[Reconstructive surgery of sequelae of compartment syndrome of the lower leg and/or foot. Presentation of a new classification].

The sequelae of an undiagnosed insufficiently treated or unpreventable (by crush injury) compartment or postischemic syndrome, most often after lower leg fracture or popliteal artery injury, are caused by necrosis and contracture of the extrinsic foot muscles. Therefore claw toes, pes equinus or other forms, such as a severe pes equino varus related to the compartment involved will decide the kind of foot deformity. In cases of a combined compartment syndrome of the lower leg and foot, not only the extrinsic but also the intrinsic muscles especially the short flexors are involved, leading to e

[Resection of navicular bone for severe midfoot deformity in children].

PURPOSE OF THE STUDY: The navicular bone lies at the apex of the deformity in severe talipes planovalgus with forefoot abductus, in the residual cavus of congenital talipes equinovarus, in certain cases of congenital convexity, and in certain types of neurological equinovarus. Resection of the navicular bone can be proposed to correct certain deformities. MATERIAL AND METHODS: This series included 15 feet operated on in 13 children from 1980 to 2003. The deformity to be corrected was a residual cavus after surgery for congenital talipes equinovarus (five feet), plantar and medial dislocation

Fibrodysplasia ossificans progressiva.

Fibrodysplasia ossificans progressiva, a rare genetic disabling disease characterized by heterotopic bone formation, is of special interest for general medicine since the bone morphogenetic proteins (especially BMP-4) involved in its pathogenesis are known to play a role in skeletal morphogenesis, and the gene antagonist to BMP-4 noggin might be useful in preventing lamellar bone formation. We present two cases with characteristic musculo-skeletal abnormalities and histopathological features (inflammatory infiltrates which destroyed muscle tissue replacing it by proliferating fibroblasts). [Distal shortening osteotomy of the metatarsals using the Weil technique: surgical treatment of metatarsalgia and dislocation of the metatarsophalangeal joint].

PURPOSE: Weil technique of the distal shortening osteotomy of metatarsal is a relatively new method of the surgical treatment of metatarsalgia and dislocation of metatarsophalangeal (MTP) joints. A retrospective study evaluates the first experience in Weil osteotomy at the authors' department. MATERIAL: Between May 1999 and the end of 2000 twelve patients (14 feet) were operated on in which the Weil technique of the osteotomy was performed on 28 metatarsals. Indication for the surgery was chronic metatarsalgia with dislocation of the MTP joint, with excessive length of one or more lesser meta

Foot deformities in rheumatoid arthritis and relevance of disease severity.

OBJECTIVE: To investigate foot deformities in rheumatoid arthritis (RA) in relation to the disease severity. METHODS: Radiographs of 100 weight bearing feet of 50 patients who had had RA for >10 years (mean 13.5 years) were studied. The patients were classified into 2 study groups according to the severity of disease. We measured hallux valgus angle (HVA), intermetatarsal angle between first and 2nd (M1/2), and intermetatarsal angle between first and 5th (M1/5) on anteroposterior (AP) radiographs, as well as calcaneal pitch (CP) and first metatarsal pitch (MP) on lateral radiographs. The diff

Hallux flexion deformity secondary to entrapment of the flexor hallucis longus tendon after fibular fracture.

This article presents a case of entrapment of the flexor hallucis longus tendon after open reduction and internal fixation of a Weber C ankle fracture resulting in interphalangeal joint contracture of the hallux. Pathology involving other tendons at the foot and ankle associated with ankle fractures is reviewed. Other scenarios of flexor hallucis longus pathology are discussed. Flexor hallucis longus anatomy, as related to distal fibular fractures, is outlined, and a recommendation is made to consider flexor hallucis longus entrapment as a cause of hallux dysfunction after open reduction and i

[Long-term results with women's feet].

When ladies complain of painful feet, they usually have hallux valgus. They can hardly accept that their ailment has something to do with too narrow shoes; therefore, it is not easy to explain to them, that usually the problem could be solved with comfortable large foot-wear and that an operation is not always necessary. On the other hand, operations for hallux valgus must not be labelled simply as a luxury procedure. Many patients can be relieved of pain and given the opportunity to wear acceptable fashionable shoes, provided that the appropriate method is chosen for each patient individually

A modification of the Regnauld procedure for hallux limitus.

Surgical correction of hallux limitus can be challenging to the foot surgeon. The procedures available to date are numerous. Satisfactory results require proper candidate selection, and other procedures must be weighted carefully. The Regnauld procedure has become an accepted technique in this extensive surgical armamentarium. This manuscript describes a means of fixation that eliminates many of the complications associated with the original nonfixed technique, and allows for early range of motion to be initiated.

[Post-operative iatrogenic hallux varus. Surgical treatment. Apropos of 19 cases].

The post operative iatrogenic hallux varus associates varus, big toe's dorsal flexion and supination. The pathogeny is dominated by musculo-tendinous lack of balance created by the initial operation and excessive exostosectomy. The authors have nineteen times surgically corrected this deformation. The procedure included in all cases, a medial arthrolysis; five times associated with a screwed capito-metatarsal osseous shelf (or graft) (when there was a metatarsal's head maiming); thirteen times associated with a metatarso-phalangeal arthrodesis (in cases of impaired joint surfaces). [Management of hallux varus].

