MSC survival, paracrine signaling, and regenerative capacity. We previously demonstrated that an ECM-retaining micronized bone marrow product (BMAX™) preserves pro-regenerative MSC phenotypes in vitro. Human bone marrow from a single donor was processed into conventional BMAC or BMAX™. Post-traumatic osteoarthritis w

Biomechanical analysis of ACL, PCL, and/or meniscus transection in Turkey: A potential large OA animal model.

Osteoarthritis (OA) is a progressive disease characterized by cartilage degradation, subchondral bone remodeling, and osteophyte formation, leading to pain and functional decline. Animal models are crucial for studying injury mechanisms, biomechanical changes, and potential OA treatment. The bipedal turkey model shows promise as a valuable large-animal model due to its arthroscopic feasibility and relevance to traumatic knee injury research. However, the underlying kinematic mechanisms remain poorly understood. This study aimed to elucidate these mechanisms through biomechanical analysis of th

Surgical treatment of pulmonary hydatidosis: six years of experience and results at the National Hospital Guillermo Almenara Irigoyen, Lima, Peru.

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

Regulation of senescence-associated secretory phenotypes in osteoarthritis by cytosolic UDP-GlcNAc retention and O-GlcNAcylation.

UDP-GlcNAc serves as a building block for glycosaminoglycan (GAG) chains in cartilage proteoglycans and simultaneously acts as a substrate for O-GlcNAcylation. Here, we show that transporters for UDP-GlcNAc to the endoplasmic reticulum (ER) and Golgi are significantly downregulated in osteoarthritic cartilage, leading to increased cytosolic UDP-GlcNAc and O-GlcNAcylation in chondrocytes. Mechanistically, upregulated O-GlcNAcylation governs the senescence-associated secretory phenotype (SASP) by stabilizing GATA4 via O-GlcNAcylation at S406, which compromises its degradation by p62-mediated sel

Osteoarthritis.

Osteoarthritis is a heterogeneous whole-joint disease that can cause pain and is a leading cause of disability and premature work loss. The predominant disease risk factors - obesity and joint injury - are well recognized and modifiable. A greater understanding of the complex mechanisms, including inflammatory, metabolic and post-traumatic processes, that can lead to disease and of the pathophysiology of pain is helping to delineate mechanistic targets. Currently, management is primarily focused on alleviating the main symptoms of pain and obstructed function through lifestyle interventions su

Identification of a Possible Endocannabinoid-Mediated Mechanism of Action of Cetylated Fatty Acids.

Some musculoskeletal disorders, including osteoarthritis; arthrosis; post-traumatic injuries; and other inflammatory tendon, joint and muscular afflictions, still represent unmet medical needs. Cetylated fatty acids (CFAs) are key components of widely distributed over-the-counter products, especially for topical use, which are intended to reduce symptoms associated with these conditions. Nevertheless, the mechanism of action of CFAs' analgesic and anti-inflammatory properties has not yet been clearly established. Endocannabinoids, such as 2-arachidonoylglycerol (2-AG) and anandamide (AEA), are

Knowledge, mechanisms, and intervention of the polytrauma clinical triad in military pain medicine.

This up-to-date review focused on the recent advances in the concept, interrelationship, and mechanisms of the polytrauma clinical triad (PCT), and intervention knowledge and approaches to its prevention and treatment from the perspective of military pain medicine, hoping to provide a scientific basis and reference of PCT. Many soldiers suffering from chronic pain coexist with post-traumatic stress disorder and traumatic brain injury. If the above three diseases exist, it is called PCT. Three diseases in this common triad are interrelated and reinforce each other. This triad is challenging to

Biomarkers of aging as it relates osteoarthritis: we can't improve what we can't measure.

