Magnetic resonance imaging of the knee: an overview.

Magnetic resonance imaging (MRI) provides a new and important tool for investigating knee disorders. This article reviews the basic techniques available for investigation of the knee using MRI and describes the anatomy as delineated by this method. A large number of disorders are best evaluated by MRI, including internal abnormalities such as those of the cruciate ligaments and the menisci. Other disorders include abnormalities of the patella and quadriceps mechanism, cartilage, and synovium. Bony abnormalities such as osteonecrosis and tumors, as well as bone contusions and fractures, also ma

[Injuries and damage caused by excessive stress in classical ballet].

Typical injuries of highly professional classical ballet dancers were evaluated in 108 members of German opera companies. We demonstrated the predominance of overuse-syndromes to acute injuries related to the excessive training-extends. Disorders of the feet, particularly tendinitis followed by low-back pain, chondromalacia patellae, jumpers knee, shin splints and muscle strains are most frequent. Considerable time often elapsed before diagnosing was made because complaints were ignored by dancers. Our inquiry includes range of motion of the spine and the extremities, serial x-rays and a syste

[Transposition of the tibial tuberosity in recurrent dislocations and painful femoro-patellar syndromes. Study of a continuous series of 140 operated knees].

Surgical displacement of the tibial tuberosity has been performed in 140 cases over a 10-year period. Medial displacement with or without ventralization of the tuberosity gave excellent results in all 11 cases presenting with recurrent patellar dislocation. Anterior displacement of the tuberosity using a modified Maquet procedure was performed in 129 knees presenting with femoro-patellar pain (chondromalacia patellae, femoro-patellar osteoarthritis). Indication for surgery was intractable femoropatellar pain. Surgical technique (isolated anterior translocation or anterior and internal transloc

Anterior knee pain: diagnosis and management.

Anterior knee pain is caused by a number of distinct conditions which can only be diagnosed accurately by arthroscopy. Chondromalacia patellae is an important cause of disability in young people which rarely progresses to osteoarthritis. Treatment is by non-operative quadriceps strengthening exercises or by conservative surgery combined with careful supervised rehabilitation.

The role of arthroscopy in children and adolescents.

A retrospective review of the first 5 years' experience with arthroscopy at the Adelaide Children's Hospital was conducted. Two hundred and twelve arthroscopies were performed in 192 patients. The average period of follow-up was 5.9 months. The most common arthroscopic finding was anterior cruciate ligament (ACL) injury, followed by meniscal lesions, chondromalacia patellae, patellar dislocations, and osteochondritis dissecans. There was a high rate of associated lesions with ACL tears and acute patellar dislocation. Septic arthritis responded well to arthroscopic drainage and lavage. We concl

Anterior Knee Pain (Chondromalacia Patellae).

In brief: Anterior knee pain (chondromalacia patellae) may be the most troublesome, ubiquitous overuse syndrome in the sports world. Complaints of insidious onset of vague, activity-related pain coupled with evidence of wasting of the vastus medialis muscle usually establish the diagnosis. The two major goals of treatment-alleviating the symptoms and strengthening the vastus medialis-can often be attained with minimal intrusion into the patient's athletic endeavors. Treatment may include nonsteroidal anti-inflammatories, icing, and physical therapy modalities (eg, high-intensity galvanic stimu

Chondromalacia induced by patellar subluxation in the rabbit.

A unilateral patellar malalignment was induced in 20 young and 20 mature rabbits by lateral displacement of the tibial tuberosity, the other knee serving as osteotomized in situ control. At 6 weeks, all the knees appeared macroscopically normal, but histologically definite cartilage degeneration was found on the experimental side. At 3 months, macroscopic changes occurred in 5 of 10 mature rabbits, and histologic cartilage degeneration was found in all the experimental knees, most pronounced in mature animals, and particularly in joint facets submitted to high pressure. This experimental model

[Comparative clinical and arthroscopic study of the knee joint in chondromalacia patellae].

It is not easy to perform clinical diagnosis of chondromalacia patellae. Many cases take a clinically silent course and are discovered by chance only. Analysis for 275 arthroscopies resulted in almost complete agreement between clinical and arthroscopic diagnosis if the damage to the cartilage was diagnosed before arthroscopy. On the other hand, in chondromalacia that remained clinically silent, damage to the cartilage was present in 67% of the cases as detected by arthroscopy. The authors offer detailed comments on the problem of localisation of cartilaginous changes.

Insall proximal realignment for disorders of the patella.

Proximal realignment with standard lateral retinacular release (Insall operation) was performed on 35 knees with a follow-up time of two to 11 years. The results for chondromalacia patellae were satisfactory to excellent results in 20 of 23 knees (87%) at two years and in eight of 15 knees (55%) at five to 11 years. Satisfactory to excellent results in 11 of 12 knees (92%) were noted for recurrent patellar dislocation. This procedure should be reserved for advanced chondromalacia patellae (Grades III and IV) and recurrent patellar dislocation. Anatomic studies suggest that the pain relief was

The role of radiography in the evaluation and treatment of common anarthrotic disorders of the patellofemoral joint.

