BACKGROUND: Violin playing involves prolonged upper limb exertion and asymmetrical posture, potentially leading to pain and postural defects. Despite the known prevalence of playing-related musculoskeletal disorders, data on young adult violinists compared to a control group remains insufficient. The aim of this study was to assess pain and postural problems among young adult violin players, and to compare them with those of non-violin players. Additionally, an attempt was made to identify the causes of these problems in violinists and to indicate directions that should be developed in further

BACKGROUND: Distal femoral osteotomy (DFO) influence on intra-articular parameters, particularly joint line convergence angle (JLCA), joint line obliquity (JLO), remains under-investigated.
HYPOTHESIS: This study aimed to assess the impact of DFO on JLCA and JLO and to identify potential "tipping points" of correction influencing JLCA changes, hypothesizing that large femoral corrections affect intra-articular alignment. Secondly, to correlate preoperative deformity and postoperative intra-articular parameters with clinical outcomes.

BACKGROUND: The Mitchell and Chevron osteotomies are two widely performed surgical techniques for the correction of hallux valgus deformities. While both approaches are commonly used, there remains limited biomechanical evidence comparing their structural stability, particularly when paired with advanced fixation techniques. This study aims to evaluate and compare the biomechanical properties of these procedures when utilizing the third-generation minimally invasive (MIS) screw fixation technique. METHODS: Sixteen synthetic first metatarsal (Sawbones Pacific Research Laboratories, Vashon, WA)

BACKGROUND: Percutaneous hallux valgus correction is gaining popularity in the United States. However, its benefits compared to the traditional open technique remain debated. Adoption of the technique has been slow, largely due to the lack of long-term studies incorporating patient-reported outcomes in the U.S.
PURPOSE: To evaluate the one-year clinical and radiographic outcomes of percutaneous chevron-Akin osteotomy for hallux valgus correction using patient-reported functional outcome measures.
STUDY DESIGN: Retrospective cohort study METHODS: We identified 295 patients who underwent percu

PURPOSE: A goal of kinematic alignment (KA) total knee arthroplasty (TKA) is to align the femoral component to restore the distal and posterior femoral joint lines to prearthritic. To restore the joint line on the worn distal condyle, the resection thickness is adjusted by 2 mm for cartilage loss based on the reported average cartilage thickness on magnetic resonance imaging. It remains unclear whether a 2 mm fixed adjustment is ideal for all patients, especially those with thicker cartilage. This study examined whether a 2 mm fixed adjustment in patients with cartilage thickness ≥3.0 mm negat

BACKGROUND: Accurate hindfoot alignment (HA) assessment is essential for surgical planning of foot and ankle procedures, as measurement errors may lead to inappropriate indication. Weight-bearing computed tomography (WBCT) enables HA assessment through coronal images, typically aligned perpendicular to the second ray. However, this forefoot-based reference may not be appropriate in all clinical scenarios, particularly when isolated hindfoot evaluation is required. This exploratory study sought to evaluate how 3 anatomical reference axes affect HA measurements in patients with different foot de

BACKGROUND: Malunion of tibial plateau fractures with valgus deformity can cause chronic pain and functional instability, especially in young, active patients. Anatomical correction of the deformity is essential to restore joint function and prevent progression of osteoarthritis. The corrective approach can be intra-articular or extra-articular osteotomy, depending on the type and location of the deformity.
CASE PRESENTATION: A 32-year-old woman presented with pain and instability of the right knee 2 years after trauma. Radiological examination showed a lateral tibial plateau fracture malunio

BACKGROUND: Elbow injuries are common among baseball players, often involving the medial epicondyle of the humerus in childhood and the ulnar collateral ligament in adulthood. During skeletal immaturity, excessive valgus stress may lead to fragmentation or hypertrophy of the medial epicondyle. If such morphological abnormalities persist into adulthood, they may compromise elbow stability. The forearm flexor-pronator muscles contribute to dynamic elbow stability under valgus stress during throwing. However, the influence of medial epicondyle morphology on this stabilization remains unclear. Thi

BACKGROUND: Distal femoral osteotomy (DFO) represents an important intervention for addressing valgus and varus deformity about the knee. Closing wedge DFO (CWDFO) is less used and documented than opening wedge DFO for correcting coronal-plane knee deformities. This study evaluates the safety and efficacy of lateral CWDFO (LCWDFO) and medial CWDFO (MCWDFO) by analyzing complication rates and clinical, radiographic, and patient-reported outcome measures.
METHODS: A retrospective review was done of 19 patients (average age: 42.2 years; range 23-75) who underwent CWDFO between January 2016 and D

ObjectiveLower limb malalignment accelerates the progression of knee osteoarthritis (KOA). Knee realignment osteotomy is a well-established treatment for unicompartmental KOA with malalignment. Traditional planning in KOA patients corrects deformities with an osteotomy at the metaphysis but overlooks Paley's approach, which targets the center of rotation angulation (CORA). Osteotomy at the metaphysis may induce secondary translational deformities, which remain unstudied in KOA patients. This study aims to identify the CORA in KOA patients with tibial malalignment.MethodsThirty tibiae (10 varus

PURPOSE: Hip involvement in hereditary multiple exostoses (HME) may lead to coxa valga, femoral impingement and progressive hip subluxation. This study aimed to evaluate the outcomes of a single-stage technique combining proximal femoral varus osteotomy with femoral neck osteochondroma trimming performed through the osteotomy site.
METHODS: A retrospective series of fifteen patients (23 hips) with HME was reviewed. Clinical outcomes were assessed using the Postel-Merle d'Aubigné score. Radiographic evaluation included femoral and acetabular parameters assessed pre-operatively, immediately pos

Spondyloepiphyseal dysplasia congenita (SEDC) often involves hip deformity and dislocation. We present a case of an eight-year-old boy with bilateral femoral head deformity and hip dislocation treated by open reduction and femoral valgus osteotomy. Surgery was performed via an anterior approach. After removal of obstructive factors, reduction was only maintained at 30° adduction, necessitating valgus osteotomy to stabilize the hip. Postoperative fixation was achieved with hip spica casting. This case suggests that femoral valgus osteotomy may be essential to maintain reduction in SEDC-associat

BACKGROUND: The Coronal Plane Alignment of the Knee (CPAK) classification method describes knee phenotypes. The rise in robotic-assisted total knee arthroplasties (RA-TKA) has enabled surgeons to fine-tune bony cuts, minimizing soft tissue release while prioritizing balanced gaps rather than predetermined alignment targets, a technique known as functional alignment (FA). As a patient’s preoperative CPAK changes when using FA, our aim was to assess which preoperative CPAK phenotypes are maintained post-TKA and further define this change with a simplified classification. METHODS: We retrospectiv

BACKGROUND: Hallux valgus (HV) remains a common forefoot deformity with a global prevalence of 19%. It is largely agreed that surgical correction of HV with minimally invasive techniques have good outcomes. Options include the percutaneous minimally invasive screw fixation (SF) and the intramedullary plate fixation (IPF) technique, with both employing different fixation techniques and hence stability of fixation. In this article, we aim to compare these 2 minimally invasive correction techniques and analyze the radiologic and clinical outcomes, including their time to consolidation.
