Deformity correction and arthrodesis of the midfoot with a medial plate.

Nine feet in eight patients undergoing tarsometatarsal (Lisfranc) or other midfoot arthrodeses for posttraumatic or degenerative arthritis were reviewed retrospectively. All patients were treated using a medial one-third tubular plate spanning the midfoot joints to be fused. Three feet underwent fusion in situ while six feet underwent correction of residual planus, planovalgus, or cavovarus deformity at the time of fusion. All patients achieved fusion within 12 weeks. A good or excellent result was achieved in seven of nine feet. There was no radiographic or clinical evidence of pseudarthrosis

Deformity following fracture in diabetic neuropathic osteoarthropathy. Operative management of adults who have type-I diabetes.

We studied fifteen lower extremities that had a deformity following a fracture in the region of the ankle or the tarsal bones, in fourteen adults who had type-I diabetes and neuropathic osteoarthropathy. The skeletal deformities associated with these fractures were either diagnosed late (two limbs) or occurred in lower extremities that had been treated by means other than non-weight-bearing and immobilization (thirteen limbs). All deformities were severe and secondary to a non-union or malunion of the fracture; all were ultimately treated with operative reconstruction. Before the reconstructio

Management of acquired adult dropfoot.

Dropfoot is a catchall term for ankle equinus, equinovarus, and equinovalgus. The deformity can be flexible or rigid and may be associated with other pathology. In the adult, dropfoot may be congenital or acquired. Acquired dropfoot results from weakness of the ankle dorsiflexors, overpull of the plantarflexors, contracture of the soft tissues, bony deformity, or any combination of these factors. Appropriate treatment includes observation, orthotic devices, bracing, tendon transfers, arthrodesis, and neurolysis. The purpose of this paper is to review the pathophysiology and treatment of acquir

Stabilization of the interphalangeal joint of the big toe: comparison of three methods.

Three methods of stabilizing the IP of the big toe were compared. In group A, 10 patients underwent tenodesis of the extensor hallucis longus to the extensor digitorum brevis tendon. All of them developed a toe-drop; two patients had significant symptoms that required IP fusion. In group B, 19 patients underwent IP fusion using smooth or threaded intramedullary Kirschner wire fixation. There were nine nonunions, three requiring refusion. In group C, 32 patients underwent IP fusion using intramedullary screw fixation. There was one nonunion with screw failure that required revision. Although no

A new model of plastic ankle foot orthosis (FAFO (II)) against spastic foot and genu recurvatum.

A plastic ankle foot orthosis (AFO) was developed, referred to as functional ankle foot orthosis Type 2 (FAFO (II)), which can deal with genu recurvatum and the severe spastic foot in walking. Clinical trials were successful for all varus and drop feet, and for most cases of genu recurvatum. Electromyogram studies showed that the FAFO (II) reduced the spasticity of gastrocnemius and hamstring muscles and activated the quadricep muscles. Gait analysis revealed a reduction of the knee angles in the stance phase on the affected side when using the FAFO (II). Mechanical stress tests showed excelle

Metatarsal lengthening by distraction osteogenesis: a report of two cases.

Two cases of lengthening of metatarsals by distraction osteogenesis are reported. One of these cases is an acquired deformity of the first metatarsal; the other is a congenital short fourth metatarsal. By following the principles set forth by Ilizarov, it was possible to lengthen the metatarsal bones and surrounding soft tissues without open lengthening of tendons or secondary bone grafting of the distraction gap. We believe this method is an improvement over previously described methods of metatarsal lengthening.

Hindfoot arthrodesis in adults utilizing a dowel graft technique.

Surgical fusion may be indicated to stabilize deformed or painful hindfoot joints. Although more commonly performed in children or adolescents, it is the salvage procedure available for adults. Twenty-six patients (30 feet; 20 women, six men) followed for an average of 44.1 months after hindfoot arthrodesis were investigated in a prospective study. Most (20 patients, 24 feet) had inflammatory arthritis, usually rheumatoid, two had previous fractures, and four patients had other diagnoses. All had fusions done using a dowel graft technique. Twenty-four patients (92%) were completely satisfied w

[Secondary interventions in malunited fractures of the ankle joint].

When pain persists and activity is limited as a result of lack of union in a fracture of the upper ankle joint, for us this is a clear indication for a secondary procedure. The goal, of course, is to reconstruct the normal anatomy and correct any deformities present. The time interval between the trauma and secondary reconstruction is no contraindication. The arthrosis present and the patient's age are also not drawbacks. The only contraindication is how severely is function impaired. The final result is not dependent on the degree of malposition nor the arthrosis present before the procedure.

Fasciotomy of the foot: an anatomical study with special reference to release of the calcaneal compartment.

Three patients with calcaneal fractures developed clawing of the lesser toes as a late sequela. Believing that this complication may be the result of contractures from an occult compartment syndrome of the foot, an investigation of the anatomical compartments of the foot was performed. The various compartments of 17 unembalmed adult lower limb specimens were injected with dyed gelatin in a controlled fashion. After freezing, the feet were sectioned either transversely or sagittally. The distribution of the dyed gelatin was then studied. Nine compartments were identified. These were the (1) med

Neuropathic ankle joint in Charcot-Marie-Tooth disease after triple arthrodesis of the foot.

Charcot-Marie-Tooth disease results in pes cavus or equinocavovarus deformity. An accepted surgical correction is triple arthrodesis of the foot. In a retrospective study of 45 Charcot-Marie-Tooth patients, two cases developed neuropathic ankle joint after triple arthrodesis. This procedure prevents mobility of the tarsal joint and consequently increases stress on the ankle joint. If coupled with decreased joint sensation, it may predispose the ankle to neuroarthropathy. This paper emphasizes joint position sense as an important consideration before triple arthrodesis or any other surgical pro

Ball-and-socket ankle joint.

