Ollier's Disease Regenerate Healing Rates: Is there Cause for Concern?

BACKGROUND: Ollier's disease, characterised by multiple enchondromas, can result in significant limb length discrepancy and angular deformities. Anecdotal evidence suggests faster healing rates and a risk of premature consolidation during lengthening. The aim is to evaluate healing rates in Ollier's and assess the need for change in practice. MATERIALS AND METHODS: A total of 12 patients with Ollier's disease underwent limb lengthening utilising an external fixator or intramedullary lengthening nail. A latency period of 6 days was followed by a lengthening rate of 1 mm per day. Parameters, in

Lateral Decubitus Transradial Carotid Artery Stenting in Ankylosing Spondylitis: A Novel Positioning Strategy.

BACKGROUND Carotid artery stenting is conventionally performed with the patient in supine position. However, patients with ankylosing spondylitis and severe spinal rigidity are frequently unable to tolerate supine positioning due to fixed spinal deformity ("bamboo spine"). General anesthesia for surgical carotid endarterectomy carries additional risks in such patients owing to anticipated difficult airway management secondary to cervical spine ankylosis and fixed flexion deformity. These anatomical and anesthetic constraints necessitate alternative procedural approaches. CASE REPORT A 70-year-

A rare case of deep-seated angioleiomyoma causing fixed flexion contracture of the knee: a case report.

INTRODUCTION AND IMPORTANCE: Articular angioleiomyoma (ALM) is a rare benign tumor originating from the vascular smooth muscle layer, with very few documented cases involving joint structures. Its deep anatomical location and atypical presentation pose significant diagnostic challenges, often mimicking chronic joint pathology. This case highlights the importance of considering articular ALM in patients presenting with unexplained joint contractures and persistent pain. CASE PRESENTATION: A 40-year-old female presented with a 4-year history of progressive right knee pain and fixed flexion defo

Staged Flap Reconstruction of Massive Abdominal Loss of Domain and Sacral Pressure Injury to Restore Quality of Life: A Case Report.

Simultaneous massive abdominal wall loss of domain (LOD) and chronic sacral pressure injury presents an exceptional reconstructive challenge. Optimal sequencing of reconstruction in patients with multifocal defects and impaired mobility remains poorly defined. In this case report, we describe a strategically staged approach prioritizing restoration of trunk stability to enable definitive pressure ulcer reconstruction in a complex patient. A 34-year-old male developed a 32 × 25 cm ventral abdominal wall defect with LOD and a persistent stage IV sacral ulcer following catastrophic complications

Prevalence and related factors of Dupuytren's contracture in a community-dwelling Japanese population.

Dupuytren's contracture (DC) is a fibroproliferative disorder of the palmar fascia with substantial variation in prevalence across populations. However, epidemiological data from Asian countries remain limited, and little is known about upper-limb-specific quality of life or dietary correlates associated with DC. This cross-sectional study investigated the prevalence of DC in a community-dwelling Japanese population, assessed its impact on upper-limb function, and examined demographic, medical, lifestyle, and dietary correlates. DC diagnosis and severity were evaluated by hand surgeons using t

Stepwise Surgical Correction of Fixed Flexion Deformity in Total Knee Arthroplasty: Functional Outcomes.

INTRODUCTION: Fixed flexion deformity (FFD) in advanced knee osteoarthritis (OA) presents a significant intraoperative challenge during total knee arthroplasty (TKA), primarily due to soft-tissue contractures and bony deformities that impede optimal alignment and functional recovery. This study aims to evaluate the effectiveness of intraoperative correction of FFD and to assess short-term functional outcomes following TKA. MATERIALS AND METHODS: In this prospective observational study, 55 knees (mean age 57.28 ± 9.35 years) with Grade 4 OA and FFD underwent TKA between March 2022 and March 20

Serial Casting and Hinged Elbow Bracing in the Treatment of Fixed Flexion Deformity of the Elbow Following Obstetric Brachial Plexus Palsy.

BACKGROUND: Fixed flexion deformity (FFD) of the elbow is a recognized sequela of obstetric brachial plexus palsy (OBPP). Physiotherapy, stretching, and extension splinting are commonly used, but established contractures may persist despite these measures. This study evaluated the outcomes and complications of serial casting and hinged elbow bracing for OBPP-related elbow FFD. METHODS: A retrospective case series of 89 treatment courses in 73 patients with OBPP-related elbow FFD treated between 1996 and 2014 was performed. There were 80 serial casting courses, 8 hinged elbow brace courses, an

Surgical Level Selection in Checkrein Syndrome: A Pathology-Oriented Approach to Flexor Hallucis Longus Lengthening.

