Congenital pseudarthrosis of the proximal tibia: a case report.

Congenital pseudarthrosis of the tibia (CPT) is a rare and challenging pediatric orthopedic disorder that predominantly affects the distal or middle third of the tibia, with involvement of the proximal third being exceedingly rare. Literature on this condition remains limited. This case presents proximal tibial CPT with proximal tibial dysplasia in a 7-year-old male patient with neurofibromatosis type 1. Based on the fundamental treatment principles of CPT, the child underwent pseudarthrosis resection, bone grafting, and internal fixation with a Rush rod combined with a plate. Following the ac

Validation of Foot Angular Measurements Using Intraoperative Simulated Weightbearing Fluoroscopic Images.

BACKGROUND: Weightbearing foot and ankle radiographs are essential because skeletal geometry changes under load. Radiographic measurements, which guide management decisions, have been validated using weightbearing radiographs. Simulation of weightbearing intraoperatively would be ideal and may improve the accuracy of deformity correction in foot and ankle surgery. This study aims to validate the accuracy of angular measurements on intraoperative simulated weightbearing fluoroscopic foot images. METHODS: A prospective study of 50 patients with a mean age of 51.3 years undergoing elective foot

Magnesium Bioabsorbable vs Titanium Screws in Hallux Valgus Surgery: A Prospective, Randomized Medium-term Trial.

BACKGROUND: Metal implants, the gold standard for osteotomy fixation in foot surgery, do have inherent problems. A relatively high percentage of patients undergoing hallux valgus correction require secondary surgery for metal implant removal. Although available since the 1980s, bioabsorbable implant use in foot surgery has been limited because of concerns of osteolysis and inferior strength. Magnesium screws are bioabsorbable implants without the inherent risks associated with other bioabsorbable implants. We prospectively compared outcomes of MAGNESIX to metal implants in hallux valgus deform

A systematic review of radiological outcomes and implant positioning in robotic-assisted functionally aligned robotic total knee arthroplasty.

INTRODUCTION: Functional alignment (FA) or functional knee positioning is a patient-specific strategy for total knee arthroplasty (TKA) that utilizes robotics to balance coronal, sagittal, and axial planes while preserving joint-line orientation and soft-tissue tension within predefined guardrails. Although early clinical outcomes are encouraging, the radiographic profile and workflow consistency of robotic FA have not been clearly synthesized. METHODS: In accordance with PRISMA guidelines, English-language studies of primary robotic FA-TKA with ≥2-year follow-up were searched. Eligible desig

Distal Metatarsal Articular Angle and First Metatarsal Pronation: A Weightbearing CT Correlation Study.

BackgroundThe distal metatarsal articular angle (DMAA) is a measurement used in surgical decision making of hallux valgus correction. However, it is difficult to measure on plain radiographs, is subject to projection bias, and its role in pathology is unclear. With the advent of weight-bearing CT (WBCT), our understanding of hallux valgus as a multiplanar deformity has evolved. The aim of this study was to investigate whether there is a relationship between the DMAA and pronation of the first metatarsal head in patients with hallux valgus.MethodsThis was a single-centre, retrospective analysis

Hemiepiphysiodesis with a novel transphyseal bridge implant for lateral patellar luxation in a growing dog.

Angular limb deformities are developmental bone-shape anomalies that typically occur due to abnormal physeal growth before skeletal maturity and result in musculoskeletal malalignment leading to abnormal limb appearance and mechanical dysfunction. The conformational and mechanical changes resulting from a bone deformity often require surgical correction. Surgical correction of a bone deformity after skeletal maturity is typically an invasive, open procedure requiring a corrective osteotomy followed by stabilization of the bone using implants. Early intervention in the skeletally immature anima

Secondary deformity following paediatric tibial lengthening with circular fixators.

PURPOSE: To evaluate the incidence, magnitude, and risk factors for secondary angular deformity after tibial lengthening with a circular external fixator in skeletally immature patients. We hypothesized that younger age and low bone healing index would predispose to late valgus drift. METHODS: A retrospective review was conducted on 98 children who underwent tibial lengthening using a circular external fixator, with a minimum follow-up of one year. Radiographic assessment included full-length standing AP radiographs and lateral tibial views obtained preoperatively, at fixator removal, six mon

A comprehensive weightbearing computed tomography study: the pathogenesis of first metatarsal pronation in sesamoid bone displacement, due to hallux valgus deformity and progressive collapsing foot deformity (PCFD).

PURPOSE: This study aimed to explore the biomechanical interrelationships in Progressive Collapsing Foot Deformity (PCFD), also known as flatfoot, with concurrent hallux valgus (HV), first metatarsal pronation and sesamoid bone displacement. The primary purposes were to quantify correlations between arch collapse, first metatarsal rotation, HVA (hallux valgus angle), and sesamoid displacement using weight-bearing computed tomography (WBCT), which provides superior three-dimensional insights compared to traditional radiographs. The central research question was: How is M1 rotation related to al

Effects of Strengthening the Intrinsic Muscles of the Foot in Adults with Flatfoot: A Scoping Review.

