Step-down biomechanics in unilateral patellofemoral pain: a cross-sectional study of young females.

BACKGROUND: Despite extensive research on biomechanical abnormalities in the affected lower limb of young women with patellofemoral pain (PFP) during step-down tasks, the presence of such abnormalities in the unaffected limb remains unclear. This study aims to fill the gap in existing literature by investigating bilateral biomechanics in this population, with the goal of providing an evidence base for targeted rehabilitation and preventive strategies. METHODS: In this cross-sectional study using purposive sampling, 16 young females with unilateral PFP and 16 healthy controls, all aged 18–30

Timely Intervention Saves the Limb in a 7-Year-old Child with Distal Femoral Physeal Injury: A Case Report.

INTRODUCTION: The distal femoral physis makes an important contribution to limb length and is an uncommon bone to fracture in children. Complications associated with the fracture, such as vessel injury in the short term and angular deformities and limb length discrepancies in the long term, can be detrimental. CASE REPORT: A 7-year-old child presented with a distal femoral physeal injury, Salter-Harris type 2, without any associated neurovascular injury, following a road traffic accident. He was operated on the same day with closed reduction and fixation with multiple K wires. Recovery was un

Prenatal ultrasound diagnoses fetal amniotic band syndrome causing fetal gastroschisis: A case report.

BACKGROUND: Amniotic band syndrome (ABS) is a congenital disease which occurs before 12 weeks of gestation. The rupture of the fiber bundle or fiber sheath around the embryo or fetus, resulting in the fetal development of this site is affected, eventually leading to deformity. But now, there are few case reports of abdominal wall fissure and visceral valgus caused by ABS. This case can broaden our horizon and let us learn more about ABS. CASE PRESENTATION: Female, 25 years old, the yellow race, primiparida, color ultrasound in early pregnancy and color doppler ultrasound at 11 to 13 weeks sug

Severe Coronal Plane Deformity Correction in Total Knee Arthroplasty Is Associated With Increased Surgeon Physiologic Demand.

BACKGROUND: Coronal plane deformity in total knee arthroplasty (TKA) can introduce major variability in difficulty between cases based on the type and degree of deformity. The purpose of this study was to compare the energy expenditure of TKA in varus and valgus knees. METHODS: There were 85 patients undergoing primary, elective cemented TKA who were prospectively enrolled, and various demographic and surgical variables were collected. Surgeon physiologic parameters were collected via a smart garment that recorded heart rate variability (beats per minute), minute ventilation (mL/minute), and

Two-dimensional vs three-dimensional analysis of weightbearing Charcot neuropathy deformities.

BACKGROUND: Clinical standards for diagnosing Charcot neuroarthropathy (CN) rely on two-dimensional (2D) radiographs, despite the complex three-dimensional (3D) deformities. This study utilized a 3D measurement method, previously applied to nonweightbearing imaging, for weightbearing imaging to quantify deformities in CN patients and compared these to 2D weightbearing radiographs (WBR). METHODS: This retrospective study included seven CN patients who underwent both 2D WBR and 3D weightbearing CT scans (WBCT). Five observers manually annotated 2D radiographs, and placed 3D fiducial markers on

Intramedullary locking plates device versus headless screw fixation for treatment of hallux valgus deformity: A retrospective observational study.

This retrospective observational study aimed to evaluate the outcomes of subcapital osteotomy using an intramedullary locking plate for the treatment of hallux valgus (HV) deformity. We hypothesized that plate fixation would provide superior angular correction and lower recurrence compared to screw fixation. Between January 2018 and July 2023, a total of 49 feet underwent corrective osteotomy for HV. Of these, 36 feet were treated with headless screws and 13 with intramedullary locking plates. All cases had a minimum follow-up duration of 12 months. Radiographic outcomes were assessed using we

Degree of Hallux Valgus Is Associated With Dynamic Foot and Knee Kinematics in Ballet Turnout Movements.

BACKGROUND: Hallux valgus (HV) is a common forefoot deformity that may compromise lower-extremity biomechanics, particularly during turnout movements in ballet. However, the kinematic consequences of dynamic high-load, turnout-specific dance tasks remain unclear. This study investigated the relationship between the degree of HV and lower-extremity joint kinematics in ballet dancers. METHODS: Fourteen female ballet dancers with visible HV (mean HV angle: 26.5 ± 8.6 degrees) performed 3 tasks: demi-plié, sauté, and grand jeté. A 10-camera motion capture system and a force plate were used to ass

Robotic total knee arthroplasty with functional positioning safely addresses major coronal deformities: Comparable complications and survivorship.

PURPOSE: Robotic-assisted total knee arthroplasty (TKA) has emerged as a reliable strategy to improve surgical accuracy and enable functional alignment (FA), also referred to as functional knee positioning (FKP). However, its application in patients with major coronal plane deformities remains under-investigated. This study aimed to evaluate complication rates, implant survival, radiographic outcomes and patient-reported measures in patients with severe deformities undergoing robotic-assisted TKA with FA/FKP principles compared to matched controls. METHODS: A retrospective comparative study w

Quantitative morphological evaluation of the first metatarsal head in juvenile hallux valgus.

BACKGROUND: Hallux valgus, a common deformity of the forefoot, is characterized by lateral deviation of the hallux at the metatarsophalangeal joint, often requiring surgery. Juvenile hallux valgus, defined by its development during childhood or adolescence, has a high postoperative recurrence rate. However, its etiology remains unclear. Several studies have found that the radius of curvature of the first metatarsal head is smaller in individuals with adult hallux valgus than in healthy controls, possibly contributing to progression of deformity or recurrence. However, to our knowledge, no deta

One-Year Comparative Outcomes of Scarf/Akin Osteotomy Versus Modified Lapidus Procedure in the Correction of Moderate to Severe Hallux Valgus.

