Charcot foot syndrome (Charcot neuroarthropathy affecting the foot), particularly in its latter stages, may pose a significant technical challenge to the surgeon. Because of the lack of anatomic consistency, preoperative planning with virtual and physical models of the foot could improve the chances of achieving a predictable intraoperative result. In this report, we describe the use of a novel, inexpensive, 3-dimensional template printing technique that can provide, with just a normal printer, multiple "copies" of the foot to be repaired. Although we depict this method as it pertains to repai

OBJECTIVE: To evaluate the association of posterior tibial tendon dysfunction and lesions of diverse ankle structures diagnosed at MRI with radiologic signs of flat foot.
MATERIAL AND METHODS: We retrospectively compared 29 patients that had posterior tibial tendon dysfunction (all 29 studied with MRI and 21 also studied with weight-bearing plain-film X-rays) with a control group of 28 patients randomly selected from among all patients who underwent MRI and weight-bearing plain-film X-rays for other ankle problems. In the MRI studies, we analyzed whether a calcaneal spur, talar beak, plantar

Flexible flatfoot is one of the most common deformities. Arthroereisis procedures are designed to correct this deformity. Among them, the calcaneo-stop is a procedure with both biomechanical and proprioceptive properties. It is designed for pediatric treatment. Results similar to endorthesis procedure are reported. Theoretically the procedure can be applied to adults if combined with other procedures to obtain a stable plantigrade foot, but medium-term follow up studies are missing.

Young’s procedure contains an action mechanism that works better than other techniques on the pathophysiology of FFD. It respects the anatomy and biomechanics of the foot to reach the necessary muscular balance. The benefits of this technique include that the ATT is not detached, so its function mechanism is still active; the new trajectory of the ATT provides a powerful sling function at the level of the navicular; and the horizontal trajectory of the ATT and the osteoperiosteal flaps constitute a powerful inner capsular–tendinous–ligamentous support. What is more, an insufficiency of the ATT

Subtalar arthroereisis, often combined with Achilles tendon lengthening, is a simple and effective way to treat flexible flatfoot in adults. The most common complication is pain in sinus tarsi, which usually disappears after removal of the implant. Midterm results are good and it does not hinder other treatments in the future.

OBJECTIVE: Equinovarus foot deformity following stroke or traumatic brain injury compromises walking capacity, interfering with activities of daily living. In soft-tissue surgery the imbalanced muscles responsible for the deviant position of the ankle and foot are lengthened, released and/or transferred. However, knowledge about the effectiveness of surgical correction is limited. The aim of the present study was to carry out a systematic review of the literature to assess the effects of surgical correction of equinovarus foot deformity in patients with stroke or traumatic brain injury.

BACKGROUND: Flexor digitorum longus (FDL) is a commonly used donor tendon for transfer to substitute for a dysfunctional tibialis posterior in the management of adult acquired flatfoot deformity. The tendon transfer is attached to the navicular bone via a drill hole. Interference-fit screws are an alternative means of securing a transferred tendon allowing for a shorter length of graft and less extensive dissection. The aim of this cadaveric study was to investigate the length of FDL tendon harvest required for transfer and to determine if this length of tendon can be obtained without formal d

Diabetic polyneuropathy is a progressive and irreversible disease, which leads to disability, changes in functioning in daily activities, and frequent falls and injuries in diabetes patients. It is one of the major reasons for the occurrence of foot ulceration and amputation of lower extremities. The aim of the study was to assess the relationship between muscle weakness and other complications of diabetic polyneuropathy. The study included 71 patients with electrophysiologically confirmed diabetic polyneuropathy. Through programmed questionnaires, our methodology encompassed examination of de

The flexible form of acquired hallux varus has historically been treated with a number of applicable soft tissue procedures, most of which combine medial capsular release with adjunct procedures aimed at restoring the transverse plane balance of the digit. In this report, a surgical technique for correction of hallux varus that employs the use of an implanted button and suture device, namely the Mini TightRope(®), is described. Rather than relying on the patient's own soft tissues, this device applies tension to anchor the hallux in the proper alignment. The device is easily adjusted during th

BACKGROUND: Lower-extremity pathologic abnormalities have been common in military recruits for many years. Many of these conditions can become chronic and persist even after retiring from military service. We hypothesized that certain foot abnormalities are more prevalent in veterans versus nonveterans. The purpose of this study was to evaluate what foot and ankle disorders are associated with veteran status while controlling for other demographic factors.
METHODS: The National Health Interview Survey (Podiatry Supplement) from 1990 was used for this secondary data analysis. The data were div

