In young people complaining of patellar pain there is often an anatomic basis (an increased quadriceps angle or a high-riding patella). Forty-eight knees with one or other of these abnormalities underwent proximal quadriceps realignment. The results were excellent or good in 94%. Shaving of chondromalacic cartilage was also done in 11 knees but did not seem to improve the results, and is not now recommended except for "blister" lesions. Extensor mechanism dysplasia is an etiologically correct name for the disorder and therefore preferable to "chondromalacia patellae."

Accumulated data from many sources seem to indicate that prosthetic resurfacing of the patella may be an acceptable alternative to patellectomy in the treatment of chondromalacia patellae or patellofemoral arthrosis. A patellar prosthesis which is made of cobalt-chromium alloy is attached to a remnant of the original patella by means of the acrylic cement, polymethyl methacrylate. A new radiological technique is presented in which serial axial radiographs of the patellofemoral joints are taken under conditions in which the muscles about the knee and hip are contracted in a manner similar to that during weight-bearing. The spectrum of patellofemoral disorders ranging from chondromalacia of the patella to degenerative arthritis is a difficult, often vexing problem. Many methods of treatment, both surgical and nonsurgical, have been used without uniform or predictable success. The infrapatella strap, a new concept in nonsurgical management of patellofemoral pain, can be effective in 77% of patients. Its simplicity and comfort, patients' acceptance, and the apparent lack of contraindications when it is worn as instructed justify further clinical trials.

Despite our current knowledge and accumulated information, orthopedic surgeons often experience difficulties in the diagnosis of internal derangement of the knee. The well-known clinical and radiological findings of idiopathic osteolysis (Hajdu-Cheney) are described in a 17-year old man. The Insall-Salvati method of determining the position of the patella by radiography was employed in fifty-one patients with chondromalacia patellae that had been proved at arthrotomy. There was no definite relationship between chondromalacia patellae and patella alta. Four methods of surgical treatment of chondromalacia patellae have been evaluated after periods ranging from two to thirty years (average seven years), to discover the success rate, complications and indications for each. A total of 140 operations had been performed in 98 patients. A condition causing knee pain with retropatellar crepitus similar to chondromalacia patellae is described. The symptoms seem to be due not to chondromalacia of the patella, but to a local synovial thickening proximal to the articular surface of the medial femoral condyle. Thirteen patients, aged 10 to 21 years, were operated upon because of this condition, three of them bilaterally. In most of them the changed synovial membrane was excised. The operation in eleven of the knees was followed by considerable or complete relief. Of the remaining five knees, three were temporarily improved.

The articular cartilage of the patella was studied in 100 knees at necropsy. In twenty-one of these knees the cartilage changes were related to the trabecular architecture of the underlying bone. The content and the synthesis of nucleic acids in chondromalacian, osteoarthritis and normal cartilage was compared. The chondromalacian cartilage differed from osteoarthritis in that the content of nucleic acids was less. Also, the cell density was less in chondromalacian than in normal cartilage as opposed to previous findings in osteoarthritis. The synthesis of DNA was greater in chondromalacian than in normal cartilage but less than in osteoarthritis. With regard to the RNA synthesis, however, the chondromalacian cartilage showed a higher rate than both normal and osteoarthritic cartilage.

Chondromalacia patellae is difficult to diagnosis clinically with accuracy. In order to clarify the relevant symptoms and signs, 78 patients presenting with a clinical diagnosis of chondromalacia were examined by arthroscopy. In 49% of the knees no abnormalities were found. The Warkany syndrome is characterized by mental retardation, relatively specific facies, absent or dysplastic patellas, joint contractures, plantar/palmar furrows, distinctively abnormal toe posture, vertebral anomalies, narrow pelvis, ureteral-renal anomalies, or other abnormalities. There is no association with advanced parental age, and birthweight and length are usually commensurate with gestational age. In spite of continuing progress, total knee arthroplasty still presents many problems. In this clinic 25 arthroplasties have been performed in 22 patients over the last 4 1/2 years. Guépar hinges were used in 21 knees and Marmor modular knees in 4. Acrylic cement was used in all cases.
