Engineered monoclonal antibody tobevibart enhances HBsAg capture by Fc receptor-positive cells and activates HBV-specific T cells.

BACKGROUND & AIMS: Effective immune targeting is likely essential for achieving functional cure of chronic hepatitis B (CHB). Tobevibart, a human monoclonal antibody against hepatitis B surface antigen (HBsAg), neutralizes HBV and HDV. This study aimed to characterize the effects of the engineered GAALIE Fc of tobevibart on HBV immune responses. METHODS: We studied tobevibart and its equivalent, HBC34∗-GAALIE, in vitro using electron microscopy, reporter cells, and primary human or mouse immune cells to assess dynamics of HBsAg reduction, dendritic cell (DC) activation, and T-cell stimulation

Diagnosed Patients With Chronic Hepatitis B and Delta Virus in Italy in 2024: An Estimation From a National Real-World Database.

BACKGROUND AND AIMS: Hepatitis B (HBV) and Hepatitis Delta virus (HDV) infection have undergone significant changes in Italy over the past few decades, but reliable and updated prevalence of chronic hepatitis B (CHB) and Delta (CHD) data are lacking. The aim of the study was to describe the epidemiology of CHB and CHD in Italy in 2024, based on real-world data. METHODS: The number of patients with a healthcare expenditure exemption for CHB (016.070.32) and CHD (016.070.33) was obtained from 21 Regional Health Authorities. To understand how many CHB or CHD patients did not have these specific

High Prevalence of Cirrhosis or Hepatocellular Carcinoma at Hepatitis Delta Infection Diagnosis Reflects Alarming Delays in Testing.

Delays in timely diagnosis and treatment of hepatitis delta virus (HDV) contribute to more severe liver disease at presentation. We aim to evaluate the prevalence and predictors of advanced liver disease at presentation among a national cohort of United States (U.S.) Veterans co-infected with chronic hepatitis B (CHB) and HDV. We retrospectively evaluated all U.S. Veterans with chronic HBV from 1/1/2010 to 12/31/2024 who underwent anti-HDV testing to evaluate the proportion who had advanced liver disease (cirrhosis, cirrhosis-related complications and hepatocellular carcinoma) at the time of H

Hepatitis D Virus Positivity among Patients from Liberia with Chronic Hepatitis B Virus Infection.

Hepatitis D virus (HDV) infection in individuals with chronic hepatitis B virus (HBV) is associated with accelerated liver disease and hepatocellular carcinoma (HCC). In this retrospective cohort of 1,337 HBsAg-positive patients at an urban safety-net hospital in Minneapolis, Minnesota, 41.6% were tested for anti-HDV antibodies, of whom 8.8% were positive. Among those testing positive, 28% had polymerase chain reaction-confirmed HDV infection. HDV Ab-positive patients were predominantly of African descent (88%), with 71% being Liberian. In comparison with HDV Ab-negative patients, those with H

Bulevirtide for chronic hepatitis delta: from clinical trials to real life data: an expert opinion report.

Hepatitis D virus (HDV) is a small RNA virus that requires Hepatitis B Surface Antigen (HBsAg) for its envelope. Eight genotypes with more than 80 % sequence homology and many subgenotypes have been described. Worldwide prevalence of chronic hepatitis delta (CHD) is estimated at about 5 % of chronic hepatitis B cases, translating to 15-20 million individuals. The diagnosis of HDV infection involves presence of antibodies to hepatitis D antigen (anti-HDV antibodies). Anti-HDV total antibody indicates HDV exposure (not infection). To document infection; the patient needs to undergo PCR testing a

Unexpectedly Low Prevalence of Hepatitis Delta Virus Infection in Southern Viet Nam.

Viet Nam faces a significant burden of viral hepatitis-associated liver disease, but the contribution of HDV, the most severe form of viral hepatitis, remains underinvestigated. HDV is substantial in the Northern and Central regions, but has not been documented in the South of Viet Nam. To investigate HDV prevalence and its association with severe liver disease in Southern Viet Nam, we used the standardized assay (LIAISON® XL Anti-HDV) to detect HDV antibodies (anti-HDV) in 721 HBsAg positive individuals with hepatitis flare (n = 158), liver cirrhosis (LC) (181), hepatocellular carcinoma (HCC)

Hepatitis D Virus Seroconversion Rate Among People With Chronic Hepatitis B Virus Infection in France and The Gambia (Inci-D).

BACKGROUND: Superinfection with the hepatitis D virus (HDV) leads to a more aggressive form of chronic hepatitis B. While around 5% of hepatitis B surface antigen (HBsAg)-positive individuals are estimated to be hepatitis B virus (HBV)-HDV dually infected globally, the time point of superinfection is unknown and repeated HDV testing is not yet supported by international guidelines. Inci-D is a post hoc analysis from 2 prospective cohorts to evaluate the HDV superinfection rate. METHOD: The Inci-D cohort consists of 2 HBV cohorts of clinical meta-data and stored plasma samples or dried-blood s

The cascade of care for hepatitis B - any lessons from the HIV field?

INTRODUCTION: HIV and hepatitis B continue to be global public health challenges. Each virus causes 1.2 million new infections annually. By 2025, roughly 40 million people live with HIV, and over 250 million have chronic hepatitis B. AREAS COVERED: Using PubMed, we analyzed approaches to control HIV and HBV epidemics. Whereas no vaccine exists to prevent HIV acquisition, successful protective HBV vaccines are available. The high incidence of HBV is largely explained by low immunization coverage and poor HBV screening. Linkage to care and sustained provision of antivirals remain suboptimal for

Hepatitis D virus infection prevalence in persons with human immunodeficiency virus and hepatitis B virus coinfection in Germany.

