Trends in Hepatitis D Virus Prevalence Among HBsAg-Positive Individuals With and Without HIV in Italy: A Converging Epidemiology.

BACKGROUND AND AIMS: Hepatitis D virus (HDV) infection has historically been more prevalent among people with HIV (PWH) due to shared transmission routes. Whether HIV status still identifies a subgroup at higher risk of HDV in contemporary settings remains unclear. METHODS: We performed two cross-sectional analyses assessing anti-HDV prevalence among HBsAg-positive PWH enrolled in the ICONA cohort in 2009-2012 and 2019-2023, compared with HIV-negative individuals from the MASTER and PITER cohorts. RESULTS: Anti-HDV prevalence among PWH declined from 19.1% to 12.5%, converging with that obser

Occult hepatitis B infection - unveiling clinical burden and new diagnostic strategies.

INTRODUCTION: Occult B infection (OBI) represents a mild form of hepatitis B characterized by low-level HBV-DNA in the absence of positive serum HBsAg. OBI is associated with liver disease and HBV transmissibility, although to a lesser extent than overt (HBsAg+) chronic hepatitis B. AREAS COVERED: Residual HBV replication in the liver of OBI patients has been associated with i) increased risk for cirrhosis and/or liver cancer, ii) HBV transmission to recipients of organs or hemoderivates, iii) overt HBV reactivation under immunosuppression, and iv) extrahepatic manifestations. We searched PubM

Comparative performance of the Siemens Atellica and Abbott Architect assays for HBsAg quantification in patients with chronic hepatitis B or hepatitis Delta.

BACKGROUND: Quantitative hepatitis B surface antigen (qHBsAg) is a routine marker for monitoring patients with chronic hepatitis B (CHB) or Delta (CHD). New commercially available assays require independent evaluation in clinical laboratories before being adopted for routine use. This study assessed the performance of the new Siemens Atellica IM Quantitative HBsAg (QHBs) assay in patients with CHB or CHD. METHODS: Over a six-month period, all serum specimens from patients with CHB or CHD that were submitted to the laboratory for qHBsAg testing were prospectively analyzed using both the histor

Combination therapy with tobevibart and elebsiran potently reduces hepatitis B virus surface antigen levels in preclinical in vivo models.

RNA interference (RNAi) therapeutics targeting hepatitis B virus (HBV) RNAs and monoclonal antibodies (mAbs) targeting HBV surface antigen (HBsAg) represent potential strategies for enabling functional cure in chronic HBV patients. Tobevibart (VIR-3434) is an investigational, Fc-engineered human mAb that targets HBsAg with pan-genotypic neutralizing activity. Elebsiran (VIR-2218) is an investigational small interfering RNA targeting a conserved region of the HBV genome. The in vitro antiviral activity of elebsiran was assessed in HBV-infected primary human hepatocytes and hepatoma cells and sh

Viral Hepatitis B, C and Delta in Greenland: A National Cross-Sectional Study.

Viral hepatitis B infection is considered endemic in Greenland as in other Arctic areas. However, updated data are warranted after the implementation of screening, vaccination and treatment programmes during the past decades. We aimed to investigate the prevalence and disease characteristics of chronic viral hepatitis B (HBV), C (HCV) and delta (HDV) in Greenland. We performed a cross-sectional nationwide study using data from the electronic medical records extracted from 2014 to 2023. We identified 299 patients with HBV (positive of HBsAg) and 45 patients with HCV (positive of HCV-Ab and/or H

Consensus document on the diagnosis, management, and treatment of chronic hepatitis B virus and hepatitis D virus infection in Spain.

Chronic infection by hepatitis B virus (HBV) and hepatitis D virus (HDV) continues to pose a major public health challenge in Spain due to its high morbidity and mortality and the risk of progression to cirrhosis and hepatocellular carcinoma. Although effective tools exist for the control of hepatitis B and recent therapeutic advances have been made for hepatitis D, significant inequalities persist in early diagnosis, access to treatment, and appropriate patient follow-up. This consensus document has been jointly developed by the Spanish Association for the Study of the Liver (AEEH), the Spani

Hepatitis D Infection: a puzzle still unsolved?

Hepatitis D Virus (HDV) infection, also known as "delta" hepatitis, remains one of the most severe forms of viral hepatitis. First discovered in 1977, HDV affects approximately 5% of individuals with chronic Hepatitis B Virus (HBV) infection, translating to around 10-20 million people globally. Over 40 years after the discovery of HDV, our understanding of the virus and its associated disease remains incomplete. However, recent advances in therapeutic research offer new hope for patients with Hepatitis D. With the ongoing development of targeted therapies and the combination of various pharmac

How to test for hepatitis D virus infection: evidence and rationale to inform policy recommendations and future directions.

