Development of a therapist-supervised flexibility and resistance exercise intervention for adults aged 50 years and over with a distal radius fracture in the Wrist Injury Strengthening Exercise (WISE) trial.

AIMS: The definitive Wrist Injury Strengthening Exercise (WISE) trial is a multicentre superiority randomized controlled trial (RCT) evaluating usual care advice plus a therapist-supervised flexibility and resistance exercise intervention versus usual care advice alone for adults aged ≥ 50 years with a distal radius fracture (DRF). This paper describes the development and delivery of the therapist-supervised exercise intervention evaluated in the definitive WISE RCT. METHODS: The intervention was developed iteratively in accordance with Medical Research Council (MRC) guidance for complex inte

Association between the dietary index for gut microbiota and fracture prevalence among women aged 50 years and older: a NHANES-based study with a preliminary exploratory pilot analysis.

INTRODUCTION: Gut microbiota has been implicated in bone metabolism, but whether a diet supporting gut microbial health is associated with fracture prevalence in older women remains unclear. This study aimed to examine the association between the dietary index for gut microbiota (DI-GM) and fracture prevalence in women aged 50 years and older. METHODS: We analyzed cross-sectional data from five NHANES cycles, including 6,318 women aged 50 years and older (mean age 63.8 ± 9.6 years). DI-GM scores were derived from 24-h dietary recalls based on 13 dietary components. Self-reported hip, wrist, o

All-Suture Fixation for Acute Turf Toe Injuries.

BACKGROUND: Turf toe is a hallux metatarsophalangeal joint (MTPJ) injury caused by hyperextension of the great toe with axial loading1,2. High-grade injuries may threaten return to sport and require surgery3,4. Here, we describe a capsular reconstruction of the great toe MTPJ with all-knotless suture anchors and suture tape augmentation for turf toe injuries, in isolation or in combination with a traumatic bunion, designed to reduce suture burden, minimize soft-tissue irritation, and facilitate recovery. DESCRIPTION: The procedure begins with the patient supine. A direct medial approach to th

A comparative study of distal chevron osteotomy and minimally invasive surgery for surgical correction of hallux valgus deformity.

BACKGROUND: Hallux valgus is a common forefoot deformity that significantly impacts patients' functional status and quality of life. This study aims to compare the mid-term clinical and radiographic outcomes of DCMO and MIS in hallux valgus correction. METHODS: A retrospective cohort study for 60 patients (34 DCMO, 26 MIS) was analyzed. Clinical outcomes were evaluated using the Manchester-Oxford Foot Questionnaire (MOXFQ) and Foot Function Index (FFI), while radiographic parameters such as hallux valgus angle (HVA), intermetatarsal angle (IMA), distal metatarsal articular angle (DMAA), and s

Morning lighting intervention reduces fatigue and improves sleep in Parkinson disease.

Fatigue and sleep disturbances are common in Parkinson Disease (PD) and significantly reduce quality of life, yet effective pharmacological treatments are limited. Light therapy offers a promising non-pharmacologic approach, but most lighting interventions have focused on improving sleep. Using a novel and easily administered morning lighting intervention, we previously demonstrated improvement in sleep duration. We have now extended the prior study through the addition of 23 individuals to investigate the intervention's effect on fatigue and sleep. In this single-arm intervention study, parti

Quantitative Whole-Leg Radiographic Assessment of Coronal Knee-Ankle Alignment: Associations with Talar Inclination and Exploratory Changes After Knee Osteotomy.

Background/Objectives: Standing whole-leg radiographs are central to coronal deformity assessment, but mechanical axis deviation (MAD) may not fully characterize the orientation of individual joint surfaces. We evaluated ankle joint orientation across coronal alignment phenotypes and, secondarily, explored descriptive changes after knee realignment osteotomy. Methods: This retrospective single-center study included 382 whole-leg radiographs from 374 patients and an exploratory longitudinal subgroup of 80 limbs from 75 patients before and after high tibial osteotomy (HTO) or distal femoral oste

The Effect of Metabolic Control of Rickets on Deformity Correction Following Hemiepiphysiodesis in Children With X-linked Hypophosphatemic Rickets.