PURPOSE: People with HIV (PWH) who have chronic hepatitis B virus (HBV) coinfection are at increased risk of also having hepatitis D virus (HDV) infection given the shared transmission pathways. The current prevalence of HDV in Germany among people with HIV/HBV, however, is unknown. The aim of this study was to determine the percent with HDV screening as well as the current HDV prevalence among German PWH with HBV coinfection and underlying risk factors for HDV infection. METHODS: 21 German HIV treatment centers (6 university clinics, 15 private practices) recruited all people with a confirme

[Diagnosis and treatment of viral hepatitis B and D in 2024].

Despite the availability of vaccines, hepatitis B remains a significant cause of fulminant hepatitis, liver cirrhosis and hepatocellular carcinoma worldwide. The increase in reported hepatitis B cases in Germany is attributed to factors such as immigration and the hepatitis B surface antigen (HBsAg) screening introduced in 2020 as part of health check-ups. The indication for treatment depends on various factors, including the level of hepatitis B virus (HBV) DNA and inflammatory activity. Nucleos(t)ide analogues are the preferred treatment option, but functional cure, defined as HBsAg loss, is

Characteristics and outcome of anti-hepatitis D virus positive patients with hepatocellular carcinoma.

BACKGROUND & AIMS: Chronic hepatitis D virus (HDV) often leads to end-stage liver disease and hepatocellular carcinoma (HCC). Comprehensive data pertaining to large populations with HDV and HCC are missing, therefore we sought to assess the characteristics, management, and outcome of these patients, comparing them to patients with hepatitis B virus (HBV) infection. METHODS: We analysed the Italian Liver Cancer database focusing on patients with positivity for HBV surface antigen and anti-HDV antibodies (HBV/HDV, n = 107) and patients with HBV infection alone (n = 588). Clinical and oncologica

A regular screening for hepatitis delta virus among chronic hepatitis B carriers improves the diagnostic of this infection and of subsequent cirrhosis development.

BACKGROUND AND OBJECTIVE: The prevalence of Hepatitis Delta Virus (HDV) is underestimated and the assessment of fibrosis is recommended for this infection. We tested the diagnostic impact of an annual screening for HDV serology in Hepatitis B Surface Antigen (HBs Ag) chronic carriers and followed the progression of fibrosis in these patients. METHODS: Between January 2014 and October 2021, we annually tested all chronic HBs Ag-positive patients for HDV antibody (HDV Ab). Each HDV Ab positive patient underwent annually repeated elastometry. Patients with detectable HDV RNA levels (group 1) wer

An updated assessment of hepatitis delta prevalence among adults in Canada: A meta-analysis.

Foreign-born (FB) persons represent a large proportion of adults with chronic hepatitis B (CHB) in Canada due to higher prevalence rates in countries of birth for FB persons. Suboptimal awareness and low rates of hepatitis delta virus (HDV) testing contribute to underdiagnosis and gaps in accurate estimates of Canada HDV prevalence. We aim to provide an assessment of CHB and HDV prevalence in Canada using a comprehensive literature review and meta-analysis. A comprehensive literature review of articles reporting HBsAg seroprevalence and anti-HDV prevalence was conducted to calculate country-sp

Toward the elimination of hepatitis B: networking to promote the prevention of vertical transmission of hepatitis B virus through population-based interventions and multidisciplinary groups in Africa.

The WHO African Region had 81 million people with chronic hepatitis B in 2019, which remains a silent killer. Hepatitis B virus (HBV), hepatitis delta virus (HDV), and HIV can be transmitted from the mother to child. If the HBV infection is acquired at infancy, it may lead to chronic hepatitis B in 90% of the cases. WHO reports that 6.4 million children under 5 years live with chronic hepatitis B infection worldwide. The prevention of mother-to-child transmission (PMTCT) of HBV is therefore critical in the global elimination strategy of viral hepatitis as we take lessons from PMTCT of HIV prog

Prospects for Controlling Hepatitis B Globally.

Infection with the hepatitis B virus (HBV) is highly prevalent globally. Over 250 million people suffer from chronic hepatitis B, and more than 800,000 patients die each year due to hepatitis B complications, including liver cancer. Although protective HBV vaccines are recommended for all newborns, global coverage is suboptimal. In adults, sexual transmission is by far the most frequent route of contagion. The WHO estimates that 1.5 million new HBV infections occur annually. Oral nucleos(t)ide analogues entecavir and tenofovir are the most frequent antivirals prescribed as HBV therapy. Almost

Current and future use of antibody-based passive immunity to prevent or control HBV/HDV infections.

With the increasing momentum and success of monoclonal antibody therapy in conventional medical practices, there is a revived emphasis on the development of monoclonal antibodies targeting the hepatitis B surface antigen (anti-HBs) for the treatment of chronic hepatitis B (HBV) and hepatitis D (HDV). Combination therapies of anti-HBs monoclonal antibodies, and novel anti-HBV compounds and immunomodulatory drugs presenting a promising avenue to enhanced therapeutic outcomes in HBV/HDV cure regimens. In this review, we will cover the role of antibodies in the protection and clearance of HBV infe

Cell Culture Models for Hepatitis B and D Viruses Infection: Old Challenges, New Developments and Future Strategies.

Chronic Hepatitis B and D Virus (HBV and HDV) co-infection is responsible for the most severe form of viral Hepatitis, the Hepatitis Delta. Despite an efficient vaccine against HBV, the HBV/HDV infection remains a global health burden. Notably, no efficient curative treatment exists against any of these viruses. While physiologically distinct, HBV and HDV life cycles are closely linked. HDV is a deficient virus that relies on HBV to fulfil is viral cycle. As a result, the cellular response to HDV also influences HBV replication. In vitro studying of HBV and HDV infection and co-infection rely
