Restoration of limb alignment and joint stability across the functional range of motion is a primary goal of total knee arthroplasty (TKA). Multiple alignment strategies have evolved to achieve these objectives, yet a single optimal strategy remains controversial. This study evaluates practice variation among kinematic (KA), functional (FA), and mechanical alignment (MA) strategies in TKA with respect to tibial insert selection, posterior cruciate ligament (PCL) preservation, and deformity tolerance. A systematic PubMed review identified English-language studies of primary TKA published betwee

BACKGROUND: National joint replacement registries inform prosthesis selection and health policy using a consistently recorded set of patient, implant, and surgical variables. However, preoperative radiographic alignment and detailed diagnostic complexity are rarely, if ever, captured. Surgeons pay close attention to limb alignment and deformity because both reflect the complexity of the knee being replaced and are associated with the risk of failure. However, the incremental value of recording these measurements, beyond the variables that registries already collect, has not been quantified. As

BACKGROUND: Osteotomy to correct lower-limb deformity may be necessary when residual growth precludes asymmetric epiphysiodesis. Interlocking intramedullary nailing (IMN) provides minimally invasive stable fixation, spanning fragile segments, and compatible with bone lengthening, but requires precise preoperative planning and meticulous operative technique, as the room for postoperative adjustment is very limited. Our aim here is to present practical indications, planning principles, instrumentation, operative steps and postoperative management for safe and effective correction of femoral and

Introduction describes first metatarsophalangeal (MTP) joint arthrodesis as a historical and currently powerful option for severe forefoot deformities, including arthritis, hallux valgus, hallux varus, and trauma. Shortening of the first ray, particularly after MIS bunion procedures and revisional surgeries, creates biomechanics challenges that can compromise fusion. To address large segmental defects, the senior author advocates interpositional femoral head allograft to restore first-ray length and provide structural support for stable MTP joint arthrodesis. The approach avoids autograft draw

BACKGROUND: Knee osteoarthritis (OA) is a leading cause of chronic pain and disability. Regenerative therapies such as intra-articular injections of platelet-rich plasma (PRP) and nanofat (and adipose-derived stromal vascular fraction) have emerged as promising treatment options, but comparative evidence on their effectiveness remains limited.
OBJECTIVE: To compare the efficacy of intra-articular PRP injections to that of nanofat injections for pain relief and functional improvement in patients with predominantly unilateral knee OA over a 6-month follow-up.

BACKGROUND: Non-correctable (fixed) knee deformities in total knee arthroplasty (TKA) are technically demanding, and the intraoperative factors driving recovery in this subgroup remain unclear. We hypothesized that, in non-correctable knees, a strategy that minimizes bone resection and preserves the patient's native medio-lateral laxity asymmetry, rather than chasing symmetric gaps through additional bone removal, would be associated with superior patient-reported functional outcomes at one year, whereas no such association would be observed in correctable knees.
METHODS: We reviewed 861 cons

Complex fractures of the femoral shaft rise problems due to high energy trauma with major soft-tissues injuries and bone comminution so increasing operative difficulties, risk of infection and delayed union. Our aim was to appraise the outcome of these problems when using a closed intramedullary locked nailing. There were 52 men and 16 women. Aged 16 to 83 years. 52 patients had multiple-injuries. There were 17 open fractures. The 68 cases were subdivided according to a classification in 3 types. Stable fractures type A and B1 were excluded. The shaft was divided into 5 zones. Patients were in

Even with the development of the Pfizer-BioNTech BNT162b2 vaccine, which provides protection against COVID-19 and demonstrates high efficacy in generating immune responses, the complexities of the dynamics linking pro- and anti-inflammatory cytokine profiles with anti-spike IgG production remain unclear. The study aims to elucidate these immune dynamics after vaccination. This prospective cohort research was done at the University of Diyala from January 2022 to January 2023, evaluating the immunological response to the Pfizer-BNT162b2 mRNA vaccine in 180 healthy students. PURPOSE: Cubitus varus is a common triplane deformity in adults associated with supracondylar humeral fractures experienced as a child and consists of varus, extension, and internal rotation components. When corrective osteotomy is indicated, these three components should be measured precisely. This study aimed to evaluate the accuracy of radiographic and physical measurements of cubitus varus deformities in adults compared to values measured on three-dimensional (3-D) bone surface models of the adult bilateral humerus.
METHODS: Three-dimensional bilateral humerus models were developed using