Checkrein syndrome presents as a dynamic flexion deformity of the hallux that varies with ankle position and may occur after trauma. Surgical management has traditionally focused on proximal intervention targeting the flexor hallucis longus; however, inconsistent outcomes and recurrence indicate that a single-mechanism explanation may be insufficient. This narrative review proposes a pathology-oriented framework for surgical level selection based on a dual-mechanism concept distinguishing proximal tethering from distal intertendinous coupling at the knot of Henry. Within this framework, deform

Triceps-sparing approach results in a better elbow function than a triceps-detaching approach in total elbow arthroplasty: a multicenter prospective cohort study.

BACKGROUND: New surgical approaches have been developed to optimize functional outcomes after total elbow arthroplasty (TEA). Historically, the triceps-detaching approach was predominantly used, involving partial release of the triceps tendon to improve surgical exposure, at the cost of a higher risk of post-operative extensor weakness. More recently, there has been a shift toward triceps-sparing techniques, in which the triceps tendon is preserved at the cost of limited articular visibility, possibly risking component malalignment. There is no consensus on the best surgical approach, and pros

Through-Knee Amputation Following Total Knee Arthroplasty With Retention of Femoral Component: A Case Report.

INTRODUCTION: Extensor mechanism disruption following total knee arthroplasty (TKA) is an uncommon but serious complication. Surgical management is often complex, particularly in cases of failed reconstruction or persistent instability, and may ultimately necessitate amputation. CASE PRESENTATION: We report a 79-year-old female with a history of TKA and prior patellar tendon repair, who developed a fixed flexion deformity and periprosthetic knee dislocation. Given the failure of conventional interventions and compromised soft tissues, she underwent a through-knee amputation. The femoral compo

Preoperative gap phenotypes in functionally aligned robotic total knee arthroplasty: derivation and internal coherence of a driver-based classification.

Robotic total knee arthroplasty (TKA) enables repeated intraoperative quantification of preoperative alignment, flexion deformity, and medial and lateral compartment gaps in extension and at 90° flexion before component positioning, but the preoperative patterns guiding functional-alignment planning remain incompletely characterized. This study aimed to derive a pragmatic driver-based classification from preoperative robotic data and evaluate its internal coherence across the surgical workflow. We analyzed 68 consecutive primary functionally aligned robotic TKAs performed by a single surgeon u

Total Hip Arthroplasty in Bilateral Ankylosed Hip by Direct Anterior Approach: A Case Report with Technical Tips.

INTRODUCTION: Bony ankylosis of the hip results in profound functional disability and poses significant challenges during total hip arthroplasty (THA), especially when associated with spinal ankylosis. Altered anatomy, severe soft-tissue contractures, fixed deformities, and disturbed spinopelvic mechanics complicate patient positioning, anesthetic management, and component placement and increase the risk of post-operative instability. Although THA has demonstrated favorable functional outcomes in ankylosed hips, limited literature highlights the role of the direct anterior approach (DAA) in pa

Nerve Transfers to Rebalance the Spastic Wrist: An Anatomical Feasibility Study.

PURPOSE: Spastic wrist flexion deformities result from an imbalance between spastic wrist flexors and weak or paralyzed wrist extensors. Nerve transfers offer the potential to restore this balance by combining neurotomy of the spastic donor nerve with reinnervation of the paralyzed recipient muscle. This study evaluated the anatomic feasibility of addressing the spastic wrist flexion deformity by transferring a motor branch from the flexor carpi ulnaris (FCU) to the extensor carpi radialis brevis (ECRB) and/or longus extensor carpi radialis (ECRL). METHODS: Ten fresh-frozen cadaveric upper-ex

Complex Primary Total Knee Arthroplasty for Severe Bilateral Genu Varum with Fixed Flexion Deformity: Mechanical Axis Restoration, Soft-tissue Balancing, and Functional Recovery.

Severe bilateral genu varum associated with end-stage medial compartment osteoarthritis is a technically demanding indication for total knee arthroplasty (TKA). The principal reconstructive problems are fixed coronal malalignment, medial soft-tissue contracture, posterior osteophytes, flexion contracture, asymmetric tibial bone loss, and the need to obtain rectangular flexion and extension gaps without causing postoperative instability. A 50-year-old male presented with progressive bilateral knee pain for 1 year, more severe on the right side, with difficulty in walking, stair climbing, squatt
