Clinical and radiological results of allogenous bone graft versus synthetic calcium-phosphate graft in opening wedge high tibial osteotomy.

PURPOSE: To compare the clinical and radiographic data after opening wedge high tibial osteotomy (OWHTO) using an allogenous bone graft versus a tricalcium phosphate (TCP) graft. METHODS: This prospective clinical study enroled 42 patients (84 knees) between September 2022 and March 2023, undergoing bilateral OWHTO for genu varum. Patients' knees were randomized intraoperatively into two groups: those receiving an allograft or TCP graft to fill the osteotomy defect. Preoperative assessments included pain severity via the visual analogue scale (VAS) and joint function using the Western Ontario

Intra-articular alignment after distal femoral osteotomy: A multicenter analysis of intra-articular changes.

BACKGROUND: Distal femoral osteotomy (DFO) influence on intra-articular parameters, particularly joint line convergence angle (JLCA), joint line obliquity (JLO), remains under-investigated. HYPOTHESIS: This study aimed to assess the impact of DFO on JLCA and JLO and to identify potential "tipping points" of correction influencing JLCA changes, hypothesizing that large femoral corrections affect intra-articular alignment. Secondly, to correlate preoperative deformity and postoperative intra-articular parameters with clinical outcomes. PATIENTS AND METHODS: A retrospective multi-center analysi

Guided growth for the correction of angular lower limb deformity: Correction rates for idiopathic and non-idiopathic aetiologies.

INTRODUCTION: Coronal plane knee deformities are frequent in children and may arise from idiopathic or nonidiopathic causes, leading to cosmetic, functional, or early degenerative issues. Guided growth using tension band plates is an established corrective method, but its predictability - especially in nonidiopathic cases - needs further evaluation. This study analyzed correction rates (CR) and treatment outcomes in different etiologies. METHODS: A retrospective review included patients treated for coronal plane deformities with tension band plates from 2009 to 2021. Patients were classified

A fixed 2 mm adjustment for worn distal femoral cartilage when the cartilage of the unworn compartment is ≥3 mm does not negatively affect outcome scores after kinematically aligned total knee arthroplasty.

PURPOSE: A goal of kinematic alignment (KA) total knee arthroplasty (TKA) is to align the femoral component to restore the distal and posterior femoral joint lines to prearthritic. To restore the joint line on the worn distal condyle, the resection thickness is adjusted by 2 mm for cartilage loss based on the reported average cartilage thickness on magnetic resonance imaging. It remains unclear whether a 2 mm fixed adjustment is ideal for all patients, especially those with thicker cartilage. This study examined whether a 2 mm fixed adjustment in patients with cartilage thickness ≥3.0 mm negat

Changes in Tarsal Tunnel Volume and Pressure After Minimally Invasive Lateral Sliding Calcaneal Osteotomy: A Cadaveric Study.

BACKGROUND: Hindfoot varus deformity is often corrected using lateral sliding calcaneal osteotomies, which may compress the tarsal tunnel and risk tibial nerve entrapment. Minimally invasive surgery (MIS) techniques may mitigate soft tissue disruption, but their impact on tarsal tunnel biomechanics remains unclear. Purpose is to evaluate the effect of calcaneal lateralization using a 3-mm MIS burr on tarsal tunnel volume and pressure in a cadaveric model. METHODS: Ten matched pairs of fresh frozen cadaveric lower limbs underwent lateral-to-medial calcaneal osteotomy using a 3-mm MIS burr. Com

The Closing Wedge Distal Femoral Osteotomy: A Series of 19 Cases for the Management of Genu Valgum and Genu Varum.

BACKGROUND: Distal femoral osteotomy (DFO) represents an important intervention for addressing valgus and varus deformity about the knee. Closing wedge DFO (CWDFO) is less used and documented than opening wedge DFO for correcting coronal-plane knee deformities. This study evaluates the safety and efficacy of lateral CWDFO (LCWDFO) and medial CWDFO (MCWDFO) by analyzing complication rates and clinical, radiographic, and patient-reported outcome measures. METHODS: A retrospective review was done of 19 patients (average age: 42.2 years; range 23-75) who underwent CWDFO between January 2016 and D

Uncovering the Impact of Center of Rotation of Angulation Location on High Tibial Osteotomy in Knee Osteoarthritis: A Potential Pathway for Improved Outcomes.

