Rising Revision Burden in Total Wrist Arthroplasty: National Medicare Trends and Projections to 2060.

BACKGROUND: Total wrist arthroplasty (TWA) is an alternative to arthrodesis for end-stage wrist arthritis, offering preserved wrist motion and functional improvement. Historically limited by high complication and revision rates, TWA has seen renewed interest with advances in implant design and expanding indications beyond rheumatoid arthritis. The purpose of this study was to analyze national trends in TWA utilization and project future procedure volumes for both primary and revision TWA through 2060 using Medicare data. METHODS: Procedure volumes for Medicare Part B beneficiaries undergoing

Median nerve shape and position in response to varied wrist postures during pinch.

This study investigated changes in median nerve flattening ratio (FR) and transverse position in the carpal tunnel across wrist flexion-extension (FE) postures during pinch in healthy adults. We hypothesized that deviated wrist FE postures from neutral would increase FR and alter nerve transverse position, and that pinch force would increase FR and amplify postural effects on position. Eleven healthy participants underwent ultrasound transverse imaging at the distal carpal tunnel of the right hand with the wrist splinted in three 30° extension, neutral, and 30° flexion with neutral radioulnar

Evaluating Circle-Tracing Performance Using Machine Learning in a Haptic Interaction Framework.

Hand and wrist function is commonly assessed using qualitative clinical tests that provide limited objective information and data in motor performance over time. Advances in haptic technology and machine learning provide new opportunities to develop more quantitative approaches. In this study, we developed a quantitative circle-tracing assessment using a haptic pen. The primary objective was to use unsupervised machine learning to compare low performance and high-performance repetitions to determine if there were significant differences in variability. Twenty-nine healthy participants complete

Capitate Reconstruction Using the Second Metatarsal Head for Avascular Necrosis: Long-Term Clinical Validation of an Anatomically Matched Osteochondral Reconstruction.

Avascular necrosis (AVN) of the capitate is a rare entity, and reconstruction of the capitate head remains challenging because restoration of lunocapitate articular congruity is difficult, particularly in advanced cases. A recent anatomical equivalence study demonstrated that the second metatarsal head (2MH) closely matches the capitate head with respect to key articular parameters. We report a case of capitate head reconstruction using a free vascularized 2MH osteochondral graft in a patient with recalcitrant AVN, who developed severe collapse of the capitate head following a failed pedicled

Beyond BMI: The Role of Anthropometric Measurements in Predicting Plantar Pressure Distribution and Gait Biomechanics.

Plantar pressure (PP) is a key biomechanical parameter for assessing foot function, gait abnormalities, and injury risk. Although body mass index (BMI) correlates with PP magnitude/distribution during gait, it is a coarse metric that collapses body composition into a ratio of mass to height, implicitly assuming uniform body proportions and mass distribution. The effects of individualized anthropometric parameters on PP signatures remain under-characterized, with a gap in incorporating body-shape measures beyond BMI. This exploratory study examined associations among anthropometric measurements

Bioinspired Concept Reinforced Superhydrophobic Textile-Based Strain Sensor with Stable Human Motion Monitoring in Air and Water.

Conventional textile-based strain sensors suffer from critical limitations in moisture-rich or aquatic environments due to their inherent hydrophilicity and weak interfacial adhesion between functional coatings and fibers, which severely restrict their practical application for human motion monitoring in amphibious scenarios. Herein, we proposed a bioinspired concept reinforced superhydrophobic textile (BCRST)-based strain sensor via a combined dipping-spraying technique, which integrated a biomimetic copolymer (HE-co-SV) as an adhesive bridge between a superelastic textile substrate and a flu

Robotic-assisted transduodenal ampullectomy for a neuroendocrine tumour: A step-by-step surgical video.

BACKGROUND: When endoscopic resection of ampullary neuroendocrine tumors (NETs) is not feasible due to tumour size or location, transduodenal ampullectomy can offer a parenchymal-sparing alternative to a formal pancreaticoduodenectomy (Whipple procedure) [1,2]. We present a robotic-assisted transduodenal ampullectomy (RATA) in a 71-year-old male with an incidental 14 mm ampullary NET found during routine screening. Preoperative 68Ga-DOTATATE PET-CT demonstrated intense somatostatin-receptor avidity (SUVmax 66.8) and absent FDG uptake. The MDT recommended surgical excision due to size and Ki-67

Diagnostic accuracy of standard practice wrist MRI for scapholunate ligament injuries: a retrospective arthroscopic correlation study.

