Anterior displacement of the tibial tuberosity in chondromalacia patellae.

Sixty-eight knees in 62 patients wiht degenerative changes of the patellar joint surface, chondromalacia or arthrosis, were operated on with anterior displacement of the tibial tuberosity. Complications were encountered in no less than 22 joints and the results appeared to be somewhat related to these complications, particularly in women, even if a direct relationship between complications and remaining symptoms could not be demonstated. In a follow-up examination an average of 13 months after operation, approximately 90 per cent of the patients had improved, men as well as women. In an additi

Osteochondritis occurring at multiple sites.

Five patients with osteochondritis juvenalis occurring at multiple sites are presented. In all cases at least one hip was affected, one child developing symptoms at the age of 8 months. Other sites included the tarsal navicular, the patella and the calcaneal apophysis. The relevance of these observations to the aetiology of coxa plana is discussed.

Intra-osseous venography in patellofemoral disorders. A preliminary report.

We have performed intra-osseous venography on 22 patients with chondromalacia patellae or patellofemoral osteoarthritis. Venous engorgement of the patella was demonstrated in both conditions regardless of the severity of the pathological changes.

Anterior displacement of tibial tuberosity for patellofemoral disorders: a preliminary report.

Thirteen patients with chondromalacia Stages 1, 2, and 3 and with patello-femoral osteoarthritis were treated by anterior displacement of the tibial tuberosity and followed for 22 to 33 months. Rated on a scale of objective anatomic and clinical criteria, the results were satisfactory in 85% of the patients. These results corroborate reports of other authors concerning this operation.

The place of arthroscopy in the diagnosis and treatment of internal derangement of the knee: an analysis of 1000 cases.

An analysis of 1,000 consecutive arthroscopies of the knee, representing almost 10 years' experience, has made it possible to identify the patient in whom arthroscopy is deemed essential, very useful, or in some instances, superfluous. In the latter category are the young, often teenagers, with pain as their main symptom and no positive physical findings. In osteochondritis dissecans, added information beyond that supplied by X-rays was obtained infrequently, and routine arthroscopy for chondromalacia of the patella is probably unnecessary. Pain and other symptoms in the older age group are mo

[The patella and the femoro-patellar joint during rheumatoid polyarthritis].

The authors report 2 series of cases of rheumatoid arthritis, one prospective of 115 cases, the other retrospective of 72 cases, and note the frequency of clinical and radiological patellar and femoro-patellar involvement during this disease. Signs of active rheumatoid disease in the patella were present in 31 cases. The most common lesions of the femoro-patellar joint space are narrowing and lateral dislocation of the patella. The femoro-patellar lesions evolve in parallel to the femoro-tibial lesions, but dissociation is possible. The early detection of a femoro-patellar syndrome permits eff

Chondromalacia patellae in athletes. Clinical presentation and conservative management.

A comprehensive conservative management program for chondromalacia patella in athletes has been presented. The clinical manifestations and the efficacy of conservative treatment have been documented in the prospective study of 100 consecutive athletes. The over-all success rate was 82%, and 18% were considered to be failures of conservative treatment. Only 8 of these 100 athletes have required surgical treatment. It was concluded that this type of conservative management is effective in the majority of athletes with symptomatic patellofemoral syndromes, and that surgical treatment should be ne

[Surgical treatment of chondromalacia patellae according to Maquet/Bandi--indication, technique, results (author's transl)].

Degenerative changes of the femoropatellar joint are being observed increasingly more often. These may be due to various disturbances of the gliding movement and gliding plane. The article describes the clinical and x-ray symptoms and the therapy; in most cases, additional surgery will be necessary besides antepositioning according to the technique described by Bandi.--Follow-up examinations of 32 patients showed that in 28 of 35 surgically treated knee joints, a positive influence had been exercised on the complaints, load capacity and walking distance.

[Chondromalacia patellae--arthrographic observations on the genesis and diagnosis of cartilaginous damage (author's transl)].

Thanks to recent advances in arthrography, diagnosis of cartilaginous lesions has increasingly become a task of the radiologist. An accurate assessment of the cartilage of the femoropatellar joint can be established via double-contrast technique and so-called "défilé" projections, whereas the axial projection of the patella without contrast medium shows only secondary arthrotic changes of bone and is unsuitable for demonstrating early cartilaginous damage. The classification of dysplastic patellae into different types according to Wiberg and Baumgartl yields a statistical correlation with the

Chondromalacia Patellae.

Chondromalacia patellae can occur when structure or activity disrupts the delicate balance between the quadriceps and the patella. Dr. Levine discusses etiology, diagnosis, and treatment.

The tangential x-ray investigation of the patellofemoral joint: x-ray technique, diagnostic criteria and their interpretation.

The X-ray technique for the tangential visualization of the patellofemoral joint is described; 3 X-ray signs (patellofemoral angle, patellofemoral index and patellofemoral displacement) are proposed as diagnostic aids in cases of subluxation of the patella and chondromalacia patellae. Theoretically, the pathogenesis of chondromalacia patellae and patellofemoral osteoarthritis may be explained as manifestations of cartilage damage secondary to lateral patellofemoral hyperpressure and medial patellofemoral hyperpressure.

Chondromalacia patellae: a system of classification.

Degeneration of the cartilage on the under surface of the patella is shown to be the most frequent cause of "internal derangement of the knee" (63%). The axial view of the patellofemoral joint in 4 angles of flexion constitutes the procedure of choice for diagnostic exploration of patellar cartilage with, perhaps, the restriction for the medial compartment where arthroscopy is probably preferrable. The information obtained from single contrast arthrography of the patellofemoral joint has contributed greatly to the understanding of the sometimes confusing problem of chondromalacia of the patell
