Combined hamstrings lengthening and patellar tendon retensioning for neurogenic knee flexion in spastic diplegia.

PURPOSE: This study evaluates the effectiveness of combining hamstrings lengthening with patellar tendon retensioning to address neurogenic knee flexion deformity in children with spastic diplegia. METHODS: Sixty-eight patients with spastic diplegia (mean age: 10.7 years) underwent the combined surgical approach. Knee kinematic parameters, including knee flexion at initial contact (KIC), midstance (KmSt), swing phase (KMSw), and total range of motion (K-ROM), were analyzed preoperatively and at a 24-month follow-up. Outcomes were compared to a healthy control group (n = 20). Postoperative com

Role of Distal Femur Anatomical Plate in Fracture of Distal Femur.

INTRODUCTION: Distal femur fractures are mostly caused due to vehicular injuries in the middle-aged population. Operative treatment has been preferred with open reduction and internal fixation in these fractures. Due to the availability of anatomical locking compression plates (LCPs), better results and improved functional outcomes have been observed. This study aims to observe the surgical outcome of distal femur fractures operated with an anatomical LCP. MATERIALS AND METHODS: A prospective study was carried upon 48 patients with 40 male and 8 female patients having fractures of the distal

Use of the trans-scaphoid portal for the arthroscopic treatment of scaphoid nonunion in the middle third with flexion deformity: technique description and patient series.

Nonunion is one of the main complications associated with scaphoid fractures. Over the past decades, the use of arthroscopy in the treatment of scaphoid nonunion has proven to be a valuable technique, even in cases presenting deformity and carpal instability. This approach facilitates rapid consolidation while minimizing damage to the joint capsule and ligaments, thereby preserving the already compromised vascular supply of the scaphoid. Due to their simpler anatomy, dorsal portals are commonly employed in wrist arthroscopy, reducing the risk of injury to neurovascular structures during joint

Fracture of Fused Vertebrae More Than 20 Years After Anterior Cervical Fusion: A Case Report.

Anterior cervical fusion surgery is widely performed for cervical spine disorders. While generally effective, late complications such as vertebral fractures in fused segments are rare but clinically significant, especially in elderly patients with bone fragility. We report here the case of an 84-year-old woman who developed a flexion deformity fracture at a fused cervical segment more than 20 years after anterior cervical corpectomy and fusion with autologous iliac bone grafting and C4 laminoplasty. She presented with neck pain and radicular symptoms in her left arm after a fall. Initial conse

Clinical Features of Gollop-Wolfgang Complex in North Africa: A Case Study.

Gollop-Wolfgang Complex (GWC) is a rare congenital musculoskeletal anomaly marked by distal femoral duplication and tibial aplasia. While often linked with other systemic defects like those in the VACTERL association, our case uniquely presents an isolated manifestation of this complex. The exact genetic cause of GWC isn't fully understood, highlighting a gap in our knowledge of limb development disorders. Treatment typically involves early surgical intervention, such as knee disarticulation and prosthetic fitting, though limb salvage procedures are also recognized. Despite its global rarity (

Reduced spinopelvic mobility does not correlate with knee flexion deformity in patients undergoing total knee arthroplasty.

PURPOSE: The aim of this study was to evaluate the impact of reduced spinopelvic mobility (SM) on knee flexion deformity (KFD) in patients undergoing total knee arthroplasty (TKA). METHODS: A retrospective analysis on 213 patients (271 knees) undergoing robotic-assisted primary TKA was conducted. Sagittal spinopelvic alignment (SSA) parameters-sacral slope (SS), pelvic incidence (PI), and pelvic tilt (PT)-were measured on lateral standing and sitting spinopelvic radiographs. Patients were stratified according to established SM classifications: standing-sitting difference in SS ≥ 10° versus <

Dynamic External Fixation Using Suzuki Frame for Intra-articular Middle Phalanx Base Fracture with Dorsal Subluxation: A Case Report.

INTRODUCTION: Injuries involving the base of the middle phalanx with intra-articular extension and dorsal subluxation are challenging to manage, particularly when joint congruity and motion preservation are critical. This case report demonstrates the use of the Suzuki frame for dynamic external fixation in such a scenario and discusses broader clinical considerations. CASE REPORT: A 37-year-old male sustained an intra-articular middle phalanx base fracture with dorsal subluxation of the left index finger during a cricket injury. Examination showed swelling, tenderness, and a flexion deformity

Correction of Fixed Knee Flexion Deformity in Patients With Cerebral Palsy Using Suture Anchors for Anterior Distal Femur Hemi-epiphysiodesis.

BACKGROUND: Fixed knee flexion deformity in skeletally immature patients with cerebral palsy (CP) is a common contributor to mobility impairment. Established techniques for guided growth using metal implants are associated with symptomatic hardware. This study provides early results following anterior distal femur hemi-epiphysiodesis (ADFHE) with a novel technique utilizing suture anchor-based nonmetallic implants. METHODS: We reviewed a consecutive series of skeletally immature patients with CP treated for fixed knee flexion contracture with ADFHE using this novel technique between April 202

Assessment of the ligamentous stress distribution in the pathomechanics of the boutonniere deformity through a computational 3D model.

