All-Suture Fixation for Acute Turf Toe Injuries.

BACKGROUND: Turf toe is a hallux metatarsophalangeal joint (MTPJ) injury caused by hyperextension of the great toe with axial loading1,2. High-grade injuries may threaten return to sport and require surgery3,4. Here, we describe a capsular reconstruction of the great toe MTPJ with all-knotless suture anchors and suture tape augmentation for turf toe injuries, in isolation or in combination with a traumatic bunion, designed to reduce suture burden, minimize soft-tissue irritation, and facilitate recovery. DESCRIPTION: The procedure begins with the patient supine. A direct medial approach to th

A comparative study of distal chevron osteotomy and minimally invasive surgery for surgical correction of hallux valgus deformity.

BACKGROUND: Hallux valgus is a common forefoot deformity that significantly impacts patients' functional status and quality of life. This study aims to compare the mid-term clinical and radiographic outcomes of DCMO and MIS in hallux valgus correction. METHODS: A retrospective cohort study for 60 patients (34 DCMO, 26 MIS) was analyzed. Clinical outcomes were evaluated using the Manchester-Oxford Foot Questionnaire (MOXFQ) and Foot Function Index (FFI), while radiographic parameters such as hallux valgus angle (HVA), intermetatarsal angle (IMA), distal metatarsal articular angle (DMAA), and s

Quantitative Whole-Leg Radiographic Assessment of Coronal Knee-Ankle Alignment: Associations with Talar Inclination and Exploratory Changes After Knee Osteotomy.

Background/Objectives: Standing whole-leg radiographs are central to coronal deformity assessment, but mechanical axis deviation (MAD) may not fully characterize the orientation of individual joint surfaces. We evaluated ankle joint orientation across coronal alignment phenotypes and, secondarily, explored descriptive changes after knee realignment osteotomy. Methods: This retrospective single-center study included 382 whole-leg radiographs from 374 patients and an exploratory longitudinal subgroup of 80 limbs from 75 patients before and after high tibial osteotomy (HTO) or distal femoral oste

Development and validation of a U-Net-based deep learning model for automatic measurement of the hallux valgus angle and intermetatarsal angle.

PURPOSE: To develop and validate a U-Net-based deep learning model for the automated measurement of the hallux valgus angle (HVA) and intermetatarsal angle (IMA) on weight-bearing dorsoplantar foot radiographs and to compare model performance with expert reference values. METHODS: A total of 1227 weight-bearing dorsoplantar radiographs were obtained. The dataset was divided into training (n = 858), validation (n = 184), and testing (n = 185) datasets. Data augmentation was applied to the training set, yielding 10,296 samples. Six anatomical landmark points were labeled to define the axes of t

A combined classification system for preaxial foot polydactyly and its derived surgical strategies.

BACKGROUND: Pediatric polydactyly is a relatively common congenital condition of hand and foot, affecting physical and mental development of children. Hand polydactyly has been brought into focus recently. By contrast, the studies on foot polydactyly are limited. Among the types, there is little international research on preaxial foot polydactyly since it is a relatively rare type, and the domestic research on it is fewer. The purpose of our study is to propose a new classification for guiding operation. METHODS: We collected all patients with preaxial foot polydactyly who had preoperative ra

Case Report: Bilateral pes planovalgus: an uncommon pediatric orthopedic presentation of Charcot-Marie-Tooth disease type 4B1.

Charcot-Marie-Tooth (CMT) disease is an inherited motor and sensory neuropathy that typically presents with cavovarus foot deformity. Pes planovalgus is uncommon in CMT and, while noted incidentally in multicenter cohort studies of CMT4B subtypes, has not been described as the predominant orthopedic manifestation in a published case report of genetically confirmed CMT4B1. We report the case of a 7-year-old girl who presented with progressive painful bilateral pes planovalgus, hallux valgus, frequent falls, and gait imbalance, despite otherwise normal developmental milestones. Neurological exam

Risk factors for surgical site infection requiring reoperation after osteotomy around the knee: a multicenter retrospective study.

BACKGROUND: Osteotomy around the knee (OAK) is an established joint-preserving procedure for knee osteoarthritis; however, postoperative surgical site infection (SSI) remains a serious complication that may compromise bone healing and functional outcomes. Evidence regarding risk factors for SSI requiring reoperation after OAK is limited. This study aimed to identify perioperative factors associated with SSI requiring reoperation following OAK. METHODS: This multicenter retrospective study included 247 patients (302 knees) who underwent OAK at one institution and two affiliated hospitals betwe

Osteotomies in Flatfoot Surgery: A Narrative Review and Practical Framework.

Background: Progressive Collapsing Foot Deformity (PCFD)-historically termed adult-acquired flatfoot deformity or posterior tibial tendon dysfunction-is a three-dimensional disorder of the hindfoot, midfoot, medial column, and, in advanced cases, the ankle. Joint-preserving reconstruction depends on matching each procedure to the dominant mechanical component of the deformity. Objective: This narrative review describes four osteotomies used in adult PCFD reconstruction-lateral calcaneal lengthening osteotomy (LCLOT), medial-sliding calcaneal osteotomy (MSCO), Cotton osteotomy, and medial-closi

[Attenuated X-linked hypophosphatemia: late diagnosis in an adult patient].