ObjectiveLower limb malalignment accelerates the progression of knee osteoarthritis (KOA). Knee realignment osteotomy is a well-established treatment for unicompartmental KOA with malalignment. Traditional planning in KOA patients corrects deformities with an osteotomy at the metaphysis but overlooks Paley's approach, which targets the center of rotation angulation (CORA). Osteotomy at the metaphysis may induce secondary translational deformities, which remain unstudied in KOA patients. This study aims to identify the CORA in KOA patients with tibial malalignment.MethodsThirty tibiae (10 varus

Analysis of CPAK change in robotic functional alignment TKA: a new simplified classification.

BACKGROUND: The Coronal Plane Alignment of the Knee (CPAK) classification method describes knee phenotypes. The rise in robotic-assisted total knee arthroplasties (RA-TKA) has enabled surgeons to fine-tune bony cuts, minimizing soft tissue release while prioritizing balanced gaps rather than predetermined alignment targets, a technique known as functional alignment (FA). As a patient’s preoperative CPAK changes when using FA, our aim was to assess which preoperative CPAK phenotypes are maintained post-TKA and further define this change with a simplified classification. METHODS: We retrospectiv

Prevalence of Common Foot Conditions in Children-A Cross-Sectional Study in Danish Children Aged 6 to 16 Years.

Background: Caregivers often seek medical assistance when their child experiences podiatric medical ailments. Podiatric medical diseases such as ingrown toenails, callosities, warts, metatarsus varus, and hallux valgus frequently occur in children and adolescents. However, treatment, prevention, and rehabilitation are often based on empirical experiences; thus, as a first endeavor, clinical and epidemiologic mapping of podiatric medical diseases in children is warranted. We describe the prevalence of common foot conditions-callosities, ingrown toenails, hallux valgus, metatarsus varus, and war

The Magnitude of Varus Correction and Its Influence on Postoperative Patellar Height and Posterior Tibial Slope in Medial Open-Wedge High Tibial Osteotomy.

Objectives: Medial open-wedge high tibial osteotomy (MOWHTO) is a surgical procedure used to treat medial compartment osteoarthritis of the knee with varus deformity. The aim of this study was to examine whether a larger correction angle in medial open wedge high tibial osteotomy (MOWHTO) leads to greater changes in postoperative patellar height (PH) and posterior tibial slope (PTS). Methods: Data from 83 patients who underwent MOWHTO were retrospectively analyzed. Lower limb alignment was evaluated using the hip-knee-ankle angle (HKAA). The PH was assessed using the Insall-Salvati index (ISI)

Surgical Correction of Large Talar Tilt in Varus Ankle Osteoarthritis II: A New Treatment Algorithm Based on the Tibial Plafond Inclination and Arthritis Types.

Medial opening-wedge supramalleolar osteotomy (MOWSMO) is a joint-preserving surgical option for varus ankle osteoarthritis (OA); however, its ability to correct large varus talar tilt (TT), particularly in advanced diseases like Takakura stage IIIB, remains limited. Varus TT represents a complex three-dimensional deformity characterized by coronal malalignment and internal rotation, which cannot be reliably corrected by isolated supramalleolar realignment. Building on our previous work, we propose a new treatment algorithm for large varus TT based on preoperative tibial plafond inclination (T

Free-Form Bilateral Multiplanar External Skeletal Fixation for Pes Varus Correction in a Miniature Dachshund.

This report describes the surgical correction of a pes varus deformity in a 13 mo old miniature dachshund using free-form bilateral multiplanar external skeletal fixation (ESF). The dog exhibited lameness in the right pelvic limb, with a varus deformity of the distal tibia and grade 2 lateral patellar luxation. Radiographic and computed tomography imaging confirmed the presence of varus in the distal diaphyseal portion of the right tibia, without procurvatum or internal torsion. Medial opening wedge osteotomy and free-form bilateral multiplanar ESF successfully corrected the deformity. Dynamiz

Palliative high tibial osteotomy achieves comparable outcomes to corrective osteotomy in varus knees with medial compartment osteoarthritis: A long-term clinical and radiological retrospective study.

PURPOSE: The long-term outcomes of palliative high tibial osteotomy (PO) remain insufficiently defined. This study compared the clinical and radiological outcomes of PO with those of corrective osteotomy (CO) in patients with varus knee deformity. Secondary aim was to evaluate arthroplasty-free survival. It was hypothesized that PO and CO would yield comparable results and similar conversion rates to total knee arthroplasty (TKA). METHODS: A retrospective cohort study was conducted on patients who underwent lateral closing wedge-high tibial osteotomy (LCW-HTO) between 2001 and 2017. Patients
