A review of intra-articular knee injuries in racquet sports diagnosed by arthroscopy.

Because of the recent rapid increase in the number of knee injuries related to racquet sports, the authors undertook a retrospective study of such injuries seen over a 5-year period at the Toronto Western Hospital Sports Medicine Institute. The 121 patients who presented over the study period with a racquet-sports-related knee injury requiring arthroscopy represented 30% of all racquet-sports related injuries seen during that period. The mean age at presentation was 32.8 years and two-thirds of the patients were men. All the major racquet sports were represented. In all, 213 lesions (7 bilater

Chondromalacia patellae.

Chondromalacia patellae is a relatively common condition affecting adolescents or young adults, involving the articular cartilage of the patella and opposing femoral condyles. Etiological factors, clinical features and radiographic signs of this condition are discussed. A conservative treatment approach is put forth emphasizing rehabilitative exercise, bracing and spinal manipulation.

An electron microscopic study of early pathology in chondromalacia of the patella.

For a study of the ultrastructural pathology of chondromalacia patellae, we studied biopsy specimens that had been obtained at operation in twelve young patients who had pain in the anterior aspect of the knee due to mechanical derangement and also had the pathological changes in articular cartilage that are characteristic of chondromalacia. The initial pathological finding was swelling of the superficial matrix associated with breakdown of the collagen fiber network, especially at matrix streaks in the superficial and transitional zones. Fibrillated cartilage contained surface fissures that p

Treatment of chondromalacia patellae by lateral retinacular release of the patella.

A series of 59 knees in 58 patients were surgically treated from 1977 to 1982 for chondromalacia patellae, and were followed for an average of 1.2 to 4.6 years after the operation. The operative procedure was open lateral retinacular release in all knees. In knees with recurrent patellar subluxation, the number of poor results increased from 24% to 70%, and in knees with no subluxation from 21% to 24%. The difference after 4.6 years was statistically significant. No correlation between the severity or location of the cartilage changes and the operative effect was found. Open lateral retinacula

Bilateral spontaneous concurrent rupture of the patellar tendon in the absence of associated local or systemic disease.

A 26-year-old man incurred bilateral spontaneous rupture of the patellar ligament. This may be the eighth case in previously healthy individuals reported in the literature. Further review of other case reports suggested various etiological factors. Surgical repair generally produces excellent results.

Classification of patellofemoral disorders.

Patellofemoral disorders represent a large portion of the average orthopedist's practice. Despite the improvements in patellofemoral radiographs and arthroscopic diagnostic techniques, these disorders are too frequently misunderstood and frustrating to treat. This report proposes a clinical classification for patellofemoral disorders that, it is hoped, will aid our understanding and improve our results of treatment. A major feature of this classification is the recognition that a developmental and familial abnormality, patellofemoral dysplasia, is the etiology for most patellofemoral disorders

Familial recurrent dislocation of patella with autosomal dominant mode of inheritance.

Recurrent dislocation of the patella is considered to be a manifestation of generalized joint laxity. An autosomal dominant mode of inheritance was previously suggested in a few instances; however, male-to-male transmission has not yet been reported. We report a recurrent dislocation of the patella occurring in five generations of one family with male-to-male transmission and without generalized joint laxity.

Tuberculosis of the patella.

The case of an 8-year-old boy with tuberculous arthritis of the right knee in whom patellar disease was the only radiological abnormality is presented. The patellar findings were a large lytic area with sclerotic borders containing a dense image corresponding to a sequestrum. Open biopsy was necessary for diagnosis, since the radiological abnormalities are consistent with other chronic infectious diseases.

[Importance of emission computed tomography in chronic disorders of the knee joint].

Twenty-eight patients with chronic abnormalities of the knee were examined by 3-phase scintigraphy and ECT. The unobscured images produced by ECT provide additional information for evaluating the localisation, extent and activity of the lesions. The diagnosis of meniscus abnormalities in particular revealed pathognomonic patterns. The indications and value of ECT was compared with other diagnostic measures and particularly with invasive techniques (arthroscopy and arthrotomy).

The lumbar facet arthrosis syndrome. Clinical presentation and articular surface changes.

We describe a lumbar facet syndrome in which disabling symptoms are associated with normal or near-normal plain radiographs. Local spinal fusion relieved symptoms in 12 patients; the excised facet joint surfaces showed some of the histological changes seen in chondromalacia patellae and in osteoarthritis of other large joints. The most frequent change was focal full-thickness cartilage necrosis or loss of cartilage with exposure of subchondral bone, but osteophyte formation was remarkably absent in all specimens. We suggest that there are both clinical and histological similarities between the

Chondromalacia patellae: assessment with MR imaging.

