Evaluation of the patient with patellofemoral complaints requires a comprehensive physical and radiologic investigation. The tangential view is most helpful in the evaluation of such patellofemoral disorders as chondromalacia, lateral patellar compression syndrome, and recurrent subluxation or dislocation. Commonly utilized tangential views include those described by Hughston, Merchant, Laurin, and Ficat. Isolated lateral retinacular release was performed on 102 knees in 76 patients. Diagnoses included chondromalacia patellae, patellar compression syndrome, and subluxation and dislocation of the patella. Patients reported that their symptoms improved after isolated lateral retinacular release in 95 of 102 knees. A retrospective review of all patients diagnosed at arthroscopy as having a medial patellar plica with no other intraarticular pathology was conducted. Characteristic clinical findings, including pseudolocking, were identified. The clinical response of 68 patients (78 knees) to shelf resection was noted at 3 and 6 months. The 6-month review demonstrated 60% to be totally relieved of their preoperative pain and 31% with occasional discomfort, but capable of full activities. In 60 autopsy knee joints findings on morphology and topography of medio- and suprapatellar plicae and level of insertion of the medial vastus muscle in relation to localization and degree of severity of chondromalacia were determined. Thirty-six patients with patellofemoral disorders treated with various operative procedures were evaluated after an average follow-up period of 29 (6 to 72) months. Instability of the patella, either recurrent dislocation or subluxation, was present preoperatively in 27 knees (Group A). Twelve patients (Group B) were operated on for chondromalacia, which was not combined with a history of instability. Good results were obtained in 15 cases in Group A and in only three cases in Group B. Simple procedures, lateral release alone and medial duplication alone appeared to be ineffective methods.

A case is presented with several total dislocations of patella after an operative lateral retinacular release for chondromalacia patellae. The patient, a 27-year-old woman, had no history of preoperative luxations or subluxations. She had, however, a Q-angle of 20 degrees--a fact that may have been significant in the pathogenesis.

A lateral retinacular release performed through a 2 ½-3 cm lateral parapatella incision can give rewarding results for patients with chondromalacia, degenerative joint disease, recurrent subluxation or recurrent dislocation of the patella. The factors predisposing to recurrent dislocation and the results of conservative treatment were investigated in 79 cases of patellar dislocation. This paper presents 30 cases in which problems related to internal derangement of the knee were found on arthroscopy to be due to abnormalities of the synovium. These patients were seen over a period of three years and represented 14% of those whose symptoms were thought to be due to chondromalacia patellae or damage to the medial meniscus. Correlation of the symptoms produced with the synovial abnormality allows the syndrome to be defined and identified.

Sixty seven patients with pain in the knee were studied. A compound tomography (CT) score indicating chondromalacia was devised, based on the results of CT after arthrography. This score takes account of the regularity, the congruity, and the imbibition of contrast material.
