The balanced finger: biomechanics of intrinsic and extrinsic systems and principles of reconstruction.

BACKGROUND: The balance of the finger is maintained through a complex interaction of intrinsic and extrinsic mechanisms that coordinate motion, stability and force transmission across the metacarpophalangeal and interphalangeal joints. Although flexor muscle mass exceeds that of the extensors, equilibrium is achieved through precisely calibrated tendon positioning, lever arms and passive restraints. This balance permits efficient motion with minimal energy expenditure and allows rapid transition between power and precision function.Physiological balance:Active balance is generated by coordinat

Design and Research of Multi Segment Exoskeleton Reconfigurable Soft Finger Actuator.

PURPOSE: The growing number of patients with hand dysfunction caused by conditions such as stroke has led to increasing demand for soft finger rehabilitation actuators. However, existing devices of this type often face issues such as irregular deformation, insufficient driving force, the inability to achieve segmented control, and poor rigidity retention. METHODS: A multi-segment exoskeleton design is proposed, which achieves a functional separation between actuation and load-bearing. Utilizing the principle of virtual work and the Yeoh constitutive model, derive the pressure-to-angle transfe

Organic Crystal-MXene Composites as Temperature-Tolerant Strain Sensors.

Flexible organic crystals offer significant opportunities for organic electronics; however, their potential applications in fields like flexible sensors are at present limited by poor charge transport that result in failure under reversible conditions. In this report, we propose a universal interfacing design strategy for the fabrication of versatile conductive MXene coatings on the surface of flexible organic crystals that preserve the electric conductivity of the MXenes while they also remain mechanically flexible and robust across a wide temperature range of nearly 300°C both above and belo

Reconstruction of Wassel-Flatt type IV-VII radial polydactyly with a modified Bilhaut-Cloquet procedure: a consecutive series of 22 cases.

INTRODUCTION: Radial polydactyly reconstruction requires procedures that achieve both functional and aesthetic outcomes. When both duplicates are of similarly small diameters, a combination procedure may be necessary. Furthermore, primary surgical alignment is crucial to ensure a straight thumb and avoid future deviations. This study evaluates the modified Bilhaut-Cloquet procedure for the reconstruction of Wassel-Flatt types IV-VII radial polydactyly in paediatric patients. METHOD: The study included 22 patients aged 6-60 months. Inclusion criteria include similar-sized duplicated thumbs tha

Toward Flexible Printed Electronics: A Spider-Silk-Inspired, Strong and Tough Thermoplastic Polyamide Elastomer.

The advancement of intelligent and compliant flexible electronic devices demands polymer substrate materials that exhibit both high strength and toughness, which are critical for the performance and durability of printed circuits. Inspired by the structure of spider silk, we designed and synthesized a series of naphthalene-containing thermoplastic polyamide elastomers (TPAE) in this work. The optimized TPAE-N40%-PEG25% exhibits an impressive hydrogen bonding density of up to 41.8%, leading to outstanding mechanical properties: a tensile strength of 37.3 MPa, an elongation at break of 470.8%, a

Bilateral extensor pollicis et indicis accessorius: clinical and anatomical perspectives.

PURPOSE: The extensor muscles of the hand play a crucial role in thumb and finger movement. Thumb extension is primarily mediated by the extensor pollicis longus muscle and extensor pollicis brevis muscle, and extension of the index finger is facilitated by the extensor indicis muscle and extensor digitorum muscle. This report describes a rare anatomical variation, the extensor pollicis et indicis accessorius, in which a single tendon bifurcated to insert on both the index finger and thumb. METHODS: This variation was identified bilaterally during routine dissection of a 92-year-old female do

Variants in the CxxC domain of the epigenetic regulator KDM2B support its role in developmental eye anomalies.

KDM2B encodes an epigenetic regulator that binds to promoter-associated CpG islands via its CxxC zinc-finger domain, protecting them from DNA methylation. It also helps establish transcriptional programs essential for development by recruiting the non-canonical Polycomb Repressive Complex 1.1 to lineage-specific genes. Heterozygous variants in KDM2B were recently associated with a neurodevelopmental disorder. Notably, some individuals with variants in the CxxC domain also exhibited congenital heart, kidney and/or structural eye anomalies. By screening 706 families with developmental eye disord

Choosing Local Flaps Versus Occlusive Dressings in Fingertip Amputations: A Systematic Review and Meta-Analysis With Proposed Algorithm.

PURPOSE: Fingertip amputations present a reconstructive challenge because of the varying degrees of tissue loss, bony exposure, and functional impact. Often, providers must decide between occlusive dressings and local flap reconstruction. This systematic review aims to analyze outcomes of both treatment modalities to determine the benefits and risks of each. METHODS: We conducted a systematic review and meta-analysis in accordance with Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) guidelines. Studies directly comparing occlusive dressings and local flap reconstruc

Characteristics of repeated untethering pattern in patients with myelomeningocele from infancy to adolescence.

OBJECTIVE: Radiological evidence of secondary spinal cord tethering has been observed in patients with myelomeningocele (MMC), even after primary surgical treatment. A significant number of these patients may develop symptomatic tethered cord syndrome (TCS) several times during the clinical course, with worsening of preexisting neurological deficits, or bladder or bowel function. Early untethering is necessary to prevent further progression of symptoms. The aim of this study was to investigate factors influencing postoperative outcome of spinal cord untethering (SCU) on recurrent TCS. METHODS