The careful evaluation of the pediatric flatfoot begins with a detailed history and is followed by a thorough physical examination. Flatfeet may be congenital or acquired, and flexible or rigid. It is important to classify and evaluate a flatfoot, as the treatment recommendations depend on the assessment and proper diagnosis. The entire lower limb should be evaluated as a unit as one considers the suprapedal influences affecting foot position. The rigid flatfoot results in a fixed foot position that causes suprapedal compensation. On the other hand, most children presenting for evaluation of a

A 65-year-old man without significant comorbidities was referred to the senior author (EG) 9 months after an ankle arthrodesis procedure with complaints of pain, swelling, and progressive hindfoot valgus. The patient had elected to have the index surgery because of severe ankle arthritis due to longstanding lateral ankle instability. Physical examination revealed a well-healed anterior, midline ankle incision with normal pulses and sensation. Painful, limited ankle and subtalar range of motion was noted along with 20 degrees of hindfoot valgus and subfibular impingement. Radiographs of the ank

Scarf osteotomy of the first ray combined with Weil distal oblique shortening osteotomies of the lateral rays has recently been proposed for the treatment of global rheumatoid forefoot deformities because of the perceived benefit of sparing the metatarsal-phalangeal joints. Furthermore, it has been proposed that undergoing this form of global forefoot reconstruction is reliable based on specific preoperative and intraoperative techniques used to realign the individual rays. Finally, it has been proposed that performing global forefoot reconstruction in the rheumatoid patient population can be

BACKGROUND: The management of adult acquired flatfoot is an evolving practice with the optimal lateral column lengthening procedure still left to considerable debate. The usual choices include lengthening with the use of autograft or allograft through a calcaneocuboid lengthening arthrodesis or Evans' calcaneal lengthening osteotomy. To our knowledge there is only one other study comparing autograft to allograft in adult lateral column lengthening procedures.(9) The purpose of this study was to evaluate differences with regard to union rates and complications when comparing the use of iliac tr

BACKGROUND: Pantalar arthrodesis is considered by many to be the final operative option before amputation for the treatment of paralytic foot deformities. The aim of the present study was to evaluate the long-term results of pantalar arthrodesis with regard to its impact on the adjacent joints and the walking ability of patients with paralytic foot deformities.
METHODS: Twenty-four patients (seventeen men and seven women) who underwent a one-stage pantalar arthrodesis between 1953 and 1973 for the treatment of sequelae of poliomyelitis were reevaluated. All patients underwent a physical and r

Malunion of a first metatarsal osteotomy or fracture can result in dorsal angulation of the distal fragment and shortening of the metatarsal, among other deformities. Dorsal malunion can be caused by improper orientation of the osteotomy, poor intraoperative fixation, or loss of fixation post-operatively due to premature weight bearing or catastrophic failure. There is little in the literature on the rate and incidence of malunion following first metatarsal fractures treated either operatively or nonoperatively. However, treatment options would be similar as for malunion following an osteotomy

Tibiotalocalcaneal arthrodesis is a successful and proven surgical procedure for patients who have significant arthritic changes, deformity, and failed previous operations. Surgical technique varies depending on the type of fixation. Basic surgical principles should not be violated. Correction of the deformity with appropriate joint preparation and stable fixation is important for a good outcome. Other adjunctive materials, such as bone growth stimulators and orthobiologics, should be used appropriately to ensure adequate primary arthrodesis.

Skewfoot is a rare deformity characterized by forefoot adduction and hindfoot valgus. Its etiology and natural history are unknown, although congenital and syndromic forms are observed. Currently, there is no consent about the treatment of skewfoot. Due to its potential resistance to the effects of therapy, it must be differentiated from other, more common deformities. Treatment involves conservative and, most often, operative measures.

BACKGROUND: Adult acquired flatfoot is often associated with Achilles tendon contracture and may be associated with isolated spring ligament insufficiency without Achilles tendon contracture. We have studied the hypothesis that standing valgus hindfoot alignment moment arm is increased in adult acquired flatfoot with Achilles tendon contracture when compared to adult acquired flatfoot without Achilles tendon contracture.
MATERIALS AND METHODS: The standing hindfoot alignment, standing lateral tibial-calcaneal angle, lateral talo-first metatarsal angle, lateral medial cuneiform arch height, an