The ball-and-socket ankle joint is a malformation of the ankle in which the articular surface of the talus is hemispherical in both the anteroposterior and lateral projections and has a congruent, concave tibial articular surface. Fourteen patients with this condition were identified retrospectively. Thirteen patients were thought to have the congenital type of ball-and-socket ankle joint which in many was associated with tarsal coalition, short limb, and ray fusion and deletion anomalies. One case of the acquired type, demonstrating less geometric rounding of the talar margins, was seen in a

Pathomechanics of congenital and acquired hallux varus: a clinical and anatomical study.

Hallux varus was observed in 69 feet among patients from a rural Indian population. This included three types of congenital hallux varus. The congenital "primary" type was the commonest and was not associated with any other foot deformity. Among the acquired forms was idiopathic hallux varus of middle age, which developed spontaneously in unshod persons and progressed gradually. Hallux varus following chronic arthritis was part of a consistent pattern of foot deformities in which the first ray collapsed in a zig-zag manner in two planes. Although the development of hallux varus is often due to

[Valgus deformities of the ankle in children].

In children valgus deformities of the ankle joint are usually due to neurological conditions (spina bifida, poliomyelitis, spasticity). The deformity is characterized by a shortened fibula, wedging of distal tibial epiphysis and valgus tilt of the talus. Other conditions producing similar deformity include congenital and acquired fibular pseudarthroses, multiple exostoses and some cases of tarsal synostosis. Early treatment of a progressive valgus in multiple exostoses is mandatory. The author describes a case of ankle valgus following an undiagnosed traumatic section of tibialis posterior ten

Acquired adult flat foot secondary to posterior tibial-tendon pathology.

Nineteen patients with the clinical diagnosis of dysfunction of the posterior tibial tendon underwent surgical exploration. Four types of lesions were identified: avulsion of the tendon at the insertion (Group I), mid-substance rupture of the tendon (Group II), an in-continuity tear of the tendon (Group III), and no tendon tear, tenosynovitis only (Group IV). These conditions could not be separated preoperatively by clinical or radiographic means. The patients in Group I were treated by reinsertion of the tendon; in Group II, by flexor tendon transfer; and in Groups III and IV, by synovectomy.

[The child's foot in juvenile polyarthritis (cP)].

The foot, with particular involvement of the ankle joint and forefoot, is an early and frequent site of inflammatory changes in juvenile rheumatoid arthritis. According to the type of disease (sero+ or sero-, polyarticular or pauciarticular, HLAB-27 positive), there is a great variety of symptoms and also of prognoses. The involvement of growth cartilage is the cause of an additional deforming factor over and above the usually progressive process. Varus and cavovarus deformities with clawtoes occur most frequently. Owing to the thickness of the joint cartilage, there is mostly no severe functi

Reconstructive foot surgery.

In rheumatoid arthritis (RA) we find a high rate of foot involvements, especially in progressive cases when we see patients submitted for hospital treatment. In about 50% of these patients the ankle joints are involved (25), in juvenile RA even in two thirds of all cases. More than half of all progressive cases show also joint affections of the forefoot. Mainly the metatarsophalangeal joints are involved, especially the first MTP joint. We also can find a high involvement rate of the interphalangeal joint of the big toe. In RA the peripheral toe joints II-V are less frequently involved. Change

The Grice extraarticular subtalar arthrodesis: a clinical review.

One hundred forty-nine Grice procedures performed between 1955 and 1982 were reviewed. The valgus deformities were secondary to poliomyelitis, cerebral palsy, myelodysplasia, flexible flatfoot, clubfoot, congenital vertical talus, and other neuromuscular diseases. Review consisted of radiographic and clinical follow-up. Overall evaluation revealed 46% unsatisfactory results because of both graft failure and residual clinical deformity. Previous unrecognized ankle valgus and instability prejudiced the results in the myelodysplasia and flexible flatfoot. Recommendations include consideration of

Treatment of symptomatic first metatarsal shortened by surgery.

There are several complications that can result from shortening of the first metatarsal in the treatment of bunion. Metatarsalgia and recurrent deformity can be quite disabling. The surgical technique described in this brief report provides improved support of the metatarsal heads in walking while decreasing pain and enables the patient to stand for longer periods of time. The procedure is reserved for salvage of those patients with severe pain and deformity and in whom other salvage procedures have failed.

Foot abnormalities in rheumatoid arthritis.

By anticipating the deformities that can occur in the rheumatoid foot and by carefully and compulsively looking for them, we can more effectively deal with the pathologic conditions. Although we usually equate conservative care with nonoperative treatment, and shoe corrections are certainly important, early operative intervention may be essential. Procedures we must use early if indicated are as follows: decompression of the posterior tibialis tendon and other medical structures in the tarsal tunnel, excision of intermetatarsal bursae, and talonavicular arthrodesis. Furthermore, relating foref

Supramalleolar osteotomy in the management of the rheumatoid hindfoot.

Supracondylar osteotomy is an alternative to other procedures for realignment of the rheumatoid hindfoot. Of 64 hindfoot operations performed since 1964, 12 were supracondylar osteotomies. The specific indication for this procedure was an ankylosed rheumatoid foot with equinovarus deformity and limited range of tarsal motion. Conventional triple arthrodesis in such circumstances creates a rigid foot, further compromising an already damaged ankle. Of the nine feet available for evaluation one to ten years after operation, seven were relieved of pain and two had anterior metatarsalgia from recur