Although patients with X-linked hypophosphatemia (XLH) usually present a phenotype characterized by rickets in children or osteomalacia in adults, short stature, lower limb deformities, and dental abnormalities, this condition can manifest with a wide phenotypic spectrum. In attenuated cases, diagnosis is often delayed, preventing early treatment. We present the case of a 50-year-old woman who was referred to our clinic by her son's pediatrician, who had been diagnosed with XLH through molecular testing. Manifestations suggesting attenuated XLH (mild valgus curvature of the lower limbs, bilate

Locomotor Health in Modern Broiler Chickens: Risk Factors, Major Disorders, Assessment Methods, and Implications for Welfare and Production Efficiency.

Genetic selection for rapid growth, improved feed efficiency, and increased breast-muscle yield has substantially enhanced broiler production; however, it has also increased the functional demands placed on the locomotor system. This review critically examines locomotor health in modern broiler chickens in relation to welfare and production efficiency. Particular attention is given to the multifactorial interactions among genotype, growth rate, body weight and conformation, skeletal development, nutrition, physical activity, housing conditions, age, and infectious factors. Major locomotor diso

Managing Severe Proximal Femoral Fibrous Dysplasia in a Young Adult Using Hemiarthroplasty in a Resource-Constrained Healthcare System: A Case Report.

Fibrous dysplasia of the proximal femur may lead to progressive deformity, pain, and pathological fractures, with shepherd's crook deformity representing one of its most severe manifestations. Although valgus osteotomy with intramedullary fixation is considered the preferred surgical treatment, extensive bone destruction and limited healthcare resources may preclude conventional reconstruction. We present a case of severe proximal femoral fibrous dysplasia successfully managed with bipolar cemented hemiarthroplasty in a resource-constrained healthcare setting. A 25-year-old man presented with

Kinematic Alignment in Total Knee Arthroplasty: Unresolved Challenges and Barriers to Overcome for Universal Application.

Restoring patient-specific alignment in total knee arthroplasty has emerged as a promising approach to improve joint kinematics and reduce the need for soft-tissue release. Kinematic alignment (KA) and its variants-including restricted, inverse, and functional alignment-seek to replicate the patient's native joint line obliquity (JLO) and pre-arthritic alignment, often guided by classification systems such as the Coronal Plane Alignment of the Knee (CPAK). However, despite the theoretical appeal, the universal adoption of KA remains controversial. This review highlights several concerns, inclu

Changes in Coronal and Sagittal Plane Alignment After Transphyseal ACL Reconstruction Performed Within Two Years of Skeletal Maturity: Results From the PLUTO Study Group.

BACKGROUND: For patients with open physes approaching skeletal maturity, transphyseal anterior cruciate ligament reconstruction (ACLR) with soft tissue autograft and metaphyseal fixation is often preferred. Although iatrogenic physeal injury is rare, growth disturbances have been reported. This study sought to evaluate postoperative changes in lower extremity alignment 1 year after transphyseal ACLR in skeletally immature patients within 2 years of skeletal maturity. HYPOTHESIS: Transphyseal ACLR would not result in clinically significant changes in alignment. STUDY DESIGN: Cohort study; Lev

Temporary screw epiphysiodesis for correction of ankle valgus in children with multiple hereditary exostoses : a case-control study.

AIMS: In patients with multiple hereditary exostosis (MHE), the development of ankle valgus is a common deformity. The aim of this study was to evaluate whether temporary screw epiphysiodesis of the medial distal tibia leads to a correction of the deformity and to provide further information on the timing and duration of the operative treatment, considering sex-specific differences and a concomitant fibular shortening or relevant exostoses. METHODS: The data of 37 MHE patients with a total of 68 ankles, and 35 patients with idiopathic ankle valgus with a total of 56 ankles, were analyzed retr

High-precision universality? A matched-pair sawbone study on the learning curve, accuracy, and implant interchangeability of a novel open-platform robotic UKA system.

BACKGROUND: Robotic-assisted unicompartmental knee arthroplasty (UKA) improves implant positioning over conventional techniques, but commercial systems commonly bind the robotic platform to a single implant manufacturer, restricting clinical flexibility. Open-platform systems offer a vendor-neutral alternative, yet their effect on novice execution precision and workflow stability during implant switching remains insufficiently characterized. METHODS: A prospective exploratory matched-pair case series was conducted on standardized synthetic bone models (Sawbones). Two robotic-naïve medical stu

Altered tibialis anterior tendon mechanics in hallux valgus: a shear-wave elastography study integrating distal insertion patterns.

BACKGROUND: While distal tibialis anterior tendon (TAT) insertion variants have been associated with hallux valgus, the in vivo mechanical behavior of the tendon has not been adequately investigated. This study aimed to evaluate tibialis anterior tendon stiffness using shear-wave elastography (SWE) in patients with hallux valgus and healthy controls, to examine its relationship with radiographic deformity parameters, distal insertion patterns, and body mass index, and to assess measurement reliability. METHODS: In this prospective case-control study, 105 patients with radiographically confirm

Meniscal Repair Achieves Comparable Mid-Term Functional Performance to Partial Meniscectomy Following Anterior Cruciate Ligament Reconstruction in Highly Active Patients.

OBJECTIVE: Concomitant meniscal injuries accompany 40%-60% of anterior cruciate ligament (ACL) ruptures. Surgeons often face a clinical choice between meniscal repair, which prioritizes long-term joint preservation, and partial meniscectomy, which is traditionally perceived to facilitate earlier functional recovery. Despite the biological advantages of repair, persistent concerns remain that its restrictive postoperative protocols might impede mid-term functional performance compared to meniscectomy. This study aims to compare quadriceps/hamstring strength, dynamic balance, functional hop perf