BACKGROUND: X-linked hypophosphatemic rickets (XLH) is a genetic disorder of chronic hypophosphatemia leading to rickets and osteomalacia that often results in lower limb deformities requiring surgical correction. Hemiepiphysiodesis is commonly used to correct angular deformities in skeletally immature patients, but outcomes in XLH are variable, and the influence of metabolic control on surgical outcomes remains unclear. We evaluated the relationship between metabolic control of rickets, measured by serum alkaline phosphatase (ALP), and deformity correction after hemiepiphysiodesis in children

Skin Graft With Amniotic Membrane: Case Report of the Sandwich Technique in a Burn Hand.

Deep burns of the hand represent a major reconstructive challenge due to the critical functional role of the hand and the high risk of long-term disabling sequelae. The "sandwich" technique, combining an autologous skin graft with an amniotic membrane, has been described as a biological dressing with potential anti-inflammatory and wound-healing properties. A 32-year-old woman presented with a full-thickness burn involving the right palm and wrist after contact with an industrial thermal press. Following eschar excision, a full-thickness skin graft was harvested from the inner region of the ip

A combined classification system for preaxial foot polydactyly and its derived surgical strategies.

BACKGROUND: Pediatric polydactyly is a relatively common congenital condition of hand and foot, affecting physical and mental development of children. Hand polydactyly has been brought into focus recently. By contrast, the studies on foot polydactyly are limited. Among the types, there is little international research on preaxial foot polydactyly since it is a relatively rare type, and the domestic research on it is fewer. The purpose of our study is to propose a new classification for guiding operation. METHODS: We collected all patients with preaxial foot polydactyly who had preoperative ra

Anatomic Variations of the Upper Extremity: Effects on Ultrasound-Guided Peripheral Nerve Blockade.

Peripheral nerve blockade provides many advantages in the perioperative period for surgery on the shoulder, elbow, wrist, or hand. Aberrant anatomic structures or anomalous courses of nerves may create challenging conditions for use of ultrasound to accurately guide needles for nerve blocks. This paper presents a review of such anomalies affecting the conduct of various peripheral nerve blocks in the upper extremity. A comprehensive literature review was performed, utilizing the PubMed and Embase databases. Variations of anatomy of the brachial plexus and associated structures are described, a

Case Report: Bilateral pes planovalgus: an uncommon pediatric orthopedic presentation of Charcot-Marie-Tooth disease type 4B1.

Charcot-Marie-Tooth (CMT) disease is an inherited motor and sensory neuropathy that typically presents with cavovarus foot deformity. Pes planovalgus is uncommon in CMT and, while noted incidentally in multicenter cohort studies of CMT4B subtypes, has not been described as the predominant orthopedic manifestation in a published case report of genetically confirmed CMT4B1. We report the case of a 7-year-old girl who presented with progressive painful bilateral pes planovalgus, hallux valgus, frequent falls, and gait imbalance, despite otherwise normal developmental milestones. Neurological exam

Clinical and genetic features, surgical management, and longitudinal follow-up in two children with progressive osseous heteroplasia.

BACKGROUND: Progressive osseous heteroplasia (POH) is an ultrarare disorder characterized by heterotopic ossification that progresses from the skin and subcutaneous tissues into deeper connective tissues. Most affected individuals harbor heterozygous inactivating variants in GNAS, whereas postzygotic mosaic variants have rarely been described. Evidence to guide surgical management remains limited. METHODS: We retrospectively characterized two children with clinically diagnosed POH who underwent genetic testing and surgical treatment. Whole-exome sequencing was performed on peripheral blood an

Musculoskeletal symptoms and pain-response behaviors among university students in Palestine: a cross-sectional study.

BACKGROUND: Musculoskeletal (MSK) symptoms are a major contributor to disability worldwide, and university students may be increasingly affected because of prolonged sedentary behavior, intensive digital-device use, and sustained academic workloads. In Palestine, these risks may be compounded by psychosocial stressors and constrained healthcare access. This study assessed the prevalence of MSK symptoms and hand discomfort and examined associated behavioral, ergonomic, and device-use factors among Palestinian university students. METHODS: A cross-sectional study was conducted among 1,052 unive