Aging is the largest risk factor for the development of osteoarthritis (OA), a major contributor to increased years lived with disability. This review reflects on how age-related changes relevant to OA have been measured at various length scales. Key discoveries include increased chondrocyte DNA damage with age and the disruption of matrix homeostasis by cellular senescence. Epigenetic clocks have yet to show predictive value for OA, while transcriptomic changes and miRNA profiles are linked to aging and senescence. Protein biomarkers have gained traction in the context of post-traumatic OA an

Chondrocyte-specific knockout of Piezo1 and Piezo2 protects against post-traumatic osteoarthritis structural damage and pain in mice.

BACKGROUND: Osteoarthritis (OA) is a debilitating joint disease characterized by cartilage degeneration, synovial inflammation, and bone remodeling, with limited therapeutic options targeting the underlying pathophysiology. Mechanosensitive ion channels Piezo1 and Piezo2 play crucial roles in chondrocyte responses to mechanical stress, mediating mechanotransduction pathways that influence chondrocyte survival, matrix production, and inflammatory signaling, but their distinct contributions to OA pathogenesis remain unclear. METHODS: Using inducible, chondrocyte-specific Aggrecan-Cre (Acan) mic

Nintedanib reduces severity of post-traumatic joint contracture by modulating fibrosis and inflammation.

This study investigates the potential of nintedanib, a tyrosine kinase inhibitor with antifibrotic and anti-inflammatory properties, to mitigate post-traumatic joint contracture (PTJC) in a rat model. Given the lack of effective pharmacological treatments for this debilitating condition, this study aims to address the unmet need for non-surgical interventions by targeting the underlying fibrotic and inflammatory processes. A total of 26 male Sprague-Dawley rats were subjected to standardized knee trauma and immobilization for 2 weeks. Rats were randomized into two groups: a nintedanib treatmen

Understanding Varus Posteromedial Instability: Biomechanics, Injury Patterns, and Achieving Successful Stability.

Understanding the varus posteromedial rotatory instability requires a detailed clinical examination and better radiographic characterization of the anteromedial facet fractures with the aid of 3-dimensional computed tomography humeral subtraction views. A better understanding of these injuries allows for the development of more reliable management algorithms to avoid overtreatment with increased risk of complications or undertreatment with the risk of progressive post-traumatic arthritis. An algorithm based on the detailed assessment of the radiographic features of the varus posteromedial rota

DNA methylation as a new frontier in treating fear-related disorders: A need for careful evaluation.

In the contemporary research landscape of mental illness treatment, fear-related disorders such as post-traumatic stress disorder continue to pose significant challenges. Although exposure therapy remains a fundamental component of treatment, its efficacy varies considerably among individuals. DNA methylation plays a pivotal role in the extinction of fear memories, providing a promising molecular mechanism that could enhance the success of exposure-based interventions. Extensive studies have consistently demonstrated a substantial association between DNA methylation and neuronal plasticity. Wh

Personalized Approaches to Diagnostic and Therapeutic Strategies in Periprosthetic Fracture-Related Infections (PFRIs): Case Series and Literature Review.

Aim: Periprosthetic fracture-related infections (PFRIs) are a serious complication of total arthroplasty, with incidence rates increasing in line with the growing number of joint replacements. PFRI can lead to prolonged hospitalization, multiple surgical procedures and suboptimal functional outcomes. The diagnosis of PFRI remains challenging due to the overlap of clinical symptoms with other post-traumatic conditions, and identification of the pathogen often fails through conventional methods. This study also highlights the importance of a personalized medicine approach in managing PFRI, where

Combining DNA methylation features and clinical characteristics predicts ketamine treatment response for PTSD.

Post-traumatic stress disorder (PTSD) exhibits extensive clinical and biological variability, making treatment challenging. The Consortium to Alleviate PTSD (CAP)-ketamine trial, the largest randomized study of ketamine for PTSD, found no overall benefit of ketamine over placebo, underscoring the necessity to identify responsive subgroups. Using pre-treatment blood DNA methylation profiles and clinical measures from the CAP-ketamine trial, we applied machine learning to predict treatment response. A model based on 1,208 methylation sites achieved higher predictive accuracy than models using cl