BACKGROUND: Hallux valgus is a common forefoot deformity. Both Scarf/Akin osteotomy and the modified Lapidus procedure (MLP) are established surgical options for moderate to severe cases, but few studies directly compare outcomes. METHODS: A retrospective cohort study of patients undergoing Scarf/Akin osteotomy or MLP between 2019 and 2020 was performed. Patient-reported outcomes (Manchester Oxford Foot Questionnaire (MOXFQ), Foot and Ankle Outcome Scores (FAOS), EQ-5D-5L) and radiographic measures (hallux valgus angle (HVA), intermetatarsal angle (IMA)) were compared at a minimum 12-month fo

Comparing the Effects of Lateral Opening Wedge Distal Femoral Osteotomy and Medial Closing Wedge High Tibial Osteotomy on Tibial Tubercle-Trochlear Groove Distance: A 3D Computed Tomography Simulation Study.

BACKGROUND: Genu valgum is a known risk factor for recurrent patellar instability, and surgical correction of deformity can be utilized as part of the management strategy to improve tracking and optimize outcomes. Tibial tuberosity-trochlear groove (TT-TG) distance is a widely used objective measurement of the lateral quadriceps force vector in patients with patellar instability. The evidence documenting the effect of lateral opening wedge distal femoral osteotomy (LOWDFO) and medial closing wedge high tibial osteotomy (MCWHTO) on TT-TG is limited, with minimal data directly comparing the biom

Reconstruction With the Contralateral Fibula for Isolated Congenital Pseudarthrosis of the Fibula.

Congenital pseudarthrosis of the tibia (CPT) remains one of the most challenging conditions to manage in pediatric orthopedics. Fibular pseudarthrosis, often associated with CPT, is similarly resistant to treatment. In contrast, isolated congenital pseudarthrosis of the fibula (ICPF) is a rare condition for which no standardized treatment has been established. Here, we report four pediatric cases of ICPF treated with a novel reconstruction technique using the contralateral fibula. All patients achieved bony union at the pseudarthrotic lesion of the affected fibula. The donor sites of the contr

Foot arch morphology and lower-limb biomechanical characteristics in university students: a cross-sectional multifactorial analysis of 1,078 participants.

To examine how medial longitudinal arch morphology relates to a multidomain set of static biomechanical correlates—covering structure (forefoot/rearfoot alignment), loading/pressure behavior, arch “spring-like” function, and postural-control surrogates—as a pragmatic screening framework in university students. In this cross-sectional study, 1,078 university students (569 males, 509 females; mean age 20.18 ± 1.43 years) underwent standardized quiet-standing assessments. Arch height, hallux valgus angle, and heel valgus angle were measured using a 3D foot scanner, and plantar-pressure–derived ou

A Case Report: Post-traumatic Windswept Deformity of the Elbow - Rare but Possible.

INTRODUCTION: Windswept deformity of the elbow, defined as cubitus varus on one side and cubitus valgus on the contralateral side, is exceptionally rare, particularly in post-traumatic settings. The majority of published literature focuses on windswept deformities of the lower limbs, with almost no evidence-based guidelines for upper limb deformities. CASE REPORT: An 8-year-old male sustained bilateral elbow injuries in separate falls, leading to an untreated cubitus varus deformity of the left elbow (carrying angle -15°, Baumann's angle 82°) and a cubitus valgus deformity of the right elbow

Lateralising step-DMMO (MIS) - a midshaft modification of the lateralising DMMO for a powerful correction of a pes adductus during surgical treatment of a hallux valgus.

Surgical treatment of hallux valgus can be very difficult when it occurs together with pes adductus. The medially displaced metatarsal bones prevent a sufficient shifting of the first metatarsal during hallux valgus correction. This can lead to inherently high reoccurrence rates. Open surgical corrections lead to a profound soft tissue trauma and prolonged swelling, while requiring relief from weight-bearing in the affected foot. In this paper, we aim to introduce a useful midshaft modification of the lateralising Distal Metatarsal Minimal-invasive Osteotomy (lat. DMMO) to perform an effective

The role of triple osteotomy in managing complex hallux valgus deformity with pronation.

BACKGROUND: First metatarsal pronation deformity is contributed to hallux valgus (HV) and a significant risk factor for recurrence. The efficacy of triple osteotomy in specifically correcting pronation deformity of severe HV remains unreported. This study aimed to evaluate the correction of first metatarsal pronation deformity achieved with triple osteotomy (Reverdin, proximal wedge and Akin) in severe HV. METHODS: We retrospectively analyzed data from 29 patients (40 feet) with severe HV and concomitant first metatarsal pronation who underwent triple osteotomy between January 2022 and Decembe

Impaired Bone Healing and Fracture Complications during Limb Lengthening in a Neurofibromatosis Type 1 Patient Receiving Selumetinib: Case Report and Literature Review.

INTRODUCTION: Neurofibromatosis Type 1 (NF1) is a genetic disorder that may lead to the development of plexiform neurofibromas (Pns). Selumetinib, a selective mitogen-activated protein kinase (MEK1/2) inhibitor, has shown clinical benefit in shrinking inoperable PNs. However, its long-term effects on the skeleton remain unclear. This case describes impaired bone healing and pathological fractures following limb lengthening in a child with NF1 receiving prolonged selumetinib therapy. CASE REPORT: A 5-year-old boy with genetically confirmed NF1 and left lower limb PN-induced overgrowth presente
