Patellofemoral malalignment is frequently accompanied by pain that may respond to conservative treatment or lateral retinacular release. Frequently, there is little or no evidence of chondromalacia patellae in these patients. This study presents consistent evidence of nerve damage (demyelination and fibrosis) in the lateral retinaculum of patients with intractable patellofemoral pain requiring lateral retinacular release or realignment of the patellofemoral joint. The changes observed in the retinacular nerves resemble the histopathologic picture of Morton's interdigital neuroma. It is likely

We studied the results in forty-seven knees in thirty-seven patients - ten male and twenty-seven female - who had recurrent dislocation of the patella and were treated by a modified Roux-Goldthwait procedure (lateral retinacular release, medial transfer of the lateral patellar tendon without advancement, plication of the medial retinaculum, and advancement of the vastus medialis). Ten of the female patients had bilateral dislocation. The results were analyzed after follow-ups ranging from 3.0 to 16.3 years (average, 5.8 years). The study confirmed that a tangential radiograph of the patellofem

Owing to the largely unsatisfactory results achieved to date following surgery for chondromalacia patellae, and owing to our greater knowledge of the functional relationships within the knee joint, the range of aetiological factors in femoropatellar pain needs to be expanded. Functional variants in the muscular and ligamentous system must be considered as well as anatomical variants. On the basis of symptomatology and the results of clinical examination, two extreme forms can be distinguished, depending on ligamentous laxity and muscular stabilisation capacity. Physiotherapy must be determined

Full thickness samples of articular cartilage were removed from areas of chondromalacia on the medial and "odd" facets of the patellae of 21 adults and examined by histology, autoradiography and electron microscopy. Surface fibrillation, loss of superficial matrix staining and reduced 35SO4 labelling was seen, with little change in the deep zone. Ten cases showed "fibrous metaplasia" of the superficial cartilage with definite evidence of cell division and apparent smoothing of the surface. Scattered chondrocyte replication appeared to occur in the surrounding intact cartilage. The findings sug

In brief: The author saw 16 dancers with a history of dance-related knee injuries that had defied conservative treatment. Arthroscopic examination revealed 15 meniscal tears and four cases of chondromalacia patellae. The meniscal tears were treated by partial arthroscopic meniscectomy and the chondromalacia patellae by shaving, drilling, and debridement of the patella. After surgery the results were rated by means of a physical examination and a patient questionnaire. The results were generally excellent, with 13 of the 16 dancers returning to a preoperative level of dance activity. However, t

In surgery of the knee, combining CO2 laser and arthroscopy provides the benefits of both techniques: work at a distance from the lesion and bleeding control for the laser, patient's comfort and functional recovery for the arthroscopy. This original method requires a CO2 laser of at least 40 watts, CO2 as gas for arthroscopy and the appropriate manual instruments. Forty patients totalling 52 lesions were operated upon by this method with 11 initial failures and 5 complications: 1 case of synovial fluid leakage due to skin burn, 2 cases of phlebitis and 2 cases of spontaneously regressive fever

In brief: Concurrent with the renaissance in bicycling has come an increase in associated injuries. The most common problem the author sees in his practice is knee pain-usually chondromalacia patellae, patellar tendinitis, or bursitis. The patellofemoral joint is under particular stress in bicycling, and many new recreational riders make two errors: setting the saddle too low and the gears too high. These mistakes cause excessive pressure on the patellofemoral joint. Obtaining the best-fitting bicycle and saddle, adjusting the saddle properly, and using lower gears will help prevent such probl

Anterior displacement of the tibial tubercle is a well-accepted orthopedic procedure in the treatment of certain patellofemoral disorders. The radiologic appearance of surgical procedures utilizing the Maquet principle has not been described in the radiologic literature. Familiarity with the physiologic and biochemical basis for the procedure and its postoperative appearance is necessary for appropriate roentgenographic evaluation and the radiographic recognition of complications.

