From a series of 72 patients treated with anterior advancement of the tibial tubercle for patellofemoral pain syndrome, secondary lowering of the patella was noted in eight. In all eight patients the results were poor following surgery, with severe disabling retro- and peripatellar pain and atrophy of the quadriceps muscle. Thirty-nine school-age children with Down's syndrome (trisomy 21) were examined for orthopedic problems and evidence of joint laxity. Of these, 10.3% had required orthopedic surgery for feet and hip problems, and 13% had disorders not yet requiring surgery. Patellofemoral pain syndrome can be a difficult condition to manage effectively. The success rate of most treatment regimes has been poor and in the long term the condition frequently recurs. The author has developed a treatment programme which has a ninety-six percent success rate. The efficiency of a knee brace with a 0 degree-30 degrees range of motion was studied in 17 patients with anterior knee pain. Eighteen patients with identical symptoms served as controls. The brace was worn for 6 weeks. Follow-up was performed after 6 weeks and 3 and 12 months. This prospective study confirms that patellofemoral pain arises predominantly in a young population and is more common in females. Patellar malalignment and its progression to patellar subluxation can be managed conservatively with improvement in the majority of cases. Arthroscopic lateral release may be necessary for selected patients with patellar malalignment and subluxation, while more aggressive surgical therapy should be considered in patients with patellar compression syndrome and recurrent dislocation. A 34-year-old Nigerian man had successive ruptures of the ligamenta patellarum repaired 2 years after the first injury. Six boys with fragmentation of the proximal pole of the patella are reported; the condition was bilateral in one. Four of the six presented with symptoms of Osgood-Schlatter's disease or Larsen-Johansson disease of the same or of the contralateral knee, but they had no symptoms or signs relating to the proximal pole of the patella; one also had features suggesting minimal chondromalacia patellae.
