Chronic HDV Infection Shows Higher HBsAg Isoform Levels than HBV Infection, Paralleling HDV Replicative Activity.

BACKGROUND & AIM: The entry of Hepatitis D Virus (HDV) depends on HBV surface proteins (HBsAg) composed of three isoforms: large-, middle, and small HBsAg. Here, we investigate the levels of total HBsAg and HBsAg isoforms and their correlations with HDV-RNA, HBcrAg, and transaminases in the setting of untreated chronic hepatitis D (CHD). METHODS: This study includes 316 HBeAg-negative patients: 192 CHD and 124 with chronic hepatitis B (CHB) as a control group. HBsAg isoforms were quantified by ad hoc-designed ELISAs. RESULTS: The composition of HBsAg isoforms varied between the two groups of

Hepatitis D in the Middle East: A Review of Disease Burden, Health-System Barriers, and Strategic Directions for Management and Elimination.

Hepatitis delta virus (HDV) is the most aggressive form of viral hepatitis and worsens outcomes in people with chronic hepatitis B (HBV). It speeds up fibrosis, causes early cirrhosis, hepatic decompensation, and increases hepatocellular carcinoma risk. In the Middle East, presumed high HBV prevalence, uneven surveillance, and significant diagnostic and treatment barriers lead to under-recognition of HDV and uncertainty about its true prevalence. This review summarizes current regional evidence on HDV distribution, diagnostic gaps, emerging treatments, and health policy shortcomings. The resul

Dried Blood Spot: A Tool to Ensure Broad Access to Hepatitis D Screening, Diagnosis, and Treatment Monitoring.

BACKGROUND AND AIMS: Chronic hepatitis D is the most severe form of viral hepatitis. Despite recommendations for systematic screening of HBsAg-positive individuals, hepatitis D infection remains underdiagnosed. Standard HDV virological markers are classically assessed in serum or plasma specimens obtained through venous whole blood sampling. This approach is costly and challenging in resource-limited settings. Dried blood spot (DBS) sampling offers a practical alternative for large-scale screening, diagnosis, and monitoring of viral hepatitis. The goal of the study is to evaluate the performan

An Exploratory Cost-Utility Analysis of Screening and Treating Immigrants in Canada for Hepatitis D.

Hepatitis D is common among immigrants in Canada, who account for approximately 23% of the population and tend to originate from high-endemic countries. Mortality rates are notably high among those with chronic infections, and immigrants are less likely to engage in screening and treatment services. This paper aims to assess the cost-utility of screening and treating Hepatitis D Virus (HDV)-positive immigrants in Canada compared with a scenario in which no screening is conducted. This analysis takes a public-payer perspective, uses a lifetime time horizon with a 1.5% annual discount rate, and

Chronic hepatitis B in a large private hospital network in Türkiye: Clinical characteristics and electronic health record-data gaps.

BACKGROUND: Türkiye has intermediate hepatitis B virus (HBV) infection prevalence and an additional burden of hepatitis D virus (HDV). Large-scale real-world data describing adults with chronic HBV-infection and the completeness of routinely collected electronic health record (EHR) data for HBV monitoring and surveillance is not well characterized. METHODS: A retrospective, cross-sectional study using EHR data from a multicenter private hospital network, including 18,284 adult patients with chronic HBV infection between 2017 and 2025 was conducted. The primary analysis included 17,927 adults

Mathematical Modeling of HDV RNA, HBV DNA, and HBsAg Dynamics During Lonafarnib-Based Therapy: Insights from the LOWR HDV-1 Study.

Lonafarnib (LNF) is an investigational drug targeting hepatitis delta virus (HDV) but not hepatitis B virus (HBV), providing a unique opportunity to model HDV kinetics and how changes in HDV affect HBV. We performed a detailed kinetic analysis and developed a mathematical model to explain serum HBV DNA, HDV RNA and hepatitis B surface antigen (HBsAg) kinetics in 15 HBV/HDV coinfected patients receiving LNF-based treatment. After a delay of 0-2 days, patients experienced a rapid 1st-phase HDV-decline followed by either a viral plateau, 2nd slower-decline phase, or viral breakthrough (VB). LNF m

Hepatitis delta virus co-infection in patients with hepatocellular carcinoma and people living with HIV in Nigeria.

INTRODUCTION: Despite Nigeria’s high burden of hepatitis B virus (HBV), the epidemiological landscape of hepatitis D virus (HDV) remains largely unexplored. Consequently, HDV remains under-recognized as a public health concern, slowing down efforts to improve surveillance and clinical care. We investigated HDV infection among distinct HBV-positive populations in Nigeria, including individuals with chronic HBV infection living with or without HIV and those with hepatocellular carcinoma (HCC) and/or cirrhosis. METHOD: A cross-sectional study that involved 390 HBV-infected adults recruited from f

Reflex Testing for Hepatitis D Infection: A Unique Opportunity to Reduce Hepatitis D-Related Chronic Liver Disease Deaths in Australia.

Chronic hepatitis D virus (HDV) infection always occurs as a coinfection with hepatitis B virus (HBV) and is the most severe form of viral hepatitis, associated with a high risk of cirrhosis, liver cancer and death. Effective treatment is now available for HDV-HBV coinfection and HDV screening is recommended for all people living with HBV, yet most people in Australia with HDV-HBV are diagnosed too late to prevent complications. This article calls for an urgent change in HDV testing policy and funding to implement reflex HDV antibody (anti-HDV) testing for all people diagnosed with HBV infecti

Persistent ALT Elevation and Liver Outcomes After DAA-Induced HCV Cure in People With HIV: Impact of HBV and HDV Co-Infections.

HBV replication is commonly suppressed by HCV, and eradication of HCV with direct-acting antivirals (DAAs) may favour HBV reactivation. Data on long-term hepatic outcomes after HCV clearance in people living with HIV (PLWH) with triple viral infections are limited. We analysed all HIV/HCV co-infected participants in the ICONA and HepaICONA cohorts with complete HBV serology who achieved a sustained virological response (SVR) after DAA therapy. Participants were classified as HIV/HCV, HIV/HCV with isolated anti-HBc positivity (IHBc), or HIV/HCV/HBV, with HDV testing when available. The outcomes

Management of Type 1 diabetes is burdensome, both to the individual and society, costing over 100 billion dollars annually. Despite the widespread use of glucose monitoring and new insulin formulations, many individuals still develop devastating secondary complications. Pancreatic islet transplantation can restore near normal glucose control in diabetic patients, without the risk of serious hypoglycemic episodes that are associated with intensive insulin therapy. Providing sufficient islet mass is important for successful islet transplantation. However, donor characteristic, organ procurement

TIGIT expression on natural killer cell subsets is an early indicator of alleviating liver inflammation following bulevirtide treatment in chronic hepatitis D.

BACKGROUND AND AIMS: Bulevirtide (BLV) is a novel and the only approved treatment option for patients with chronic hepatitis D (CHD). BLV alleviates liver inflammation early during treatment when only minor HDV RNA changes are observed. We hypothesized that BLV treatment may influence immune cells in patients with CHD and performed a high-resolution analysis of natural killer (NK) cells before and during BLV therapy. APPROACH AND RESULTS: BLV-treated patients with CHD (n=20) from a single-center cohort were longitudinally analyzed for clinical, molecular, and virological parameters. Periphera