Background: Flatfoot is an alteration of the normal structure of the foot, characterized by a partial or total reduction of the medial longitudinal plantar arch, valgus deformity of the heel, and abduction of the forefoot. While treatments often include strengthening of the intrinsic foot muscles, evidence of its efficacy in adults with flatfoot remains limited. Objectives: The main objective of this review was to evaluate the effects of strengthening the plantar intrinsic muscles in adults with flatfoot. Methods: Searches were conducted in PubMed, Embase, Cochrane, PEDro, and Web of Science d

Supramalleolar osteotomy combined with fibular osteotomy for mid-stage varus ankle osteoarthritis.

BACKGROUND: Ankle osteoarthritis affects approximately 3% of adults globally, and patients in the middle and late stages often face the risk of joint fusion or replacement. Although simple supranasal osteotomy is an accepted treatment for mid-stage ankle arthritis, its isolated application may alter the biomechanics of the talofibular joint, increase lateral pressure, and lead to postoperative pain; the role of combined fibular osteotomy remains controversial in existing literature. Therefore we performed a retrospective to investigate: 1) the clinical efficacy of the supramalleolar osteotomy

Changes in Lower Limb Axial Alignment, Gait Biomechanics, and Plantar Force in Crowe Type IV Hip Dysplasia After Total Hip Arthroplasty: A Mean Ten-Year Follow-Up Retrospective Cohort Study.

OBJECTIVES: Total hip arthroplasty (THA) is the gold standard for treating Crowe IV developmental dysplasia of the hip (DDH). However, its long-term effects on lower limb alignment, gait biomechanics, and plantar force in these patients remain underexplored, which is discussed in this article. METHODS: We conducted a retrospective cohort study that included 43 DDH Crowe IV patients who underwent THA between February 2008 and October 2019 and a control group of 43 matched healthy volunteers. Postoperative functional outcomes and quality of life were assessed using the Harris Hip Score, KOOS, A

Comparing the clinical outcomes of arthroereisis and osteotomy in the treatment of paediatric patients with idiopathic flexible pes planus: a systematic review and meta-analysis.

INTRODUCTION: Both subtalar arthroereisis and osteotomies are well-recognised surgical reconstructive options for paediatric pes planus deformity. We compared the clinical and radiographic outcomes of subtalar arthroereisis versus osteotomies in the surgical management of symptomatic idiopathic flexible pes planus in paediatric patients 2-18 years old. Specifically, we evaluated changes in key radiographic parameters and validated patient-reported outcome measures, as well as complications, to determine relative efficacy and safety. METHODS: Electronic databases (PubMed, Embase, and The Cochr

Allograft Deltoid Ligament Reconstruction and Z-Lengthening Fibular Osteotomy for Residual Valgus Instability After Ankle Fracture Fixation: A Case Report.

Residual valgus instability following ankle fracture fixation presents a reconstructive challenge, especially when medial soft tissue compromise precludes early deltoid ligament repair. Restoring medial stability, together with fibular length and syndesmotic alignment, is crucial for re-establishing joint congruity and preventing progressive deformity or degenerative complications. In this single-patient case report, we describe a novel technique combining the use of an allograft deltoid ligament reconstruction with a Z-lengthening distal fibular osteotomy in a young adult male who developed r

[Finite element biomechanical analysis of coronal alignment correction in unicompartmental knee arthroplasty for severe varus deformity].

OBJECTIVE: To investigate the effects of different correction angles of the coronal lower limb alignment (represented by the hip-knee-ankle, HKA ) on the stress of prostheses and ligaments in patients with severe genu varum undergoing unicompartmental knee arthroplasty (UKA), and to clarify the optimal reconstruction strategy for postoperative coronal alignment. METHODS: Based on the computed tomography (CT) and full-ength lower limb X-ray data of 12 patients with severe genu varum (HKA angle:15°to 20°), individualized three-dimensional finite element models were established. By adjusting the

Footwear Identity and Postoperative Experiences of White-Collar Women After Hallux Valgus Surgery: A Qualitative Study.

Background: Hallux valgus affects footwear tolerance, body image, and social participation, particularly among white-collar women who adhere to formal dress codes. While clinical outcomes of hallux valgus surgery are well described, little is known about how women in office-based occupations experience postoperative recovery. This study explored the lived experiences of women at least 12 months after surgical correction of mild-to-moderate hallux valgus using distal first-metatarsal osteotomy with adjustable intramedullary T-plate fixation. Methods: A qualitative interpretivist approach was em

What outcomes matter to patients having surgical treatment for symptomatic hallux valgus? : a systematic review of qualitative studies.

AIMS: Hallux valgus (HV) is a common deformity of the foot, for which various forms of surgical treatment are available. A recent think tank highlighted a lack of understanding about which key outcomes matter most to patients who undergo surgery for HV. The aim of this systematic review was to synthesize the existing evidence to establish whether commonly used outcome measures in foot and ankle surgery adequately reflect what is important to patients. METHODS: We conducted a systematic review of qualitative studies in which methods such as patients' interviews, questionnaires, or focus groups

Elevated Medial Distal Resection Reduces Bone Density in Valgus Total Knee Arthroplasty.

Insufficiency periprosthetic fracture after total knee arthroplasty is uncommon but serious. In valgus alignment, chronic unloading of the medial femoral condyle may reduce local bone mineral density (BMD), increasing fracture risk after coronal realignment. We retrospectively studied 40 valgus knees that underwent primary total knee arthroplasty and compared BMD at the distal femoral resection plane, measured using quantitative computed tomography, with 40 propensity score-matched varus knees (matched for age, sex, body mass index, side, and degree of deformity). In valgus knees we also simul