Patellofemoral disorders are among the most common knee problems in the world. Although they are often considered as a group, pathologically the individual disorders are as variable as fingerprints. Treatment of resistant cases requires a detailed understanding of the disorder mechanism to be corrected. Many radiographic techniques, incorporating plain radiography, arthrography, CT, and MRI have been formulated to determine formats of chondromalacia, instability, and malalignment. Many of the techniques reviewed herein reveal distinct abnormalities but do not necessarily provide insight into t

Joint pain in children. When is it serious?

Diagnosis of the cause of joint pain in children depends on the physician's ability to distinguish benign from more serious joint conditions. Benign conditions include trauma, overuse syndromes, hypermobility syndrome, chondromalacia patellae, benign recurrent limb pains, and psychogenic rheumatism. Conditions that require further evaluation and ongoing therapy include Lyme disease, rheumatic fever, juvenile rheumatoid arthritis, neoplastic disease, various orthopedic conditions, infection, seronegative spondyloarthropathies, and the rarer connective tissue diseases. If diagnosed early and tre

Degenerative changes in dog articular cartilage induced by a unilateral tibial valgus osteotomy.

We present preliminary details of the changes that occur in a new model for the study of degenerative arthritis which does not involve intraarticular surgery. A unilateral valgus tibial osteotomy was performed in a group of 8 skeletally mature dogs of mixed breed and sex. Proximal tibial cartilages, femoral condyles and patellas were examined in operated and unoperated knees of experimental and unoperated dogs. Lesions were examined histologically and analyses were made of 35SO4 uptake and uronic acid content as measures of proteoglycan synthesis and content, respectively. Water contents were

[The small patella syndrome. A combination of knee and pelvic dysplasia].

In 1979, using the name "small patella syndrome", Scott and Taor described a family in which hypoplasia or aplasia of the patella was inherited by autosomal dominant transmission. Hip dysplasias and pelvic changes accompanying the patellar changes have also been observed in some cases. On the basis of three personal observations this dysplasia syndrome is described in more detail and compared with the other syndromes involving the kneecap and pelvis. One case was discovered as a result of a femoropatellar pain syndrome, a second because of slight functional disturbances. The third case was a c

Osteoarthrosis in a knee joint due to chronic posttraumatic insufficiency of the medial collateral ligament. Nine-year follow-up.

The purpose of the present study was to examine and describe the osteoarthrotic changes that chronic, partial or complete medial collateral ligament (MCL) insufficiency causes to the knee joint. Forty-eight patients with partial and 25 with complete, isolated rupture of the MCL had a clinical and radiological reexamination of the knee on an average of nine years after the injury. In both groups the posttraumatic osteoarthrotic changes caused by the MCL insufficiency occurred very frequently on the medial side of the knee. The most characteristic findings were osteophytes and subchondral sclero

Tibial sequestrectomy in the management of Osgood-Schlatter disease.

Two groups of patients with symptomatic Osgood-Schlatter disease were compared over a 4-5 year follow-up period. One group was treated surgically by tibial sequestrectomy and the other group was managed conservatively. Tibial sequestrectomy was found to offer no significant benefit over simple conservative methods of treatment. In addition, a significant complication rate was identified with this procedure.

Review of pathogenesis and treatment of degenerative joint disease.

Treatment of degenerative joint disease (osteoarthritis) is discussed in relationship to pathogenesis of five clinical entities, defined to facilitate discussion:-1) occurring in high motion joints and associated with synovitis; 2) associated with low motion joints; 3) "non-progressive" articular cartilage erosion; 4) secondary to other identified problems (intra-articular fractures, ligamentous damage, wounds, septic arthritis, osteochondrosis); and 5) chondromalacia of the patella. In addition to direct damage to articular cartilage, synovitis and capsulitis and depletion of matrical glycosa

[Imaging of enthesopathies of the ligamentum patellae in athletes. Echography and computerized telethermography].

The authors examine the diagnostic value of two non-invasive techniques--i.e. ultrasound and computerized teletermography--in the diagnosis of patellar enthesopathies. Such a pathology is quite frequent in young athletes, at various stages. The patients were grouped in two categories, according to their age when the trauma occurred, and to the peculiarities of both enthesopathic localization and imaging. The use of both techniques, either one prevailing over the other according to the different evolutive phases of enthesopathies, provides useful information as to detecting and determining the

Contact pressures in chondromalacia patellae and the effects of capsular reconstructive procedures.

We measured patellofemoral contact areas and pressures using pressure sensitive film in ten human cadaver knees exhibiting degenerative lesions of patellar cartilage. We studied a flexion range from 20 degrees to 90 degrees and compared contact pressures before and after capsular reconstructive procedures. With an intact, normal capsule, localized lesions of grade I-II exhibited a 50% reduction in pressure (from 3.4 +/- 0.7 MPa to 1.6 +/- 0.9 MPa) directly over the lesion. Grade III-IV lesions exhibited a loss of contact pressure greater than 90%. This reduction in pressure appears to result f

The effect of lateral retinacular release in idiopathic chondromalacia patellae.

The results of lateral retinacular release were evaluated in 28 patients with idiopathic chondromalacia patellae. Follow-up was performed 3-5 years after the operation. At follow-up 13 patients were improved, while the symptoms were equal or intensified in 15 patients. However, compared with the situation before the operation, the levels of activity were increased in only two patients, while the levels of activity were unchanged or reduced in the remaining 26 patients. From this study it is concluded that the results of lateral retinacular release for idiopathic chondromalacia patellae are poo