Authors describe the congenital and acquired forms of the rarely occurring hallux varus on the basis of the material of 14 patients, treated in 1951-87 on the Orthopaedic Department of the Semmelweis University Budapest. In 11 cases the deformity was congenital and in the majority bilateral, multiple, together with other foot deformities.

Footcare for very elderly people: a community survey.

We have assessed foot problems of, and chiropody provision for, 96 people aged 80 years and over who were living at home. Seventy-seven per cent had difficulty cutting their toenails and 30% complained of pain in the feet. The commonest foot problems were corns or callus, abnormal nails, hallux valgus and other toe abnormalities. Only six subjects had normal healthy feet. Of the 47 people receiving chiropody, two-thirds were being seen privately. Private chiropody tended to be performed in the home and was more frequent than National Health Service (NHS) treatment. Fifteen subjects needed but

Reverse Austin osteotomy for correction of acquired static hallux adductus.

Hallux adductus, a horizontal plane deformity, often has an iatrogenic etiology especially as a complication of hallux abducto valgus surgery. A case at Sheridan Park Hospital is described in which the patient developed a steadily worsening static hallux adductus following foot surgery. She underwent further corrective procedures for the hallux adductus 11/2 years later. The authors conclude that if a mild static hallux adductus is observed in conjunction with bunionectomy or other surgery, it should be corrected at that time rather than some years later.

Late results after correction of hallux valgus deformity by basilar phalangeal osteotomy.

In a long-term follow-up study (average, 10.7 years) of 222 patients (351 feet), the results after proximal phalangeal osteotomy (the Akin procedure) for hallux valgus deformity were found to be unsatisfactory, both subjectively and objectively. The range of motion in the metatarsophalangeal joint was limited postoperatively in 90 per cent of the patients, and in 21 per cent there was recurrence of deformity. The appearance of the foot was poor in 75 per cent. Only 53 per cent of the patients expressed satisfaction with the result of the operation. There was a direct relationship between the d

Pathomechanics of congenital and acquired hallux varus: a clinical and anatomical study.

Hallux varus was observed in 69 feet among patients from a rural Indian population. This included three types of congenital hallux varus. The congenital "primary" type was the commonest and was not associated with any other foot deformity. Among the acquired forms was idiopathic hallux varus of middle age, which developed spontaneously in unshod persons and progressed gradually. Hallux varus following chronic arthritis was part of a consistent pattern of foot deformities in which the first ray collapsed in a zig-zag manner in two planes. Although the development of hallux varus is often due to

[Congenital and acquired foot deformities in the x-ray picture].

In addition to general radiologic aspects there are special orthopedic considerations in interpretation of x-rays of the foot. This is especially important for the diagnosis of orthopedic foot diseases. In clubfoot x-rays are useful for therapeutic planning and control. Even in the first months of life radiographs can show important disturbances of growth of the foot and displacement of the bones of the tarsus. In other congenital foot deformities x-rays are important for diagnostic reasons: they prove luxations or skeletal deformities. The most important acquired foot disease is the pronating

Treatment of symptomatic first metatarsal shortened by surgery.

There are several complications that can result from shortening of the first metatarsal in the treatment of bunion. Metatarsalgia and recurrent deformity can be quite disabling. The surgical technique described in this brief report provides improved support of the metatarsal heads in walking while decreasing pain and enables the patient to stand for longer periods of time. The procedure is reserved for salvage of those patients with severe pain and deformity and in whom other salvage procedures have failed.

McKeever arthrodesis for the painful hallux.

Three hundred nine McKeever arthrodeses for treatment of the painful hallux were performed on 234 patients at the Mayo Clinic from 1955 to 1980. Two hundred six cases were contacted, and 71 were personally examined and X-rayed. The average follow-up was 15 years (1 to 28 years). Ninety-one percent wore shoes within 2 months, 92% are painless, and 98% without other foot pathology are wearing dress shoes of their choosing. Complications, which consisted of failed fixation, infection, and recurrence of deformity, occurred in 8%. The screw fixation was removed in 30%. Eighty-six percent of the pat

Surgery of the forepart of the foot in rheumatoid arthritis.

The purpose of this paper is to introduce a surgical approach to treatment of severe forefoot deformities of rheumatoid arthritis. Briefly, the surgery consists of base of the first metatarsal osteotomy to correct metatarsus primus varus, and metatarsal head resection beginning with the fifth metatarsal and carried around in crescentic fashion through the necks of the other metatarsals, so that as an end result the great toe is the longest, the second next to the longest, etc. The short extensor tendons are dissected to their insertions and, since they have drifted laterally into the "valleys"

Results of modified Robert Jones operation for clawed hallux.

The results of modified Robert Jones operation for clawed hallux carried out on 36 feet were reviewed by clinical and radiological investigations. Twenty-nine feet (80 per cent) achieved good results while 3 feet (8-5 per cent) obtained fair results and in 4 feet (11 per cent) the results were poor. Better results were obtained in the poliomyelitis cases than in the other groups. Correction of deformity appears to be achieved by the transferred extensor hallucis longus elevating the first metatarsal neck and by the flexor hallucis longus flexing the rigid toe obtained by interphalangeal arthro
