Dissatisfaction with the Keller-type operation and the arthrodesis of the first metatarsophalangeal joint in hallux rigidus led to the use of a wedge osteotomy method at the base of the proximal phalanx. Short-term results in 8 patients suggest that further testing of this method should be worthwhile.

Twenty patients with hallux rigidus were evaluated following cheilectomy. The surgical procedure involved resection of proliferative bone at the metatarsophalangeal joint to allow at least 45 degrees of passive dorsiflexion. The follow-up ranged from 7 months to 156 months. Postoperatively, there was minimal progression of the degenrative process. Subjectively, there was uniform patient satisfaction with the procedure.

Abnormalities in the hallux valgus foot and changes after surgery were investigated by measuring the distribuiion of load on the foot in walking. Hallux valgus was associated with reduced load imposed on the toes, and on the medial side of the forefoot, compared with a large sample of healthy feet. Abnormalities correlated with the degree of the deformity. Both Keller's operation and a wedge displacement osteotomy of the first metatarsal not only failed to restore normal loading but increased the abnormalities of loading seen preoperatively. A large decrease in the angle between first and seco

A prosthesis has been designed with which to resurface the painful, degenerated first metatarsophalangeal joint. Clinical evaluation thus far has indicated satisfactory results in 16 of 18 instances. This procedure will probably give the best results in the older patient with hallux rigidus, but the length of follow-up is as yet too short to allow firm opinions to be formed.

Since 1975 the usual treatment of hallux valgus and hallux rigidus at the Medical High School, Lübeck, has been arthrodesis of the metatarophalangeal joint of the great toe. We report 48 operations performed between 1975 and 1977 on 35 patients with a follow-up of 3-32 months. The operative technique, using the "dynamic compression plate" is described in detail. The patient's assessment and the clinical and radiological situations were recorded. There were only three poor results. Thus, arthrodesis of the metatarsophalangeal joint is considered to be a reliable therapeutic method in hallux val

Compression arthrodesis of the first metararsophalangeal joint using a modification of the technique of McKeever is described. This operation consistently produces the best results for hallux valgus and hallux rigidus of the great toe. The operation has been found to be applicable where there have been previous attempts at surgical correction. It also produces good results in patients having rheumatoid arthritis when there is minimal involvement of the interphalangeal joint.

The clinical, radiological and pathological features of hallux rigidus affecting nine toes (in seven patients) are described. Characteristic chondral and osteochondral lesions are seen to occur at a specific site on the metatarsal head, and account for the limitation of dorsiflexion but relatively unrestricted plantarflexion typical of hallux rigidus. Radiologically these lesions are often missed because they are mainly cartilaginous and are later obscured by secondary degenerative changes. Histological evidence indicates a traumatic aetiology and a mechanism of injury is suggested.

Various prostheses for total replacement of the first metatarsophalangeal joint for painful hallux valgus and hallux rigidus are briefly discussed. Altogether, the results of eighty-six replacements in sixty-nine patients have been recorded after an average interval of two years. In seventy-eight operations a Silastic* prosthesis as designed by Swanson for the replacement of metacarpophalangeal joints was used, with no case of fracture or deep infection up to date. Overall, the assessment of pain showed that 98 per cent of operations gave either complete or considerable relief. For hallux valg

Freiberg's disease or osteochondrosis of the second metatarsal head may necessitate surgical intervention because of pain. Several surgical procedures have been recommended. A new surgical procedure similar to the present treatment of hallux rigidus with a silicone rubber implant is described. The procedure is simple and results were cosmetically appealing and functionally effective.

The purpose of this report is to consider involvement of the great toe by rheumatoid arthritis, defining significant deformities, describing clinical patterns and discussing the pathomechanics of these findings, in what is an ongoing disease. 200 consecutive patients admitted to hospital with classical or definite rheumatoid arthritis were screened for pain or deformity of the great toe. Feet that had undergone previous surgery or had other underlying pathology were excluded from the series. 194 feef were found to have halluceal involvement. Although hallux valgus was the commonest deformity i

A series of 46 patients who underwent 78 first metatarso-phalangeal operations is reported. The procedure for the Stone operation is detailed. The Stone operation is a satisfactory procedure in the surgical treatment of hallux valgus and hallux rigidus.

The study of 92 cases of osteoarthrosis of the first metatarso-phalangeal joint (or hallux rigidus) shows the importance of two aetiological factors : excessive length of the first metatarsal in the so-called primitive osteoarthrosis, and traumatism for the secondary forms. The results of arthrodesis are rather better than these of arthroplasty and exostosectomy, mainly in men after 50 years.

Many of the fair and poor results are directly traceable to errors in technique. Patient selection is also important. The operation can be done in adolescents since there is no growth plate at the distal end of the first metatarsal. We do not currently recommend this procedure for those over 60, for those with first metatarsophalangeal osteoarthritis or hallux rigidus or for those with moderate or severe rheumatoid arthritis. For these patients we usually do a Keller excisional arthroplasty. Our use of the osteotomy-bunionectomy operation for the patient with hallux valgus with an associated m
