Lower limb injuries present the greatest source of medical problems during basic military training. These main overuse lower limb injuries, anterior compartment syndrome, stress fractures, Achilles tendinitis, plantar fasciitis, shin splints, and chondromalacia patellae, are reviewed with respect to current knowledge of rates, diagnosis, and treatment. Part 2 shall review possible etiological factors involved in the causation of these injuries.

The long-term results (9-87 months) of the arthroscopic treatment of unilateral mediopatellar synovial plica of the knee in 102 athletes are reported. In all patients a large and/or fibrotic inflamed mediopatellar plica impinging on the femoral condyle on knee flexion was identified and trimmed. Chondromalacia patellae was detected in 38 patients. Open lateral release of the patella, combined with knife arthroscopy was carried out unilaterally in 47 patients with chondromalacia patella, who had received systematic conservative treatments and not responded on it before this surgical procedure. The present study proposes a new MRI-specific staging of chondromalacia patellae (CMP) which is based on cartilage thickness decrease and signal intensity behaviour under compression as well as cartilage morphology in the plain image. The investigation was performed in 30 patients with varying knee complaints who underwent arthroscopy after MR imaging. It was demonstrated that three CMP stages can already be differentiated by MRI under compression in arthroscopically healthy cartilage. This proves a marked improvement in the early diagnosis of CMP.

Posterior dislocation of the prosthesis after total knee arthroplasty is an infrequent but serious complication. Seven patients with this complication were treated from January 1985 until October 1989. Patellofemoral dysfunction may be one of the most common and troublesome maladies to affect a patient. The correct identification of the underlying causes is paramount. Once identified, an effective rehabilitation program should be implemented. The program should include stretching and strengthening as well as several other therapeutic modalities. An independent assessment was performed of the results of woven carbon implants of mesh or rods into articular cartilage defects performed by two surgeons in the knees of 96 patients. Eight had symptomatic osteochondritis, 31 had chondromalacia patellae and 57 had early osteoarthritis. They were reviewed up to 5 yr after operation. Malalignments of the patellofemoral joints were artificially made in eleven rabbits using four different methods in order to generate chondromalacia of the patella. Chondromalacic changes and the location similar to the chondromalacia of the human body were generated in one joint of the patching group. According to the results of our investigation, the most reasonable etiological factor of the chondromalacia is thought to be the expansion of the medial retinaculum and capsule of the knee joint, and chondromalacia patellae may not be a precursor of osteoarthritic changes.

Widespread misuse of the term chondromalacia has led to confusion regarding its true meaning. This term should not be used to diagnose patellofemoral pain, but to describe lesions of articular cartilage. Recent advances using stereophotogrammetry to evaluate the patellofemoral joint and its articular cartilage may lead to new insight into the etiology of these lesions.
