Another adult with Meier-Gorlin syndrome--insights into the natural history.

The Meier-Gorlin syndrome (MGS) is a rare autosomal recessive disorder, characterized by bilateral microtia, aplasia or hypoplasia of the patellae, and severe intrauterine and post-natal growth retardation. We describe the phenotype and report the medical history of a 25-year-old woman with MGS. Her phenotypic evolution was characterized by severe growth retardation with decelerated growth of the head and subsequently a relatively small head, normal intelligence, alteration of the facial features to a more proportionate appearance, improvement of joint function and incomplete breast developmen

Development of radiographic changes of osteoarthritis in the "Chingford knee" reflects progression of disease or non-standardised positioning of the joint rather than incident disease.

OBJECTIVE: To ascertain the extent to which the "Chingford knee" (that is, contralateral knee of the middle aged, obese, female patient with unilateral knee osteoarthritis (OA)) is a high risk radiographically normal joint as opposed to a knee in which radiographic changes of OA would have been apparent in a more extensive radiographic examination. METHODS: Subjects were 180 obese women, aged 45-64 years, with unilateral knee OA, based on the standing anteroposterior (AP) view. Subjects underwent a series of radiographic knee examinations: semiflexed AP, supine lateral, and Hughston (patellof

The epidemiology of knee and ankle injuries on Macquarie Island.

Macquarie Island is a small, rugged sub-Antarctic island with a scientific research station and a considerable reputation for knee and ankle injuries amongst the Australian Antarctic Division expeditioner population. In order to examine the accuracy of this reputation, a 10-year retrospective analysis of all knee and ankle injuries recorded by the Macquarie Island Medical Officer in the medical logs was undertaken. Knee and ankle injuries comprised 13% of the 2,678 recorded medical consultations. The majority of initial injuries occurred in the field and almost a third occurred during work rel

Exercise therapy for patellofemoral pain syndrome.

BACKGROUND: Patellofemoral pain syndrome (PFPS) is a common problem among adolescents and young adults, characterised by retropatellar pain (behind the kneecap) or peripatellar pain (around the kneecap) when ascending or descending stairs, squatting or sitting with flexed knees. Etiology, structures causing the pain and treatment methods are all debated in literature, but consensus has not been reached so far. Exercise therapy to strengthen the quadriceps is often prescribed, though its efficacy is still debated. OBJECTIVES: This review aims to summarise the evidence of effectiveness of exerc

The use of stereolithography for an unusual patellofemoral disorder.

A patient with habitual patellar dislocation which was not treated successfully by the Elmslie-Trillat procedure is described. In the knee, a unique morphologic feature of the patellofemoral joint was suspected as a cause, and a stereolithographic model was produced from the patient's computed tomography data to determine the pathologic features. Because the solid model confirmed the speculation, additional surgery was done to modify the geometry of the joint. In the surgery, cancellous bone was removed below the cartilage, and the geometry of the cartilage was modified with the subchondral bo

Measurement of patellar tracking: assessment and analysis of the literature.

Patellar tracking is defined as the motion of the patella relative to the femur or femoral groove on knee flexion and extension. Abnormalities of tracking (maltracking) are thought to relate to many disorders of the patellofemoral joint and may be defined easily or may be extremely difficult to observe. Accurate measurement of patellar tracking, and definition of normal tracking, have not been achieved yet in experimental conditions or in clinical conditions. Such information would be valuable in the diagnosis and treatment of patellofemoral disorders. In the current report, the literature is

MR imaging of infrapatellar plica injury.

OBJECTIVE: Injury to the infrapatellar plica (ligamentum mucosum) has not been previously described in the radiology literature to our knowledge. This article shows the MR imaging appearance of injury to the infrapatellar plica. CONCLUSION: Injury to the infrapatellar plica is uncommon but should be considered as a potential source of knee pain, especially if no other evidence indicates internal derangement. MR imaging can reveal a typical appearance for infrapatellar plica injury.

Non perforated septum supra-patellaris mimicking a soft tissue tumour.

Plicae synoviales of the knee are well known to present as anterior knee pain but can also cause blocking, clicking, and swelling of the knee mimicking pathology such as meniscal tears or other internal derangements, and even mono-arthritis. Embryologically, the knee is divided into a medial and lateral compartment and a supra-patellar pouch by thin membranes of synovium. Incomplete involution of these membranes gives rise to a plica. When there is no involution at all, the membrane is called a septum. There are three frequent types of plicae/septa: a suprapatellar, a medial-patellar, and an i

Orthotic devices for treating patellofemoral pain syndrome.

BACKGROUND: Patellofemoral pain syndrome is a frequently reported condition in active adults. A wide variety of conservative treatment strategies have been described. As yet, no optimal strategy has been identified. Application of orthotic devices e.g. knee braces, knee straps, forms of taping of the knee, active training devices, knee sleeves and in-shoe orthotics to support the foot have been advocated to treat this condition. OBJECTIVES: To assess the effectiveness of foot and knee orthotics for treatment of patellofemoral pain syndrome. SEARCH STRATEGY: We searched the Cochrane Musculosk

A syndrome of joint laxity and impaired tendon integrity in lumican- and fibromodulin-deficient mice.

Lumican and fibromodulin regulate the assembly of collagens into higher order fibrils in connective tissues. Here, we show that mice deficient in both of these proteoglycans manifest several clinical features of Ehlers-Danlos syndrome. The Lum(-/-)Fmod(-/-) mice are smaller than their wild type littermates and display gait abnormality, joint laxity, and age-dependent osteoarthritis. Misaligned knee patella, severe knee dysmorphogenesis, and extreme tendon weakness are the likely causes for joint laxity in the double-nulls. Fibromodulin deficiency alone leads to significant reduction in tendon

Appraisal of surgical treatment of 47 cases of patellofemoral instability.

INTRODUCTION: Patellofemoral instability is a common knee disease. Its etiology is complex and variable, with many components making different contributions in each individual, resulting in several distinct clinical presentations. Our goal was to analyze the results of surgical treatment in our hospital over a period of 10 years. PATIENTS AND METHODS: We analyzed 55 knees of 47 patients who underwent surgery for patellofemoral instability and were classified into 2 main groups: proximal realignment and combined proximal and distal realignment. Three other groups were analyzed according to the

Osteochondrosis of the superior pole of the patella: two cases with histologic correlation.

Two cases of osteochondrosis of the superior pole of the patella are reported with histologic findings. Both patients were young girls; one had mild cerebral palsy. Sixteen cases of this disorder have been documented but without histologic study. The histologic features of these two cases showed osteonecrosis with reparative changes. These findings support that this entity is similar to other osteochondroses of the quadriceps mechanism: Osgood-Schlatter disease and Sinding-Larsen-Johansson disease.

Detection of changes in cartilage water content using MRI T2-mapping in vivo.

OBJECTIVES: Osteoarthritis (OA) is the most prevalent chronic disease in the elderly, and it is generally diagnosed at an advanced state when treatment is difficult if not impossible. The early form of OA is characterized by an elevated water content in the cartilage tissue. The purpose of this study was to verify in vivo if changes in the water content of patellar cartilage typically occurring in early OA can be detected using T(2) mapping MRI methods. DESIGN: Twenty healthy volunteers performed 60 knee bends in order to compress their patellar cartilage thereby reducing its water content. M
