Effects of exercise on synovium and cartilage from normal and inflamed knees.

The effect of running activity on normal and inflamed knees was determined by light microscopic (LM) and scanning electron microscopic (SEM) observations on hamster articular cartilage. Animals were split into two groups; one housed in standard cages and one given free access to running wheels. Twenty-one days prior to analysis, half of each group was given an intra-articular injection of lipopolysaccharide (LPS) to cause an inflammation, the other half were uninjected. No remarkable changes were observed by LM in either the control running or nonrunning groups. In contrast, cartilage proteogl

[Quadriceps dysplasia and patellar tilt in objective patellar instability].

PURPOSE OF THE STUDY: Dysplasia of the vastus medialis, a well-known landmark of patellar instability, is difficult to point out pre-operatively. We propose the measurement of patellar tilt on the CT scans in order to appreciate abnormality. MATERIAL AND METHODS: We studied 3 well defined groups: 143 knees operated on for a true dislocation of the patella, either for the first time or a recurrent episode, 67 asymptomatic and nonoperated contralateral knees and 54 control knees. The patellar tilt in extension was measured on the CT-scan with the quadriceps relaxed and contracted. RESULTS: The

Insulin antagonistic effects of growth hormone in short children.

Growth hormone has several insulin antagonistic effects. To determine the time course of these effects in growth-hormone-treated children, the frequently samples intravenous glucose tolerance test was used to measure insulin sensitivity (SI) and glucose effectiveness (Sg) before, and 1 week, 1 month and 6 months after beginning growth hormone therapy in 3 patients with growth hormone deficiency (GHD), 3 patients with non-growth-hormone-deficient short stature (NGHD) and 3 with Turner syndrome (TS). Pretreatment SI was lower in TS than in the other two groups (p < 0.05), but Sg did not differ b

XYY syndrome and other Y chromosome polysomies. Mental status and psychosocial functioning.

In this report we review the data on 75 male patients with extra Y chromosome diagnosed in Leuven in the period 1968-1993 among 98,725 patients (males and females) referred for constitutional chromosomal analysis. Special attention was given to their mental performance and psychosocial functioning. 1. Fifty male with 47,XYY karyotype were diagnosed. This is very close to the incidence of XYY in newborn studies and indicates that the frequency of MR/MCA is not increased in XYY male in general. 2. In the 60 patients with "pure" Y chromosome polysomy, the most frequent indication for karyotyping

Chronic septic arthritis and osteomyelitis in a prosthetic knee joint due to Clostridium difficile.

A case of chronic septic arthritis and osteomyelitis in a prosthetic knee joint due to Clostridium difficile is reported. A knee prosthesis was installed in a 16-year-old boy for surgical treatment of an osteosarcoma of the femur. Later, the patient suffered a traumatic closed fracture of his patella, and a sterile fluid was aspirated. One month later, the joint displayed inflammation. Culture of the articular fluid yielded a nontoxigenic Clostridium difficile strain. Despite several attempts using conservative medical treatment with penicillins and ornidazole, Clostridium difficile strains wi

Aplasia cutis congenita associated with limb, eye, and brain anomalies in sibs: a variant of the Adams-Oliver syndrome?

Aplasia cutis congenita (ACC) may occur in isolation or with other congenital malformations. Peripheral limb anomalies and ACC are major elements of the Adams-Oliver syndrome, which is usually inherited as an autosomal dominant disorder. We report on a sister and brother with ACC and brain, eyes, and transverse limb anomalies. The phalanges of the hands and feet were either short or absent. The girl also had absence of right patella, was severely mentally retarded and blind with retinal nonattachment. The boy had a falciform fold in the left eye. He died at age one week and autopsy showed part

[Arthroscopic meniscectomy in anterior cruciate ligament deficient knees].