INTRODUCTION: The scapholunate ligament is the most injured intercarpal ligament. Untreated tears can lead to carpal instability and subsequent radiocarpal and midcarpal osteoarthritis. While wrist arthroscopy remains the gold standard, the diagnostic performance of standard practice preoperative MRI in clinical practice warrants further investigation. METHODS: We retrospectively analysed patients who underwent preoperative wrist MRI followed by wrist arthroscopy between 2013 and 2021. MRI findings regarding scapholunate ligament integrity were compared with arthroscopic findings serving as r

First-in-human prospective pilot study of a bipolar dual-center total wrist arthroplasty: early mechanical stability and clinical outcomes.

PURPOSE: Total wrist arthroplasty (TWA) is limited by implant loosening, instability, and non-physiological load transfer, partly because most contemporary implants rely on a single center of rotation and do not preserve native wrist kinematics. This first-in-human pilot study evaluated the feasibility, early mechanical stability, and clinical outcomes of a novel bipolar dual-center TWA system designed to preserve radiocarpal and midcarpal motion. METHODS: This prospective single-center pilot case series included five consecutive patients with end-stage wrist arthritis treated with a bipolar

Outcomes of concomitant lacertus release and carpal tunnel release in patients with double crush syndrome: A cohort study.

Concomitant lacertus and carpal tunnel release improved symptoms in 51 patients with clinical double crush syndrome. Return-to-work duration was five weeks, and 73% of the patients were satisfied three months after treatment. These findings support consideration of single-stage decompression in appropriately selected patients.

Extrinsic risk factors for tendinopathy: bridging the basic and clinical research divide.

PURPOSE: Tendon injuries frequently affect the Achilles, rotator cuff, and the tendons of the wrist extensors originating at the lateral epicondyle. Until recently, terms for tendon degeneration, disease, and injuries were debated between the basic research and clinical communities, creating barriers to advances in knowledge. Aside from acute traumatic rupture in healthy tendon, in the current 'omic age, most tendon disease, including ruptures in chronic disease, exists along a tendinopathy spectrum. While intrinsic factors such as sex, genetics, and aging are unmodifiable, extrinsic factors i

Clinical Walking Speed, Free-Living Walking Cadence, and All-Cause Mortality.

IMPORTANCE: Walking speed has been shown to be associated with mortality and is conventionally obtained from short walking tests in clinical settings. Free-living walking parameters such as walking cadence can be derived from accelerometers. OBJECTIVE: To examine whether walking parameters measured in clinical and free-living settings are associated with comparable discriminative ability for all-cause mortality risk. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study, the Whitehall II accelerometer substudy in the UK, included a baseline from January 2012 to December 2013

Driving-Context Classification from Wearable Physiological and Motion Signals During Real-World Driving.

Wearable physiological sensing is widely used in driving research to infer drowsiness and stress, but whether driving context itself is reflected in wearable signals, independent of any drowsiness label, is rarely examined directly; any such signal reflects the driver's physiological response to context, not the road environment itself. We analyzed 1536 non-overlapping 60-s windows from 18 driving sessions completed by 11 drivers wearing an Empatica EmbracePlus across three fixed real-world routes (Rural, Highway, Urban; round-trip distances 22.4-47.1 km). Because these windows are correlated

Total wrist fusions alter scapular kinematics during functional task performance.

Total wrist fusion is a surgical procedure used to manage pain in late-stage arthritis, though it results in a complete loss of wrist mobility. This loss can lead to compensatory movement patterns in other upper limb joints, which may increase the risk of secondary musculoskeletal disorders. This study investigated scapular kinematics in wrist fusion patients compared to healthy controls. Eighteen participants (nine wrist fusion patients, nine age- and sex-matched controls) were recruited. Scapular motion was tracked using motion capture during seven functional tasks. Bilateral scapular kinema
