Magnetic resonance imaging of osteonecrosis and osteoarthrosis in exercising quadriplegics and paraplegics.

Degenerative bone and joint diseases that commonly accompany spinal cord injury (SCI) may be exacerbated by physical activity of the lower extremities. To determine if orthopaedic stress imposed by electrically stimulated cycling exercise is associated with osteonecrosis or osteoarthrosis in persons with chronic SCI, ten quadriplegics and paraplegics who had each undergone electrical stimulation cycle ergometry exercise two or three times weekly for longer than 1.5 years underwent magnetic resonance examinations of the femoral heads and the dominant knee. Three of the ten subjects had no evide

Detection and staging of chondromalacia patellae: relative efficacies of conventional MR imaging, MR arthrography, and CT arthrography.

OBJECTIVE: Chondromalacia patellae is a condition characterized by softening, fraying, and ulceration of patellar articular cartilage. We compare the sensitivity, specificity, and accuracy of conventional MR imaging, MR arthrography, and CT arthrography in detecting and staging this abnormality. SUBJECTS AND METHODS: Twenty-seven patients with pain in the anterior part of the knee were prospectively examined with MR imaging, including T1-weighted (650/16), proton density-weighted (2000/20), T2-weighted (2000/80), and spoiled two-dimensional gradient-recalled acquisition in the steady state (S

MRI of the patellofemoral joint.

In routine MRI of the knee, less attention frequently is paid to the patellofemoral joint and its surrounding structures than to the menisci and ligaments. This article reviews MRI of common abnormalities of the anterior aspect of the knee. Chondromalacia patellae, pathological softening of the patellar cartilage, can be diagnosed with reasonable accuracy with MRI using standard sequences. Abnormalities of alignment can be assessed grossly on static images, but more sophisticated evaluation of patellofemoral tracking requires the kinematic technique. Other traumatic or overuse injuries involvi

Lateral release in chondromalacia patellae using clinical, radiologic, electromyographic, and muscle force testing evaluation.

Lateral release (division of the lateral retinaculum of the patella) is a common operative procedure to improve alignment of the patella in patients with chondromalacia patellae. We investigated the effects of lateral release using clinical follow-up and studied the changes in the radiologic measurements of the patellar alignment and electromyographic function of the stabilizing muscles of the knee, as well as using muscle-force tests of the thigh. The study included 75 patients. Diagnostic arthroscopy was done for all patients. In 42 patients with obvious incongruency of the patellofemoral jo

Chondromalacia patellae: fat-suppressed MR imaging.

PURPOSE: To evaluate the accuracy of fat-suppressed magnetic resonance (MR) imaging in diagnosing chondromalacia patellae. MATERIALS AND METHODS: Seventy-one patients underwent fat-suppressed MR imaging and arthroscopy of the patellofemoral compartment. Findings were classified as early or advanced chondromalacia or as normal and were correlated with arthroscopic findings. RESULTS: Early and advanced stages of chondromalacia patellae were reliably detected, with positive predictive values of 85% and 92%, respectively. Specificity in early stages was 94% and in late stages was 98%. However, t

Comparison of axial T1 spin-echo and T1 fat-saturation magnetic resonance imaging techniques in the diagnosis of chondromalacia patellae.

To test the diagnostic value of T1 spin-echo and T1 fat-saturated magnetic resonance images (MRIs), we reviewed axial T1-weighted images with and without fat saturation in 20 patients with clinically suspected chondromalacia of the patella. All scans were obtained on 1.5-MR units. The scans were randomly ordered and reviewed independently at different times by two radiologists without knowledge of the arthroscopy results. The sensitivity of the individual techniques for detecting grade 3 or 4 chondromalacia patellae was 92% for fat-saturated axial T1-weighted images alone, and 67% for axial T1

Referred knee pain in a young athlete: a case study.

Parapatellar pain is a common complaint in the active adolescent patient population. Patello-femoral pain syndrome, Osgood-Schlatter disease, Sinding-Larsen-Johansson syndrome, patellar tendinitis, and other stress failure conditions are the primary causes of these parapatellar symptoms. Not all cases of knee discomfort are related to knee pathology. This case study discusses hip pathology as a source of referred knee pain in an 8-year-old male athlete. Care must be taken to evaluate all possible sources of both primary and referred pain in all cases.

Radiological measurements in patellofemoral disorders. A review.

This paper analyzes some of the radiologic measurements used in the diagnosis and management of patellofemoral disorders. Measurements such as condylar width and height and patellar height from the true lateral view are helpful to determine the type and severity of intercondylar dysplasia. Skyline views provide information on the type and degree of patellar subluxation and dysplasia, especially of the medial aspect of the trochlear groove. In asymptomatic subjects, the trochlear angle does not exceed 140 degrees, and the lateral medial intercondylar ratio in dysplasia is > 1.7. Scanograms spec

[Treatment of Bell's palsy: Report of 36 cases].

BACKGROUND: Immune checkpoint inhibitors, including antibodies against programmed death 1 (PD-1) and cytotoxic T-lymphocyte antigen 4 (CTLA-4), are being used with increasing frequency for the treatment of cancer. Immune-related adverse events (irAEs) including colitis, dermatitis, and pneumonitis are well described, but less frequent events are now emerging with larger numbers of patients treated. Herein we describe the incidence and spectrum of thrombocytopenia following immune checkpoint inhibitor therapy and two severe cases of idiopathic thrombocytopenic purpura (ITP). CASE PRESENTATIONS

Total knee arthroplasty without patellar resurfacing. Clinical outcomes and long-term follow-up evaluation.