Magnetic resonance (MR) images of the posterior patellar hyaline articular cartilage were obtained in 23 subjects to determine if MR imaging could accurately demonstrate the patellar cartilage. Arthroscopy was used as the standard of reference. Three subjects were asymptomatic volunteers. In the remaining 20 who had patellofemoral pain, arthroscopy was performed before MR imaging in seven and afterward in 12; one did not undergo arthroscopy. MR imaging showed focal areas of swelling of the patellar cartilage, focal hypointensity, surface irregularity, areas of thinning, and areas of cartilage

Radiographic evaluation of joints resurfaced with osteochondral shell allografts.

The radiographic features of 41 cadaveric osteochondral shell (low ratio of subchondral bone to articular cartilage) allografts placed in 24 patients for articular resurfacing as an alternative to arthroplasty are presented. Underlying causes of joint disease included ischemic necrosis (20 grafts), osteochondritis dissecans (nine), chondromalacia patellae (10), and posttraumatic osteochondral fracture with degenerative disease (two). Congruity with the adjacent native articular surface and the opposite side of the joint was evident on immediate postoperative radiographs in all patients, and pr

[Patellofemoral pressure and contact surface measurement in the 'hunter's hat patella'. Experimental studies of sagittal patella osteotomy].

One reason of the chondromalacia patellae is the functional incongruence due to a patellofemoral dysplasia like for example the so called hunter's hat deformity. As a curative treatment in those cases an osteotomy of the patella may be carried out. This operation shall improve the congruence of the articular surface as well as the stability of the patella. There are experimental studies describing the femoropatellar pressure situation in eight cases with a surgical induced hunter's hat patella after osteotomy. The results represent a significant increase of the opposing surfaces in the medial

The quadriceps function in patellofemoral disorders. A radiographic and electromyographic study.

The influence of quadriceps contraction on patellar alignment was investigated in tangential views of 31 patients with patellofemoral complaints and compared with 25 controls. The congruence angle decreased significantly following quadriceps contraction in patients with patellar subluxation in contrast to those with chondromalacia and normals. Electromyography (EMG) of the vastus lateralis (VL) and vastus medialis obliquus (VMO) during maximal quadriceps contraction was obtained prior to and after 3 months of isometric quadriceps exercises in 15 knees with patellar subluxation and in 11 with i

[The role of the quadriceps muscle for the patello-femoral malalignment syndrome].

At knee extension on the Cybex Machine II (Lumex, Co. Ltd.), electromyograms (EMG) from the vastus medialis (M) and the vastus lateralis (L) were picked up by surface electrodes. The L/M ratio was calculated by the integrated value of the EMG and the power spectrum dealt with the frequency analysis of the EMG. There was a correlation between the L/M ratio and Q angle for the normal group, a constant relation for the hypermobile patella group, and an inverse correlation for the group with recurrent subluxation of patella. The vastus medialis (M)-dominant group, intermediate group and the vastus

[Congenital hemihypertrophy associated with cutaneous pigmento-vascular, cerebral, visceral and bone abnormalities].

A case of hemihypertrophy associated with multiple anomalies of the skin, bone and visceral organs is presented. A 31-year-old female was admitted for evaluation of her skin conditions. Her family history is noncontributory, while her past history discloses operations for syndactyly of the right foot, tonsillar hypertrophy, anal prolapse and ovarial cyst. Erythemas of the face and the left upper extremity were noticed during the neonatal period and hypertrophy of the right side of the body started at age 2 months. On admission, hemihypertrophy was observed in the face, trunk and extremities. M

Fibular a/hypoplasia: review and documentation of the fibular developmental field.

Fibular aplasia and/or hypoplasia is documented as a developmental field defect and the extent of the fibular developmental field is delineated. The term fibular a/hypoplasia denotes the clinical spectrum of fibular deficiency in different patients and also implies that aplasia can be present in one limb and hypoplasia in the other. Causal heterogeneity of fibular a/hypoplasia is demonstrated, thereby defining it as a developmental field defect. Most cases of fibular a/hypoplasia are isolated, sporadic events. An autosomal dominant form of isolated fibular a/hypoplasia with ankle joint anomaly