Chronic hepatitis D virus (HDV) coinfection in people with chronic hepatitis B is associated with rapid progression to liver cirrhosis and high mortality. In 2020, the estimated global HDV burden among people with chronic hepatitis B was 12 million people, with a seroprevalence of 4·5%. Treatment options are now evolving with the introduction of bulevirtide; however, testing for HDV remains very low. In 2024, WHO published comprehensive guidelines on the management of people with chronic hepatitis B, including recommendations for the first time on who to test and how to test for HDV infection.

Who to test and reflex testing for hepatitis D virus infection: evidence and rationale to inform policy recommendations and future directions.

Chronic hepatitis D coinfection affects an estimated 12 million people globally, with higher prevalence in low-income and middle-income countries, especially the African and Western Pacific regions. It leads to accelerated progression to liver cirrhosis, hepatocellular carcinoma, and increased mortality. The treatment landscape is evolving with the introduction of bulevirtide, but hepatitis D virus (HDV) testing uptake and case-finding remain low. The 2024 WHO guidelines on the management of chronic hepatitis B provide recommendations for the first time on who to test and how to test for HDV i

Hepatitis D Virus Pathogenesis: A Sense of Complications.

Hepatitis D virus (HDV) is a satellite RNA virus of the hepatitis B virus (HBV) infecting an estimated 12 million people worldwide. Chronic HDV infection is causing the most severe form of chronic viral hepatitis, leading to a rapid progression of chronic inflammation to fibrosis, cirrhosis, liver decompensation and cancer. The detailed mechanisms responsible for HDV pathogenicity and its contribution to the development of hepatocellular carcinoma (HCC) are not clearly understood. This review aims to summarize the current knowledge of HDV-induced injuries, which gradually accumulate and increa

CD16+ γδ T cells mediate antibody-dependent cellular cytotoxicity and associate with viral control in chronic hepatitis B virus infection.

BACKGROUND: Chronic hepatitis B virus (HBV) infection is characterised by immune dysfunction. While conventional T cell responses have been extensively studied, the role of γδ T cells, innate-like cytotoxic lymphocytes enriched in the liver, remains incompletely understood. OBJECTIVE: To characterise γδ T cell subsets in HBV infection and assess their association with viral control and antibody-dependent cellular cytotoxicity (ADCC). DESIGN: Peripheral blood from patients with chronic (n=83) and acute (n=16) HBV infection, healthy controls (n=31), and cord-blood donors (n=3) was analysed usi

Seroprevalence and Molecular Epidemiology of Hepatitis B and D Viruses in Mauritania: a Systematic Review and Meta-Analysis.

Mauritania has a high prevalence of chronic hepatitis B virus (HBV) and hepatitis D virus (HDV), posing a major public health burden. A comprehensive analysis of seroprevalence, transmission dynamics, and molecular epidemiology would help inform strategies for public health interventions. This study systematically reviews the published literature in the field between 1983 and 2024, employing PRISMA 2020 guidelines. Where insufficient data were available for meta-analyses on specific inquiries, summative descriptions were included. Eight studies involving 19,781 participants were included. The

Clinical and Pathophysiological Considerations Related to the Impact of Bulevirtide, a New Entry Inhibitor, in HBV-HDV Infection.

This review critically examines the inhibition of viral entry as an emerging disease-modifying strategy in chronic hepatitis B (HBV) and delta (HDV) virus infection, with particular emphasis on bulevirtide, the first-in-class of the sodium taurocholate cotransporting polypeptide entry inhibitor. This paper summarizes the analysis of 7 clinical trials that either underpinned the registration of bulevirtide or are important European real-life trials. We synthesize virological, pathophysiological and clinical evidence, highlighting the impact of this novel bulevirtide-based therapy on virological

siRNA JNJ-73763989 decreases HBsAg and HDV RNA in NA-treated hepatitis D patients but may cause ALT flares.

BACKGROUND & AIMS: HDV requires HBsAg for viral assembly. The siRNA JNJ-73763989 (JNJ-3989) targets all HBV transcripts and reduces HBsAg in patients with chronic hepatitis B. We evaluated whether HBsAg reduction with JNJ-3989 leads to a decline in HDV RNA and ALT in patients with chronic hepatitis D (CHD). METHODS: REEF-D, a two-part, phase II, randomized, double-blind, placebo-controlled study, enrolled 22 adults with CHD in Part 1. Participants were randomized 4:1 to receive JNJ-3989 (100 mg subcutaneously every 4 weeks) + nucleos(t)ide analogue (NA) for 144 weeks (active arm, n = 17) or f
