Patellofemoral compressive force was measured before and after anterior displacement of the tibial tuberosity for patellofemoral disorders on 30 knees in 28 patients. All patients selected for this experiment were those who obtained relief of pain in the activities of daily living. Five hundred questionnaires were distributed to long-distance runners who had used, or who were using orthotic shoe inserts for symptomatic relief of lower extremity complaints. Chondromalacia patellae is one of the common causes of patellofemoral disorders in young subjects. It frequently occurs in patients with lateral subluxation of the patella. Patellofemoral osteoarthritis is a common disease which may occur alone or in association with tibiofemoral gonarthrosis. A series of 45 patients with chondromalacia patellae demonstrated clinically and by arthrotomy, but without demonstrable biomechanical misalignment, was investigated retrospectively. Thirty-one of the patients had only been treated conservatively while partial chondrectomy was performed in 14. All of the patients underwent an intensive rehabilitation program. After 12 years the patients were interviewed. In the course of a review of the literature the definition, etiology, diagnosis and treatment of the disease entities chondropathy and patellar chondromalacia are compared. Without exception, the descriptions found are inconsistent and inaccurate. A new classification--chondromalacia, patellar and peripatellar syndrome--is proposed. Possible causes of these diseases in the subchrondral vascular system are mentioned explicitly ("patellar migraine", "patellar claudication").

150 knees were re-examined after operative therapy because of chondromalacia patellae. We found good and very good results in 52.7%. The results after lateral release were statistically significant better than the results after after the other methods. Because of 82% good results the lateral release operation can be recommended.

An arthroscopic procedure of excision of damaged cartilage and drilling the subchondral bone plate for treatment of chondromalacia patellae is described. Satisfactory results and absence of major complications are reported in a preliminary, short-term series of 24 cases of symptomatic posttraumatic chondromalacia patellae, with a mean follow up of 12 months.

The effect of glycosaminoglycan polysulfate (GAGPS) on damaged patellar cartilage and clinical symptoms of chondromalacia was studied on 31 patients in a placebo-controlled double-blind trial. Fifty-three consecutive patients were treated for a symptomatic plica mediopatellaris of the knee joint with arthroscopic resection of the plica. Forty-six patients were followed up 43 months after the operation. Preoperatively, 40% of the patients had anterior knee pain on exertion, and 60% had mechanical symptoms. We report on the case of a 36 year-old woman with nail-patella syndrome that was detected following a plain roentgenogram of the abdomen showing iliac horns. The nail-patella syndrome (NPS) is a hereditary disorder of tissues of ectodermal and mesodermal origin transmitted as an autosomal-dominant trait. Complete NPS is characterised by a series of skeletal disorders and specific nephropathy; in the recent years some other diseases associated with NPS were described, whereas many patients exist with incomplete NPS.