The boutonniere deformity of the digit, resulting from a flexion deformity of the proximal interphalangeal joint (PIPJ) and an extension deformity at the distal interphalangeal joint, is a well-documented condition. However, the association between alterations in forces and damage to the extensor mechanism that causes progression to a boutonniere deformity remains a gap in our understanding. To bridge this gap, we took a unique approach. We used a custom-built specialized apparatus to evaluate digital extension in eight cadaveric hands before and after transecting the central slip, oblique fib

Perioperative Care of a Pediatric Patient With Beals Syndrome.

The trismus pseudocamptodactyly syndrome (Beals syndrome) is an uncommon autosomal dominant condition first described in 1971. The disorder shares phenotypic similarities with Marfan syndrome. Affected patients classically present with two main physical features: limited excursion of the mandible and flexion deformity of the fingers that occurs with wrist extension (pseudocamptodactyly). The primary cellular defect is a mutation of the fibrillin-2 (FBN2) gene on chromosome 5q23. Mutations to this gene change the structure of the FBN2 protein, decreasing the elasticity and altering the strength

Applying a hexapod circular frame to gradually correct persistent knee flexion due to congenital aplasia of the quadriceps-a case report and review of the literature.

Congenital aplasia or hypoplasia of the quadriceps muscle are rare conditions that significantly impair the extension of the knee joint, resulting in a sustained flexion deformity. These conditions alter overall gait quality, impede daily activities and require effective, timely intervention. Surgical techniques for correction have been described, including soft-tissue release, guided growth, tendon transfer and external fixators. Among the external fixators available, hexapod circular frames are probably the most effective because they enable the precise, progressive correction of complex mul

Radiographic Classification of Hallux Valgus Interphalangeus Based on the Center of Rotation of Angulation (CORA): Prevalence, Reliability, and Surgical Implications.

INTRODUCTION: Hallux interphalangeus (also called hallux valgus interphalangeus (HVI)) is a deformity characterized by lateral deviation of the distal phalanx of the great toe relative to the proximal phalanx. It is often seen in conjunction with hallux valgus but can occur in isolation. It is classically measured by the hallux interphalangeal angle (HIA), which is formed from the longitudinal axis bisections of the proximal and distal phalanges. AIM: To recognize, discuss, and propose a classification for HVI considering the concept of center of rotation of angulation (CORA). METHODOLOGY: W

Total knee arthroplasty in a non-ambulatory patient having bilateral knees ankylosed in 90° of flexion associated with rheumatoid arthritis: a review-based case report.

BACKGROUND: Management of severe knee flexion deformity exceeding 80° with arthritis presents significant surgical challenges in total knee arthroplasty (TKA). While various studies have reported outcomes in severe flexion deformity, TKA in ankylosed knees at 90° flexion represents an unprecedented surgical scenario where further flexion and extension are not possible due to contracture of both hamstring and quadriceps. We hereby present the rarest case of ankylosed knee in 90° flexion, which was successfully managed by constrained condylar knee-total knee arthroplasty (CCK-TKA). METHODS: A 5

Impact on spino-pelvic alignment by total knee arthroplasty to correct knee flexion deformity in osteoarthritis patients: a retrospective cohort study.

BACKGROUND: Knee osteoarthritis (KOA) is a common knee joint degenerative disorder. Total knee arthroplasty (TKA) is a surgical treatment option that not only corrects knee flexion contraction but also impacts spino-pelvic alignment, leading to altered standing posture and gait. This study aimed to utilize a low-dose biplanar X-ray device (EOS) whole-body X-ray imaging to assess the impact of knee flexion deformity correction by TKA on spino-pelvic alignment in patients with KOA. METHODS: A retrospective cohort study was conducted in patients with varus-type KOA who underwent TKA between Apri

An Asymmetric Medio-Lateral Flexion Laxity Target Yields Excellent Results in Robotic-Assisted Total Knee Arthroplasty With Functional Knee Positioning.

BACKGROUND: Functional knee positioning (FKP) in robotic-assisted (RA) total knee arthroplasty (TKA) aims to optimize soft-tissue balance through patient-specific implant fine-tuning. However, it remains unclear (1) how intraoperative ligament laxity affects joint awareness and (2) whether the discrepancy between intraoperative planned and achieved laxity is affected by preoperative coronal limb alignment or fixed flexion deformity (FFD). METHODS: This retrospective study analyzed 130 patients undergoing image-based RA TKA with FKP using a cementless cruciate-retaining implant. Intraoperative

Distal Femur Extension Osteotomy for Fixed Flexion Deformity Following Distal Femur Malunion - A Case Report.

INTRODUCTION: Fixed flexion deformities of the knee joint can be due to a variety of causes - intra-articular, extra-articular, muscular, neurogenic, etc. The etiology decides the mode of management and prognosis. CASE REPORT: A 16-year-old male with a 2-year-old malunited right distal femur physeal injury presented with "fixed flexion deformity" of the knee. Clinical and radiological assessment showed that the "fixed flexion knee deformity" was actually a procurvatum deformity of the distal femur. Supracondylar extension osteotomy was done after thorough planning and using an angle blade pla

Extension disorders of fingers with congenital anomalies of the flexor digitorum profundus: a case report.

INTRODUCTION: An aberrant origin of the deep flexor muscle causing congenital flexion deformity of the middle, ring, and little fingers is relatively atypical, and its etiology remains unclear. Previous reports indicate that surgeons have employed a muscle-sliding procedure of the flexor muscles to treat the condition. However, postoperative loss of motion was observed. In our patient, we enhanced the original surgical method by incorporating intramuscular extension of the superficial digital flexor muscle, along with close follow-up and guidance on functional exercises, leading to improved ou