Results of 107 meniscectomies in 100 patients with long-lasting ACL deficiency are presented. Meniscectomy aggravates symptoms of ACL deficiency, worsening function of the knee. Bucket handle tear was observed most often--45 cases (36 displaced ones). The patients were reevaluated 3 years after surgery on an average using Tegner scale and pivot shift test. Tegner scale assessment rendered 62 per cent of excellent and good results, 32 per cent fair and poor results. Pivot shift was found in 21 patients prior to meniscectomy and in 37 patients at the follow-up examination; 18 patients were direc

Decreased torque and electromyographic activity in the extensor thigh muscles in chondromalacia patellae.

The alterations in thigh muscle properties of chondromalacia patellae patients during isometric and dynamic endurance tests were studied using a variokinetic knee testing system linked to surface EMG. A total of 41 patients (chondromalacia group) with arthroscopically certified chondromalacia of the patella were studied. The control group consisted of 31 healthy adult volunteers with no history of knee pain or trauma. Peak torque values were 21% (p < 0.01) and force output values 25% (p < 0.05) lower on the symptomatic side of the chondromalacia group than in the control group. The decrease in

Histological comparison of patellar cartilage degeneration between chondromalacia in youth and osteoarthritis in aging.

The histological findings of the patellar cartilage were compared between cases of chondromalacia, which occurs predominantly in young persons (22 patients, average age 19.8 years) and cases of osteoarthritis, which is common among the elderly (21 patients, average age 65.4 years). The histological findings of cartilage in the chondromalacia were characterized by increased density and vigorous fibrous metaplasia of chondrocytes. These findings may be considered to represent a reactive change in the chondrocyte. Cartilage degeneration in osteoarthritis, by contrast, is regressive and presents a

Total knee arthroplasty after a previous patellectomy.

We performed a retrospective, matched-control analysis of the results of total knee arthroplasty with cement in twenty-two patients who had had a previous patellectomy for either a fracture of the patella (sixteen patients) or severe patellofemoral osteoarthrosis or chondromalacia patellae (six patients). Nine of the patients were men and thirteen were women. The average age at the time of the total knee arthroplasty was sixty-nine years (range, fifty-nine to seventy-four years). The average time from the patellectomy to the total knee arthroplasty was eight years (range, two to fourteen years

Uncommon causes of knee pain in the athlete.

There are many causes of knee pain in the athlete. Clearly, common things, such as meniscal tears, ligament sprains, contusions, and patellofemoral dysfunction, are common. The infrequency of other entities, however, should not make the physician complacent. A thorough history and physical examination will, in most cases, elicit inconsistencies in the occasional athlete presenting with one of these uncommon causes of knee pain (Table 1). Diagnosing these uncommon causes, when they do present to the health care provider, allows for early and correct treatment with a more rapid return to sports.

Healing of the patellar tendon after harvesting of its mid-third for anterior cruciate ligament reconstruction and evolution of the unclosed donor site defect.

The purpose of this study was (a) to evaluate by ultrasonography the healing of the patellar tendon after its mid-third was removed for anterior cruciate ligament (ACL) reconstruction in two randomized groups of patients in whom the tendon donor site was either left open or closed; (b) to compare clinical, radiographic, and isokinetic studies of these two groups to evaluate the incidence of patellofemoral disorders. We performed 61 ACL reconstructions (22 males, 39 females) using the arthroscopically assisted in-out technique. All operations were performed by the same surgeon, and the patients

Patella partita.

Patella partita is a malformation of the knee cap, generally characterized by one or a few vertical lines of discontinuity in sagittal planes, caused by incomplete ossification within a single ossification center. In other cases, disorder of ossification in frontal planes results in frontal subdivision of the patella. While patella partita is sometimes diagnosed as a coincidental radiographic finding, in other cases, especially in elderly individuals, it is a finding associated with symptoms which require treatment.

Medial patellar synovial plica syndrome: the influence of associated pathology on long-term results.

