Ten-year functional outcomes and up to 18-year survivorship of computer-navigated total knee arthroplasty: analysis of 1677 cases from a high-volume centre.

PURPOSE: This study aimed to contribute to the evidence base for computer-navigated TKA by providing long-term survivorship and functional outcomes in a large single-centre series, addressing the limited follow-up duration and sample sizes in existing literature. METHODS: This retrospective study analysed 1677 consecutive computer-navigated Columbus TKA procedures performed between 2005 and 2011. Patient-reported outcomes were assessed through 10 years, with survivorship assessed at average 14 years (maximum 18 years). Kaplan-Meier analysis and Cox regression identified revision risk factors.

Humero-ulnar external fixation of pediatric supracondylar humeral fractures using an adult wrist external fixator.

BACKGROUND: Although cross-pinning is widely used to treat pediatric supracondylar humeral fractures, it involves risks of nerve injury, malunion, and compartment syndrome. In this two-center retrospective observational study, we compared the outcomes of humero-ulnar external fixation using an adult wrist fixator with those of conventional pinning for pediatric supracondylar humeral fractures. Given the technical complexity of Slongo lateral external fixation, we evaluated a simpler joint-spanning external fixation method. METHODS: This study enrolled 37 children: 19 treated with pinning betw

Off-axis biomechanical alterations and related interventions in medial knee osteoarthritis: a mini review.

Medial compartment knee osteoarthritis (OA) is the most common form of knee OA and can be influenced by off-axis (frontal and transverse plane) biomechanics. Abnormalities such as varus malalignment, elevated knee adduction moment (KAM), dynamic varus thrust, altered step width, lateral trunk lean, reduced tibial rotation, and abnormal foot progression angle (FPA) contribute to excessive medial loading and are associated with symptom severity and structural progression. These modifiable factors present important targets for conservative management. This mini-review synthesizes current evidence

Analysis of posterolateral and anterolateral laxity in varus knees undergoing robotic assisted total knee arthroplasty.

Achieving equal/ near equal gaps on medial and lateral side in flexion and extension is important for a well-balanced total knee arthroplasty (TKA). Assistive technologies like robotic assisted TKA (RATKA), permit intraoperative assessment of the lateral and medial gaps throughout the range of motion. Aim of study is to analyze the difference between the lateral and medial gaps in full extension and in full flexion in varus knees. A retrospective analysis of the prospectively collected data of the consecutive first 50 RATKA patients using Velys (with 1.9.4 software) system. Inclusion criteria

Relationship between knee and hindfoot coronal alignment in community-dwelling Japanese adults: Cross-sectional analysis of 1000 hip-to-calcaneus radiographs.

BACKGROUND: Hindfoot compensation for coronal knee deformity is recognized in end-stage osteoarthritis (OA), but evidence in non-surgical populations is limited. This study aimed to clarify the independent association between knee and hindfoot coronal alignment and to identify factors related to hindfoot compensation in community-dwelling adults. METHODS: In this cross-sectional study, adults aged ≥40 years underwent bilateral hip-to-calcaneus radiography. Knee varus angle, hindfoot valgus angle, medial distal tibial angle, and talar tilt angle were measured. Knee OA was defined as Kellgren-L

Comparison of Functional Recovery Between Unicompartmental Knee Arthroplasty and Medial Meniscus Posterior Root Tear Repair in Patients Aged over 50 Years: A One-Year Propensity Score-Weighted Study.

BACKGROUND: Medial meniscus posterior root tear (MMPRT) accelerates knee osteoarthritis progression. Meniscal root repair (MRR) in patients older than 50 years is controversial due to unpredictable healing and mild to moderate cartilage degeneration, which may lead surgeons to consider unicompartmental knee arthroplasty (UKA). This study compared clinical outcomes of UKA versus MRR in patients older than 50 years who had MMPRT and Kellgren-Lawrence (KL) grade ≤ 3 arthritis. METHODS: We retrospectively reviewed patients aged 50 to 79 years who had magnetic resonance image-confirmed MMPRT who u

Association between spinopelvic parameters and clinical outcomes following hip fracture: an observational retrospective study.