BACKGROUND: National conversation has justifiably been concerned with firearm-related deaths and much less attention has been paid to the consequences of surviving a firearm injury. We assessed the risk of hospital readmission, length of stay (LOS) during hospitalization, and costs within 90-days after surviving an index firearm injury and compared these data with pedestrians and occupants involved in motor vehicle crash (MVC).
METHODS: Nationwide Readmission Database, a nationally representative readmission database from 2013 and 2014 was used to create a retrospective cohort study. The prim

Subtrochanteric femur fractures pose unique challenges because of strong deforming muscular forces and high biomechanical stresses at the proximal femur, leading to a high rate of complications. It is important to review common pitfalls in the treatment of subtrochanteric fractures and provide strategies to avoid them. Key pitfalls include delayed surgical treatment, difficulties with patient positioning (especially in patients with obesity), malreduction in varus or flexion, improper intramedullary nail entry point, unrecognized limb rotation or length discrepancies, and inadequate fixation.

Chronic ankle instability (CAI) can develop in up to 40% of patients after a first-time ankle sprain. Advanced imaging and statistical shape modeling (SSM) provide the opportunity to evaluate if subtle differences in foot and ankle morphology, such as hindfoot alignment or cavus features, may contribute to structural predispositions for recurrent instability. In this study, a 14-bone SSM was created from weight-bearing CT data (n = 80) from patients with CAI (n = 23), cavus foot type (n = 29), and rectus (n = 28) foot type. Scans for the CAI group were from pre-operative imaging of patients un

BACKGROUND: Coronal plane deformity in total knee arthroplasty (TKA) can introduce major variability in difficulty between cases based on the type and degree of deformity. The purpose of this study was to compare the energy expenditure of TKA in varus and valgus knees.
METHODS: There were 85 patients undergoing primary, elective cemented TKA who were prospectively enrolled, and various demographic and surgical variables were collected. Surgeon physiologic parameters were collected via a smart garment that recorded heart rate variability (beats per minute), minute ventilation (mL/minute), and

Introduction: Medial compartment knee osteoarthritis (OA) is characterized by varus deformity. A medial open-wedge high tibial osteotomy (MOWHTO), frequently invoked in the treatment of this deformity, affects the knee as well as the ankle joints. This study aims to evaluate the radiological and clinical effects of a MOWHTO on the ankle joint. Materials and Methods: A retrospective analysis was conducted with data from 110 patients (mean age: 52 years; 74.5% female) who underwent a MOWHTO between 2020 and 2023. Radiographic assessments were conducted both preoperatively and one year after surg

PURPOSE: Robotic-assisted total knee arthroplasty (TKA) has emerged as a reliable strategy to improve surgical accuracy and enable functional alignment (FA), also referred to as functional knee positioning (FKP). However, its application in patients with major coronal plane deformities remains under-investigated. This study aimed to evaluate complication rates, implant survival, radiographic outcomes and patient-reported measures in patients with severe deformities undergoing robotic-assisted TKA with FA/FKP principles compared to matched controls.
METHODS: A retrospective comparative study w

PURPOSE: To evaluate whether a high lateral distal femoral angle (LDFA) contributes to undercorrection of lower-limb alignment following medial open wedge high tibial osteotomy (MOWHTO), and to determine the LDFA threshold above which tibial correction alone may be insufficient.
METHODS: This retrospective study analysed 68 patients who underwent MOWHTO at our hospital between 2020 and 2022. Based on their postoperative mechanical axis (MA), patients were categorised into two groups: the acceptable correction group (A-group; MA 57%-67%) and the undercorrection group (U-group; MA ≤ 56%). INTRODUCTION: Windswept deformity of the elbow, defined as cubitus varus on one side and cubitus valgus on the contralateral side, is exceptionally rare, particularly in post-traumatic settings. The majority of published literature focuses on windswept deformities of the lower limbs, with almost no evidence-based guidelines for upper limb deformities.
CASE REPORT: An 8-year-old male sustained bilateral elbow injuries in separate falls, leading to an untreated cubitus varus deformity of the left elbow (carrying angle -15°, Baumann's angle 82°) and a cubitus valgus deformity of the right elbow

Surgical treatment of hallux valgus can be very difficult when it occurs together with pes adductus. The medially displaced metatarsal bones prevent a sufficient shifting of the first metatarsal during hallux valgus correction. This can lead to inherently high reoccurrence rates. Open surgical corrections lead to a profound soft tissue trauma and prolonged swelling, while requiring relief from weight-bearing in the affected foot. In this paper, we aim to introduce a useful midshaft modification of the lateralising Distal Metatarsal Minimal-invasive Osteotomy (lat. DMMO) to perform an effective