The results obtained after 4-8 years in 42 patients submitted to arthroscopy for the treatment of medial patellar synovial plica syndrome are analyzed. There was a considerable difference in the values obtained depending on whether the plica was isolated, or associated with meniscal or cartilaginous lesion. In fact, based on the Lysholm score, treatment of isolated plica obtained 17 excellent or good results in 21 patients, while in the 21 cases where the lesion was associated with other pathologies excellent or good results were observed in only 9 subjects. Thus, associated lesions definitely

Further delineation of the ear, patella, short stature syndrome (Meier-Gorlin syndrome).

Two daughters of phenotypically normal parents are described with severe proportional dwarfism with microcephaly, peculiar craniofacial anomalies, microtia, absent patellae, joint hyperextensibility, and other anomalies. Intrafamilial variability is minimal. This combination of anomalies has many similarities to the six cases previously described with the Ear, Patellae, Short stature syndrome (Meier-Gorlin syndrome), which is distinguished by the triad of microtia, absent patellae and growth retardation. Autosomal recessive inheritance is strongly suggested by the presence of two pairs of affe

Changes in the mechanical properties of patellar tendon preparations of spontaneously diabetic dogs under long-term insulin therapy.

Diabetes mellitus is known to involve a wide range of musculoskeletal disorders including tendon contracture, tenosynovitis, joint stiffness, and osteoporosis. Recent studies with experimentally induced models of diabetes show that insulin therapy mitigates such alterations. These data are from relatively short-term experiments. This manuscript reports the results of tensile failure experiments on the canine patella-patellar tendon-tibia complex from a group of juvenile diabetic animals that received insulin therapy. The duration of the disease was 4-9 yr. The stiffness of the diabetic prepara

Reactivation of rheumatoid arthritis in knees following total knee replacement.

Between May 1985 and December 1990, 20 patients with rheumatoid arthritis had 34 total knee replacements performed. The mean follow-up period was 4.1 years and the range was 2.3 to 7.8 years. Five knees with total knee replacements (74.7%) in 4 patients were affected by activation of the rheumatoid process at an average postoperative interval of 28.6 months while the other 29 knees with total knee replacements (85.3%) remained in remission. Factors that appeared to have an influence on post total knee replacement rheumatoid activation included the activity of the disease preoperatively, and wh

[Magnetic resonance of the cartilages of the large joints].

MRI of the articular cartilage requires a careful technical approach since this structure is very thin, with a peculiar internal architecture between the supporting matrix and the cell component. On MR images the normal articular cartilage has a zonal appearance. To optimize the variables for best visualization of the internal architecture of the hyaline articular cartilage, an ex vivo and in vivo study was carried out. Accurate T1 and T2 relaxation times of the articular cartilage were obtained with a particular mixed sequence and then used to create isocontrast intensity graphs. The latter a

Pinhole scintigraphic sign of chondromalacia patellae in older subjects: a prospective assessment with differential diagnosis.

UNLABELLED: Chondromalacia patellae (CP) is an important cause of anterior knee pain. Two clinical types are known: one that typically affects young subjects and the one that affects older patients. The primary diagnostic approach is radiography reinforced with arthrography. A 99mTc-MDP bone scan is invaluable in the study of bone diseases especially when augmented with pinhole scintigraphy (PS). In this study previously unknown, specific sign of CP demonstrated by PS in six middle-aged and elderly patients is described. METHODS: Noting an increased patellar uptake in a planar spot view, a me

AT1 receptor characteristics of angiotensin analogue binding in human synovium.

1. Angiotensin II (AII) reduces blood flow, modulates vascular remodelling and is a growth factor. Human inflammatory arthritides are characterized by synovial hypoperfusion, hypoxia and proliferation. We aimed to localize and characterize receptors for AII in human synovium. 2. We used quantitative in vitro receptor autoradiography with [125I]-(Sar1, Ile8)AII and [125I]-AII on human synovium from patients with chondromalacia patellae, osteoarthritis and rheumatoid arthritis. 3. [125I]-(Sar1, Ile8)AII and [125I]-AII bound to similar sites on synovial blood vessels, lining cells and stroma. Bin