BACKGROUND: Femoral neck fractures carry a substantial risk of complications such as varus malalignment, avascular necrosis and the need for reoperation. While traditional prognostic factors have been extensively studied, the relevance of postoperative spinopelvic characteristics after internal fixation remains unclear. This study evaluated whether spinopelvic parameters measured on standardized postoperative radiographs are associated with adverse outcomes. METHODS: Ninety-six patients aged 18-60 years who underwent internal fixation for femoral neck fractures were analysed. Demographic vari

[Supracondylar dome osteotomy with plate fixation via a posterior triceps-splitting approach].

OBJECTIVE: Supracondylar correction outside the joint block is performed to achieve a symmetrical cubital axis in the frontal plane and a balanced range of motion in the sagittal plane without interfering with joint congruence. INDICATIONS: Supracondylar correction is performed mainly in the frontal plane. A simultaneous correction in the sagittal plane is effortlessly possible as well. The condylar prominence can be balanced through additional medial translation. CONTRAINDICATIONS: Dome osteotomy is not suitable for patients aged under 10 years. It is not the first-choice method for correct

Combination of midfoot derotational osteotomy and ankle arthrodesis against neglected clubfoot: a case report.

Neglected congenital clubfoot in adults is difficult to manage. Multi-joint arthrodesis is often required but is invasive and associated with complications. We report an adult case treated with ankle arthrodesis and midfoot derotational osteotomy. A 59-year-old man with untreated idiopathic clubfoot since birth presented with severe hindfoot varus and rigid midfoot inversion, leading to wheelchair dependence. Imaging showed correctable hindfoot deformities but rigid midfoot inversion with talonavicular subluxation. Surgery consisted of Achilles tendon lengthening, ankle and subtalar arthrodesi

Editorial Commentary: Is High Tibial Osteotomy All We Need? No Differences in Patient-Reported Outcomes in Medial Meniscal Posterior Root Tears With Varus Deformity Between Isolated High Tibial Osteotomy and Osteotomy With Meniscal Centralization and All-Inside Root Repair.

Medial meniscus posterior root tears in the setting of varus malalignment present a difficult clinical pathology, often leading to accelerated medial compartment osteoarthritis if untreated. Both meniscal root repair and high tibial osteotomy remain cornerstones of intervention for these patients, effectively redistributing load and improving joint biomechanics. It remains to be seen whether the unloading effect of high tibial osteotomy or restoration of meniscal hoop stress distribution and reduction of medial meniscal extrusion via root repair provides greater benefit. The addition of arthro

Predicting CT-based coronal plane knee phenotype parameters using imageless navigation and machine learning.

BACKGROUND: Coronal plane alignment of the knee (CPAK) categorizes knee phenotypes according to joint line obliquity (JLO) and the arithmetic hip‒knee‒ankle angle (aHKA). CPAK is traditionally measured via long leg radiographs, but recently other modalities such as computed tomography (CT), image-based, and imageless navigation have been used. Machine learning (ML) is a field of artificial intelligence focused on enabling systems to learn from data and improve their performance without explicit programming. The aim of this study is to understand how accurately imageless navigation measures CPA

Clinical validity of radiographic parameters for evaluating pediatric foot deformities: correlation with pedobarographic analysis.

BACKGROUNDS: Pediatric foot deformities disrupt biomechanics and can lead to long-term musculoskeletal complications. Radiographic parameters, particularly the anteroposterior talo-1st metatarsal angle (AP talo-1MT), are widely used; however, their accuracy may be compromised in the presence of hallux valgus. The clinical utility of the anteroposterior talo-2nd metatarsal angle (AP talo-2MT) has not been well established. This study evaluated radiographic parameters by correlating them with pedobarographic indices, hypothesizing that AP talo-2MT would demonstrate comparable or superior functio