Patellofemoral arthralgia is a very common syndrome affecting athletes. Most often, examination fails to define true pathology. Conservative treatment, an active exercise program, and sports may be undertaken without harm to the knee. The patellofemoral arthralgia syndrome must be differentiated from true chondromalacia patella, where there is actual degeneration of the patella's articular cartilage, and from other sources of internal derangement such as meniscal disease or osteochondral lesions. Careful attention to the history of onset, and provoking activities such as climbing stairs, kneel

The mechanism of patellar pain is still not fully understood. Many operative procedures have been proposed, but so far no single technique has been successful. Under the impression that some of the pain-relieving effect by operative treatment is due to partial denervation of the patella, a medial denervation of the patella was performed in 17 patients (20 knees), mean age 24 years, with patellar pain syndrome. At the follow-up only 11 knees were free of symptoms or had improved, and the authors conclude that though comparable with other surgical methods, medial neurotomy is not sufficient to e

We present a 9-year-old boy, the product of a consanguineous marriage, with proteinuria, edema, and microscopic hematuria with mild renal impairment since age 2 yr. Renal biopsy showed the histopathologic electron microscopic changes seen in hereditary osteo-onchyodysplasia (HOOD) [Hoyer et al, 1972; Bennet et al, 1973; DelPozo and Lapp 1970; Vernier et al, 1974; Morita et al, 1973] or "nail-patella syndrome." Neither the patient nor his relatives had bone or nail abnormalities; however, 2 other sibs had died of complications of end-stage renal disease at age 6 and 9, leading us to suspect aut

A variety of names has been given to disorders of the inferior pole of the patella occurring in young athletic individuals and several different causes have been proposed for these disorders. Occasionally a direct blow will cause fracture of the inferior pole of the patella, but the only other mechanism which seems to be responsible is subluxation or dislocation. Ten cases of distal patellar pole fracture secondary to dislocation or subluxation of the patella are reported. From these cases and an extensive review of the literature it is concluded that patellar subluxation or dislocation is the

Frequency of rare skeletal lesions in Paget's disease is compared with normal localisations. Radiological morphology of rare localisations of Paget's disease of the scapula, of the lower arm bones, of the hand bones, of the patella and the calcaneus are described. Thickening of trabeculae in the spongy bone and porosity of compact bone, deformation and enlargement of the bones are typical; this has to be differentiated and related to a small number of other bone diseases.

90 patients affected by knee pains underwent CT study of the femoropatellar joint as an adjunct to double contrast arthrography. CT findings proved to be useful for optimal evaluation of patellar malpositions and especially in the diagnosis of chondromalacia patellae.

The increase in strenuous sports participation among adult Americans has led to a high incidence of overuse syndromes. Common running-related problems include iliotibial tract tendinitis, chondromalacia patellae, "shinsplints," stress fractures and various heel and foot syndromes. Most causes of overuse syndromes can be traced to training errors, anatomic factors, poor shoes and uneven running surfaces. These syndromes generally respond to modification of the exercise program, anti-inflammatory drugs and orthotic correction of lower extremity malalignment.

A simple procedure of using a Smillie knife to release the lateral patellar retinaculum and the capsule in the unstable patella syndrome is described. The syndrome includes recurrent dislocation, recurrent subluxation and acute dislocation of the patella. Arthroscopy, to exclude any other pathology such as chondromalacia patellae, is performed before proceeding to closed lateral release. The results of this procedure are as good as those after more major surgical procedures. In a small percentage of patients a second operation such as tibial tubercle transfer or patellectomy may be necessary,

Chondromalacia patellae is a distinct clinical entity characterized by retropatellar pain that is associated with recognizable changes in the articular cartilage of the posterior surface of the patella. Several factors may be involved in the etiology, such as severe patella alta, trauma, and, in rare cases, abnormal patellar tracking. Clinical symptoms and signs are reliable in only 50% of cases, but measurable quadriceps wasting, palpable patellofemoral crepitus, and effusion are strongly suggestive. Diagnosis must be confirmed by arthroscopy or direct examination of the posterior surface of

Three hundred and nineteen patients who had chondromalacia patellae and persistent patellofemoral pain after six months of conservative management underwent arthroscopy and arthroscopic surgery. The results in four aetiological groups were reviewed at one year and five years after operation. Morbidity was minimal. Lavage produced early remission in all groups. Shaving offered a particular advantage in the post-traumatic group. Lateral release plus shaving and lavage was beneficial in the group with maltracking patellae and in half of the idiopathic group. In the group with unstable patellae, l

Patellofemoral disorders present a diagnostic and therapeutic challenge to the orthopedic surgeon. By the sequential application of such basic diagnostic tools as inspection, palpation, and various measurements to a patient in standing, sitting, supine, and prone positions, a reproducible, comprehensive physical examination of the patellofemoral joint and entire lower extremity may be achieved.
