[Cervical Lesions or Peripheral Nerve Disorders? How Far Can Neurological Examination Localize the Lesion?].

Accurate lesion localization is a fundamental component of neurological diagnosis. This review describes how neurological examination can be used to differentiate cervical spinal cord lesions, cervical radiculopathy, brachial plexopathy, and peripheral nerve disorders in patients presenting with upper-limb sensory and motor symptoms. Particular attention is given to the distribution of sensory impairment, tendon reflex abnormalities, and patterns of muscle weakness. The characteristic clinical features of cervical spondylotic myelopathy, cervical radiculopathy, brachial plexus disorders, and e

Holt-Oram syndrome presenting with failure to thrive due to a hemodynamically significant patent ductus arteriosus: the importance of early cardiac evaluation in infants with radial ray abnormalities.

BACKGROUND: Holt-Oram syndrome (HOS) is a rare autosomal dominant disorder caused by pathogenic variants in the TBX5 gene and characterized by radial ray malformations associated with congenital heart defects and/or cardiac conduction abnormalities. Although atrial and ventricular septal defects are the most common cardiac manifestations, other congenital cardiac lesions may occur. Early recognition of cardiac involvement is essential to prevent complications and improve outcomes. CASE PRESENTATION: We report the case of an 8-month-old female infant presenting with failure to thrive, feeding

Case Report: Pallister-Hall syndrome diagnosed in adulthood during evaluation of recurrent hypoglycemia.

INTRODUCTION: Pallister-Hall syndrome (PHS) is a rare autosomal dominant disorder caused by pathogenic variants in GLI3 gene. It is characterized by variable combinations of hypothalamic hamartoma, polydactyly, bifid epiglottis, renal and genitourinary abnormalities, imperforate anus, and pituitary dysfunction. Most cases are recognized during infancy or childhood; adult presentation is uncommon and hypoglycemia is not an established manifestation of PHS. CASE DESCRIPTION: A 26-year-old man with obesity and well-controlled hypertension was referred for evaluation of recurrent fasting and post

Cell cycle arrest of a craniofacial epithelial population shapes faces in mice and humans.

The mechanisms that shape human facial features and lead to abnormalities if disrupted are complex and multilayered. Orofacial clefting (OFC), the most common craniofacial disorder, results from a failed fusion of facial prominences, partly driven by persisting of cephalic epithelia. Here, we characterize an epithelial cell population, the zippering lambda (ZL), which is cell cycle-arrested and mediates facial prominence fusion in mice and humans. We find uninhibited cell cycle progression in the ZL of Pbx1/2 and p63 mutant mice with OFC. A comparison of ZL-enriched genes with human OFC whole-

To Walk or Not? A Prospective, Randomised Controlled Trial to Evaluate the Effect of Weightbearing on Patient Outcomes Following First Metatarsophalangeal Joint Fusion.

BACKGROUND: First metatarsophalangeal (MTP) joint arthrodesis is the gold standard surgical treatment of hallux rigidus and severe hallux valgus with osteoarthritis after failed nonoperative management. Despite its common use, postoperative weightbearing protocols vary considerably. To date, no prospective randomised controlled trials have directly compared outcomes between immediate weightbearing as tolerated (WBAT) and non-weightbearing (NWB) using modern fixation techniques. This study aimed to evaluate whether immediate WBAT following first metatarsophalangeal joint arthrodesis leads to di

The wandering lens: Clear Lens in the anterior chamber following blunt trauma.

Ocular trauma is a common cause of visual morbidity and can lead to various anterior and posterior segment complications. Traumatic dislocation of the crystalline lens in toto into the anterior chamber without open globe injury is a rare entity. We report a 27-year-old male who presented after blunt trauma from a wooden door with sudden visual loss in the right eye. Best-corrected visual acuity was counting fingers at 1 metre. Slit-lamp examination revealed a clear crystalline lens completely dislocated into the anterior chamber with a clear cornea, well-formed anterior chamber, round pupil an

Chitosan film based piezoelectric nanogenerator for ambient energy harvesting.

Herein, we report a flexible chitosan-based piezoelectric nanogenerator (CPENG) fabricated using a simple and scalable solution-casting method from commercially available chitosan, an abundant and environmentally friendly biopolymer. The CPENG efficiently converts low-frequency mechanical stimuli into electrical energy through the piezoelectric response of the chitosan matrix. Under repeated finger tapping, the device produces an open-circuit voltage of ~ 18 V and a short-circuit current of ~ 1.0 µA. The device also shows good signal reproducibility and stable performance under repeated mechan

A Background-Independent Structural Colored Gel Based on Cellulose/PEDOT: PSS for Highly Sensitive Dual-Mode Sensing.

