Compensatory gait differences between femoral and tibial varus knee Osteoarthritis: Implications for movement and rehabilitation.

Knee osteoarthritis (KOA) with varus deformity can arise from either femoral (FVD) or tibial (TVD) origins, but the differences in compensatory gait patterns between these subgroups remain insufficiently explored, hindering the formulation of targeted rehabilitation strategies. This study aimed to compare three-dimensional lower extremity joint kinematics and spatiotemporal parameters during gait between KOA patients with FVD and TVD, with a focus on implications for movement compensation and rehabilitation. A retrospective cohort of 48 KOA patients with varus deformity (24 FVD, 24 TVD) schedu

Greater Valgus Alignment in Pediatric and Adolescent Patients with a Primary ACL Tear Compared with Healthy Controls.

BACKGROUND: Coronal plane angular deformity remains under-investigated in the context of pediatric anterior cruciate ligament (ACL) tears. We hypothesized that baseline coronal alignment in pediatric and adolescent patients with a first-time ACL injury would differ from that in a matched healthy comparison population of patients without knee pathology. METHODS: Patients ≤18 years of age who underwent primary ACL reconstruction and had preoperative lower-extremity hip-to-ankle alignment radiographs (cases) and individuals without lower-extremity conditions that would influence alignment (contr

Primary Total Knee Arthroplasty Using a Functional Alignment Concept With Kinematic Versus Inverse Kinematic Technique: Does it Make a Difference?

BACKGROUND: There is an increasing trend toward a functional alignment in total knee arthroplasty (TKA). Functional alignment can be performed using a kinematic technique, FA(k), or an inverse kinematic approach, FA(ik). The purpose of this study was to compare the results of kinematic versus inverse kinematic techniques during primary TKA using a functional alignment concept. METHODS: A retrospective review was conducted of patients undergoing robotic-assisted TKA with 1-year follow-up, excluding patients who had a body mass index greater than 45 or valgus deformity by coronal plane alignmen

Influence of Hallux Valgus Varus Deformity on First Metatarsal Sagittal Inclination Assessment: A Comparison Between Weightbearing Radiography and Weightbearing Computed Tomography.

BACKGROUND: Radiographic assessment of the sagittal inclination of the first metatarsal (1M) is essential for evaluating foot disorders. However, 1M varus deviation may influence these measurements. This study aimed to determine whether hallux valgus (HV) varus deformity influences the sagittal radiographic inclination of the 1M by comparing weightbearing radiography (WBR) and weightbearing computed tomography (WBCT) images. METHODS: Eighty-four feet were analyzed, including HV cases with intermetatarsal angle (IMA) >15° and control feet without HV. The sagittal inclination of the 1M, its bas

Coronal Knee Alignment in Adult Spinal Deformity Patients: Implications for Total Knee Arthroplasty Planning and Coronal Plane Alignment of the Knee Classification Reliability.

BACKGROUND: Kinematic alignment in total knee arthroplasty has been increasingly investigated for its potential improved functional outcomes. The coronal plane alignment of the knee (CPAK) classification was developed to better define native coronal alignment (to act as a target) using joint line obliquity (joint line apex [distal, neutral, or proximal]) and the arithmetic hip-knee-ankle angle (varus, neutral, or valgus). Since the literature is limited in addressing how spinal deformity influences CPAKs, this study examined the distribution of CPAK types in patients who had adult spinal defor

Radiographic Joint Space Widening of the Unaffected Compartment in Varus or Valgus Knees Is Associated With Abnormal Intraoperative Ligamentous Laxity.

BACKGROUND: Krackow described type II valgus deformity as valgus alignment with obvious attenuation of the medial collateral ligament complex. However, little quantitative information exists documenting the associated medial collateral ligament laxity encountered intraoperatively. Furthermore, a corollary may exist in advanced varus deformity with associated lateral ligament laxity, but this has not been well described. This study evaluated whether increased preoperative medial or lateral radiographic joint space width (JSW) in valgus or varus knees, respectively, was associated with increased

Lateral-Conforming Dual-Pivot Versus Nonconforming Anterior-Stabilized Bearings in Valgus Knees Undergoing Primary Total Knee Arthroplasty.

BACKGROUND: Personalized alignment strategies in total knee arthroplasty (TKA) have gained in popularity to recreate native biomechanics and improve outcomes. Prior studies have reported better patient-reported outcome measures (PROMs) for preoperative valgus deformities (compared to varus) when medial-conforming bearings are used. This study evaluated PROMs in a lateral-conforming dual-pivot (DP) bearing compared to a nonconforming anterior-stabilized (AS) bearing after primary TKA. METHODS: A retrospective review was performed on 2,427 primary TKAs implanted with either a DP conforming bear

Is Preoperative Moderate to Severe Varus Alignment Associated With Survivorship and Outcome Scores at a Minimum of 5 Years After Mobile-bearing Total Ankle Arthroplasty With the Hintegra Device?

BACKGROUND: Analyses of long-term results after mobile-bearing total ankle arthroplasty (TAA) in ankles with preoperative moderate to severe varus deformity with adequate sample sizes remain limited. Because preoperative moderate to severe varus alignment may influence implant survival and functional outcomes, clarifying its long-term impact is important. QUESTIONS/PURPOSES: (1) Do the clinical and radiologic outcomes of mobile-bearing TAA differ between ankles with preoperative neutral alignment and those with moderate to severe varus alignment? (2) What are the 5- and 10-year prosthesis sur

Calcaneal shortening for talonavicular medial subluxation in pediatric equinovarus deformity.

INTRODUCTION: Medial subluxation of the talonavicular joint is a common component of equinovarus deformity. Traditional cuboid or cuneiform osteotomies may not adequately correct this deformity. This study evaluated the radiographic correction and prognostic factors of calcaneal shortening for midfoot deformity. METHODS: We retrospectively reviewed 28 patients (32 feet) with equinovarus deformity treated by calcaneal shortening between 2011 and 2021. The shortening osteotomy was performed between the middle and posterior facets of the calcaneus to facilitate lateral translation of the navicul