Total knee arthroplasty (TKA) without resurfacing of the patella by a single surgeon was reviewed retrospectively in 125 patients: 66 patients (79 knees) were included in the final study group, 32 patients were deceased, 13 were without current address and not able to be contacted, and 14 patients were unable to complete a questionnaire because of severe illness or language barrier. All patients had a diagnosis of osteoarthrosis. The decision to leave the patella unresurfaced was based on the presence of satisfactory patellar articular cartilage, absence of eburnated bone, congruent patellofem

[Surgical treatment of habitual patella dislocations by combined soft tissue surgery].

Since January 1988 21 operations using a modified Madigan and Goldthwait method have been carried out on 18 patients suffering from recurrent dislocation of the patella. This soft tissue procedure is presented here in its individual operational steps. The average age of the patients was 22 years and the majority were female (6:1). The mean follow-up time was 19.3 months. One patient who had both knees operated on reported recurrent luxation 6 months after the operation in one knee and subluxation in the other. None of the other patients suffered further dislocations. In the opinion of the pati

Acute traumatic knee hemarthrosis.

A rapidly developing hemarthrosis in an acutely injured knee is reported to be associated with "surgically significant" lesions, such as anterior cruciate ligament tears, meniscus tears, and osteochondral fractures, in < or = 90% of cases. We report two cases of adult men who showed signs of such acute hemarthroses in stable knees after significant trauma. Neither had significant intraarticular pathology, such as meniscal, articular, or ligamentous injury, at arthroscopy. Persistent postarthroscopic bleeding led to additional diagnostic testing that found mild factor VIII deficiency. Hemophili

Recurrent dislocation of the patella in Kabuki make-up syndrome.

Three patients with Kabuki make-up syndrome (KMS) who had recurrent dislocation of the patella are reported. In all, dislocation of the patella developed in adolescence, and they had generalized ligamentous laxity and dysplasia of the patellofemoral joints. In two of the three patients, surgical intervention was required; the operation was successful in one, but yielded poor results in the other. In the third patient, cardiac complications precluded operation. Orthopedists should be aware of this possible complication when treating patients with KMS.

Seronegative spondylarthropathy without spine involvement in Behçet's syndrome.

The case of a 49-year-old man affected by Behçet's syndrome (BS) without any clinical or radiological evidence of ankylosing spondylitis, exhibiting a peripheral enthesitis typical of seronegative spondyloarthropathy (SpA) is reported. The diagnosis of SpA is supported by computed tomographic evidence of sacroiliitis. This case confirms our hypothesis that patients with BS may have other forms of SpA than AS.

Magnetic resonance imaging in the diagnostic management of diseases of the knee.

Magnetic resonance imaging (MRI) of the knee is fast assuming a major role in evaluating disorders of the knee. The major advantage of MRI is that it is a totally non-invasive procedure with superior soft tissue contrast resolution and multi-planar capabilities. In contrast to arthrography, intra-articular as well as extra-articular structures in the knee can be evaluated by MRI. Indications include studying abnormalities of the menisci, ligament, patella and quadriceps mechanisms, articular cartilage, bone, bursa and other soft tissue. A review of the literature comparing MRI to arthrography

Patellofemoral pain syndrome.

Patellofemoral pain syndrome is a common problem that occurs in association with mild malalignment of the extensor mechanism of the knee or as a result of repetitive microtrauma from overuse. Patients most often present with retropatellar or peripatellar knee pain, which becomes especially noticeable when they climb or descend stairs. The diagnosis is usually made from the history and the physical examination, with relatively little information gained from other diagnostic aids. Treatment involves physical therapy, anti-inflammatory medication and the application of ice. In most patients, surg

Cartilage degeneration in the human patellae and its relationship to the mineralisation of the underlying bone: a key to the understanding of chondromalacia patellae and femoropatellar arthrosis?

According to the literature subchondral bone plays a significant role in the transmission of load through joints and in the pathogenesis of osteoarthrosis. Therefore the degeneration of the articular cartilage was investigated in the patellae from 30 dissecting-room specimens and of 20 patients, previously submitted to arthroscopy, and subchondral mineralisation of their underlying bone was at the same time assessed by means of CT osteoabsorptiometry. Lateral cartilage lesions were localised over highly mineralised subchondral bone; these appear to be due to long-term stress. They were mainly

Synovial plicae and chondromalacia patellae: correlation of results of CT arthrography with results of arthroscopy.

The purpose of this study was to compare computed tomographic (CT) arthrography with arthroscopy in the evaluation of patients with anterior knee pain. The authors studied 23 patients with patellar plicae at CT arthrography. Twenty of 21 patients presumed to have medial plicae at arthroscopy had similar findings at CT arthrography (sensitivity, 95%; specificity, 100%). However, 10 of 12 suprapatellar plicae and 11 of 11 lateral plicae seen at CT arthrography were not seen at arthroscopy. All 11 patients with a diagnosis of no or mild chondromalacia patellae at arthroscopy had similar CT arthro

Structural and in vivo mechanical characterization of canine patellar cartilage: a closed chondromalacia patellae model.

The purpose of this project was to study the relationship between the structure of the patellar cartilage and its response to static compressive loading with a closed chondromalacia patellae model. An animal model was used to induce degeneration of the patella that was monitored quantitatively and qualitatively as a function of time. Ten adult mongrel dogs had their left patellofemoral groove replaced by a customized metallic implant covered with a thin film of polyethylene for periods of 3 months (five dogs) and 6 months (five dogs). An indenter was designed to perform mechanical indentation