Hydroxypropyl cellulose (HPC) spontaneously self-assembles into a cholesteric liquid crystal (CLC) structure at certain concentrations in aqueous solution, exhibiting vivid structural colors. Due to incoherent scattering, the apparent color of translucent HPC CLCs is often diminished and nearly invisible against white or colored backgrounds. In this work, a cellulose-based mixture with structural coloration and conductivity was engineered by integrating poly(3,4-ethylenedioxythiophene): poly(styrenesulfonate) (PEDOT: PSS) into HPC. The incorporated conductive dark PEDOT: PSS suppresses the eff

Nailfold capillary density in inflammatory bowel disease: Associations with disease activity and systemic inflammation.

BACKGROUND: Nailfold capillaroscopy offers a non-invasive window into the peripheral microcirculation, but its relevance to inflammatory bowel disease (IBD) activity remains uncertain. We aimed to evaluate capillaroscopic abnormalities in adults with IBD and determine whether capillary density is associated with clinical activity and systemic inflammation. METHODS: This prospective case-control study included 85 adults with IBD and 41 age-matched healthy controls. Disease activity was assessed using the total Mayo score in ulcerative colitis and the Crohn's Disease Activity Index in Crohn's d

Follistatin regulates tooth root furcation morphogenesis.

Follistatin (Fst), an extracellular inhibitor of the TGF-β signaling pathway, plays a critical role in tooth development, particularly in crown morphogenesis and enamel patterning. While molar root number is conserved within species and between upper and lower jaws, the molecular mechanisms governing root patterning remain largely unknown. To investigate the role of Fst in root formation, we analyzed K14-Cre-mediated Fst knockout mice. During embryonic and early postnatal stages, Fst was dynamically expressed in the dental epithelium including the lingual epithelium, and primary and secondary

Outcomes after total first metatarsophalangeal joint arthroplasty for hallux rigidus: a systematic review and meta-analysis.

BACKGROUND: The role of first metatarsophalangeal (MTP) joint arthroplasty in hallux rigidus remains controversial. Although used as a motion-preserving alternative to arthrodesis, outcomes vary between studies and implant designs. This systematic review and meta-analysis evaluated clinical outcomes, range of motion (ROM), and implant survivorship following total first MTP arthroplasty. METHODS: A systematic review identified studies reporting pre- and postoperative outcomes following total first MTP arthroplasty for hallux rigidus. Pooled outcomes were the American Orthopaedic Foot and Ankle

Mosaicism for Cohen-Gibson Syndrome in a Female Individual With Overgrowth and Neurodevelopmental Delay.

BACKGROUND: The Embryonic Ectoderm Development (EED) gene encodes a core component of the Polycomb Repressive Complex 2 (PRC2), which regulates chromatin structure and plays a key role in embryonic development and cell differentiation. Pathogenic variants in EED are associated with overgrowth-intellectual disability syndrome (Cohen-Gibson syndrome, OMIM #617561), though reported cases are rare and typically involve heterozygous germline variants. This study presents an individual with confirmed postzygotic mosaicism in EED, contributing to diagnostic complexity. METHODS: A 12-year-old girl wi

Plag1 Regulates Sensorimotor Modulation in Zebrafish.

The zinc finger transcription factor PLAG1 has been implicated in multiple diverse cellular processes, yet its role in the onset of vertebrate neurodevelopment and behaviour remains largely unexplored. Notably, dysregulation of PLAG1 has been associated with Silver-Russell syndrome (SRS) in humans, a growth disorder often presenting with neurodevelopmental delay and craniofacial abnormalities. Here, we investigate the function of plag1 in zebrafish embryogenesis, focusing on behavioural phenotypes, neural development and craniofacial morphogenesis, as a potential novel model of SRS. Using anti

Use of Femoral Head Allograft with Segmental Defects of the First Ray.

Introduction describes first metatarsophalangeal (MTP) joint arthrodesis as a historical and currently powerful option for severe forefoot deformities, including arthritis, hallux valgus, hallux varus, and trauma. Shortening of the first ray, particularly after MIS bunion procedures and revisional surgeries, creates biomechanics challenges that can compromise fusion. To address large segmental defects, the senior author advocates interpositional femoral head allograft to restore first-ray length and provide structural support for stable MTP joint arthrodesis. The approach avoids autograft draw

Envenomation by a Razi's viper, Macrovipera razii (Serpentes: Viperidae) in Iran: A neglected public health threat, report of two cases, and clinical implications.

Snakebite envenomation remains a significant global health concern, with viperid snakes responsible for substantial mortality and disability. In Iran, the genus Macrovipera is of considerable medical importance. Macrovipera razii is an endemic Iranian species distributed across central and southern regions of the country; however, detailed clinical reports of envenomation by this species remain scarce. We report two cases of M. razii envenomation from Iran. The first case involved a 36-year-old male who sustained a bite to the middle finger in Nazmabad, Arak, Markazi Province, Iran, in 2017. H
